Upon arrival, I met with Shannon Keyes, NC Pre-K Teacher. I stated the purpose of my visit. The facility operates two (2) NC Pre-K classrooms located inside of a public school. I conducted a walk-through of both NC Pre-K classrooms. The facility operates with a 5-star license. The facility had a 94% compliance history prior to this visit. The NC Pre-K monitoring was completed today. There are 34 children currently enrolled and 29 were present during the visit. I observed the children present engaged in the following activities: large group teacher-directed activities, free-play in activity areas, lunch, and bathroom/handwashing routines. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used to note the requirements monitored today. Prior to the visit I sent an email requesting the staff and training worksheets. The worksheets were received prior to the visit. The facility has a new administrator, the file of the new administrator was monitored today. One (1) new classroom staff has been hired since the last monitoring visit. The file of the new classroom staff member was monitored today. One (1) existing staff file was monitored. Three (3) children's records were selected for review. All staff have valid qualifying letters. All staff with the exception of the one (1) new classroom staff member have valid CPR/FA certification. Staff information was monitored using the staff records verification form dated August 2024. The last fire inspection was conducted 18/27/25 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 10/2/2025. The facility received a Superior rating with 8 demerits. The centers Emergency Drill Log report form was reviewed during the visit and all drills were not completed as required. The last shelter-in-place drill was conducted on 3/3/26. Fire drills have been conducted and documented monthly as required. The last fire drill was conducted today during the visit, 3/31/26. The monthly outdoor inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, first aid poster, menu, applicable staff/child ratio charts, tobacco free signage, mat lists, and evacuation routes. Transportation was monitored using the DPI verification form. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, program records and management. I reviewed the required paperwork for the star rated license renewal through Pathway 2 with Ms. Keyes. The facility had an outreach assessment conducted on January 15, 2026. The facility has completed the CQI plan and is working towards completing the set goal. CQI plans for each staff member were completed and progress made towards attaining the professional goal will be noted on the plans. Documentation of Family and Community Engagement activities/opportunities was observed in a pathways to the stars binder. The completed application and additional documents will be emailed. The staff information and education worksheets were received prior to this visit. NCRLAP completed the ECERS assessment at the facility on 3/30/26. The NC Pre-K monitoring was also completed today. This NC Pre-K site serves thirty-four (34) children. The current NC Pre-K staff were the same staff indicated in the NC Pre-K Plan System. The facility uses the Creative Curriculum with Smart Tech by Teaching Strategies. On-going developmental assessments/checkpoints are conducted throughout the school year. The tool used assesses the children in social, physical, cognitive language development, literacy, math, arts, social studies and science. The information gathered from the assessments are used to prepare information that is shared with parents during parent/teacher conferences that are held two (2) times per year. Both parent/teacher conferences have been held for the school year. During these conferences a developmental and learning report is shared with the parent. The facility is meeting the 1:9 staff/child ratio requirements and the space #1 with developmental day students is meeting the 1:6 staff/child ratio requirement. The classrooms operate from 7:45am until 2:30pm to meet the required 32.5 hours per week requirement. Parents receive communication daily/weekly via Class Dojo and Ready Rosie. Family engagement activities occur monthly. Parents are encouraged to engage in their child's education and asked to complete monthly take home activities with their child. The following activity centers are present in the classrooms: books/literacy, dramatic play, blocks, art, writing, science, music, sand/water, and manipulatives. There were two (2) violations of child care requirements observed during today's visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A. Bittle hired 9/22/25 did not have First Aid certification. .1102(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter-in-place or lockdown drill was completed in August 2025 and again on 3/3/26. .0604(u);.0302(d)(8) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 4/14/2026. You are required to maintain compliance with all applicable childcare rules and regulations at all times. Upon arrival, I met with Darlene Estridge, NC Pre-K Teacher. I stated the purpose of my visit. Upon arrival space #2 was out of compliance with staff-child ratios. There was one staff, D. Estridge, present with sixteen (16) children. The assistant teacher arrived in the classroom a few minutes after my arrival. The facility operates two (2) NC Pre-K classrooms located inside of a public school. I conducted a walk-through of both NC Pre-K classrooms. The facility operates with a 5-star license with 7 points in program standards, 7 points in education and 1 quality point. The license was issued 11/13/19. The facility had a 89% compliance history prior to this visit. The NC Pre-K monitoring was completed today. There are 33 children currently enrolled and 32 were present during the visit. I observed the children present engaged in the following activities: free-play in activity areas, transitioning from lunch to the classroom, and bathroom/handwashing routines. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used to note the requirements monitored today. Prior to the visit I sent an email requesting the staff and training worksheets to Theresa Gardner, Principal. The worksheets were received prior to the visit. A substitute teacher was present filling the spot of the assistant teacher during this visit. The substitute teacher's information was added to the staff and training worksheet by the Child Care Consultant during this visit. Staff and children's records were reviewed during the visit. I selected four (4) children's records for review. I reviewed the file for one (1) staff, no new staff have been hired since the last monitoring visit. All staff have valid qualifying letters and valid CPR/FA certification. Staff information was monitored using the staff records verification form dated August 2024. The last fire inspection was conducted 9/17/24 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 1/14/2025. The last annual compliance visit was conducted on 4/27/23. The 18-month compliance history prior to this visit was 88%. The facility operates two (2) NC Pre-K classroom located within a public-school setting. A walkthrough of the indoor spaces used by children was completed and monitored for the following: supervision, staff/child ratios, CPR and First Aid certification, storage of hazardous products and medication, adequate and approved space and program records. The staff and training worksheet from the last annual compliance visit was used to monitor for current required information for the existing staff. One (1) new staff have been hired since the last monitoring visit. A staff and training worksheet containing the information for these new staff was received prior to today’s visit. The file for the new staff member was monitored today. All staff have current criminal background check qualifying letters on file. The last documented fire drill was conducted on 10/30/24. A shelter-in-place drill was completed on 8/30/23. A superior sanitation inspection was completed on 9/13/24 with 0 demerits. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan, and medications. The following activity centers were present in the classroom: books, blocks, manipulatives, dramatic play, science, art, writing and music. During the visit, I observed the children in space #1 engaged in free play in activity areas, and handwashing/bathroom routines. The teachers interacted with children in activity centers in a positive manner and assisted when needed. All interactions and language observed were positive. In space #2 I observed the children engaged in free play in activity centers, bathroom/handwashing routines, large gross motor activities, and transitioning to the outdoor learning environment. The interactions and language between the teachers and children in care were positive. The children were assisted when needed. Upon arrival, I met with Shannon Keyes, NC Pre-K Teacher. I told her the purpose of my visit. The facility operates two (2) NC Pre-K classrooms located inside of a public school. I conducted a walk-through of both NC Pre-K classrooms. The facility operates with a 5-star license with 7 points in program standards, 7 points in education and 1 quality point. The license was issued 11/13/19. The facility had a 96% compliance history prior to this visit. The NC Pre-K monitoring was completed today. There are 33 children currently enrolled and 32 were present during the visit. I observed the children present engaged in the following activities: free-play in centers, lunch and bathroom/handwashing routines. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used to note the requirements monitored today. Prior to the visit I sent an email requesting the staff and training worksheets to Theresa Gardner, Principal. The worksheets were received prior to the visit. Staff and children's records were reviewed during the visit. I selected three (3) children's records for review. I reviewed the file for one (1) staff, no new staff have been hired since the last monitoring visit. Staff information was monitored using the staff records verification form dated 9/8/23. The last fire inspection was conducted 9/25/23 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 1/30/2024. The facility received a Superior rating with 2 demerits. The centers Emergency Drill Log report form was reviewed during the visit and all drills were not completed as required. A shelter-in-place or lockdown drill was conducted in August 2023 and the next shelter-in-place or lockdown drill was conducted in February 2024. Fire drills have been conducted and documented monthly as required. The monthly outdoor inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, mat lists, and evacuation routes. Transportation was monitored using the DPI verification form. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, program records and management. The NC Pre-K monitoring was also completed today. This NC Pre-K site serves thirty-three (33) children. The current NC Pre-K staff were the same staff indicated in the NC Pre-K Plan System. The facility uses the Creative Curriculum with Teaching Strategies Gold. On-going developmental assessments/checkpoints are conducted throughout the school year. The tool used assesses the children in social, physical, cognitive language development, literacy, math, arts, social studies and science. The information gathered from the assessments are used to prepare information that is shared with parents during parent/teacher conferences that are held two (2) times per year. Both parent/teacher conferences have been held for the school year. During these conferences a developmental and learning report is shared with the parent. The facility is meeting the 1:9 staff/child ratio requirements. The classrooms operate from 7:45am until 2:30pm to meet the required 32.5 hours per week requirement. Parents receive communication weekly via Class Dojo. Family engagement activities occur monthly. Parents are encouraged to engage in their child's education and asked to complete monthly take home activities with their child. The following activity centers are present in the classrooms: books/literacy, dramatic play, blocks, art, writing, science, music, sand/water, and manipulatives. There were six (6) violations of child care requirements observed during today's visit. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. During the visit it was observed that a current schedule was not posted in space #1. GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference. GS 110-91(12); .0508(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. E. Rush has not completed First Aid trainnig. .1102(c) 1301 Center did not maintain a record of daily attendance Attendance was not completed for April 17, 2024 or April 18, 2024. GS 110-91(9) 1329 Application for enrollment did not include all required information. Children's files monitored did not contain information regarding particular fears, allergies and symptoms, unique behavior characteristics, and health care concerns. .0801(a)(1-7) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter-in-place or lockdown drill was conducted in August 2023 and the next shelter-in-place or lockdown drill was conducted in February 2024. .0604(u);.0302(d)(8) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 5/7/24. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history was 96% prior to this visit. Violations cited during the visit may negatively affect this score. Failure to maintain compliance with our requirements may result in administrative action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. The last annual compliance visit was conducted on 4/27/23. The facility operates two (2) NC Pre-K classroom located within a public-school setting. A walkthrough of the indoor space used by children was completed and monitored for the following: supervision, staff/child ratios, CPR and First Aid certification, storage of hazardous products and medication, adequate and approved space and program records. The staff and training worksheet from the annual compliance visit was used to monitor for current required information for the existing staff. Two (2) new staff have been hired since the last monitoring visit. A staff and training worksheet containing the information for these new staff was received during today’s visit. All staff have current criminal background check qualifying letters on file. A fire drill was conducted on 9/29/23. A shelter-in-place drill was completed on 8/30/23. A superior sanitation inspection was completed on 1/26/23 with 10 demerits. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan. The following activity centers were present in the classroom: books, blocks, manipulatives, dramatic play, science, art, writing and music. During the visit, I observed the children in space #1 transitioning from free play in activity areas to handwashing/bathroom routines in preparation for lunch. The teachers encouraged the children to clean up in a positive manner and assisted when needed. All interactions and language observed were positive. In space #2 I observed the children engaged in free play in activity centers and a small group of children were sitting at a table working with one (1) of the teachers. The children were observed completing bathroom/handwashing routines and transitioning to lunch. The interactions and language between the teachers and children in care were positive. The children were assisted when needed. The violations of childcare requirements observed during the visit are below. This requirement was not completed with H.
