There are concerns that: staff/child ratios are not being followed. Upon arrival, I was greeted by Carla Callahan, Owner. I shared the reason for the visit with Mrs. Callahan. I conducted a walk-through of the facility. During the visit, I observed supervision, staff/child ratio, adequate/approved space and permit restrictions. There were 61 children in attendance during the visit. Findings: I observed eleven (11) 1–2-year-old children in space #5 with two (2) teachers. In space #1 I observed ten (10) children aged 3-4 years old with one (1) teacher. In space #2 I observed five (5) 1-year-old children with one (1) teacher. I observed eighteen (18) children aged 2-3 years old in space #4 with two teachers. Four (4) infants were present in space #3 with one (1) teacher and thirteen (13) children aged 3-5 years old were observed in space #6 with two teachers. All spaces were following staff/child ratios during today’s visit. All staff were interviewed today regarding staff/child ratios. Four (4) staff including the administrator stated that the facility has been out of staff/child ratio in one (1) classroom. The administrator stated that on January 6, 2026, space #1 had two (2) teachers with eleven (11) 3–4-year-old children. One staff member left the classroom to serve lunch leaving the classroom out of staff/child ratio. When it was realized that the class was out of staff/child ratio a child was moved from space #1 to space #6. Attendance logs were reviewed during today’s visit. Based on interviews with staff the allegation that staff/child ratios are not followed is confirmed. One (1) violation of child care requirements was cited. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. It was confirmed that on 1/6/26, space #1 was not in compliance with staff/child ratios. One (1) teacher was left with eleven (11) 3-4 year-old children. 10A NCAC 09 .2818 The violation documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining how you have corrected the violations described above, and how you will prevent these violations in the future. The letter should list each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 2/12/26. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during the visit may negatively affect this score. Failure to maintain compliance with our requirements may result in administrative action against your license, which could include civil penalties. I met with Carla Callahan, Owner, and stated the purpose of my visit. Prior to this visit the facility’s compliance history was 91%. I observed the children present engaged in the following activities: indoor free-play in activity areas, teacher-directed activities, lunch, and bathroom/handwashing routines. The program has a 3-star license issued August 1, 2023. The program earned four (4) points in education, two (2) points in program standards, and one (1) quality point. License restrictions: Daytime care only; Meets enhanced ratios. The Annual Compliance Monitoring Checklist for Child Care Centers was used to note the requirements monitored today. The staff and training worksheets were received prior the visit. Since the last monitoring visit three (3) new staff have been hired. The files of the new staff members and two (2) existing staff were monitored today. Additional information was completed on the staff and training worksheet during the visit by the child care consultant these changes were initialed. Seven (7) children's records were monitored. The last fire inspection was conducted September 18, 2025, and the facility was found to be Satisfactory and approved for daytime care only. The last sanitation inspection was conducted on 3/24/25. The facility received a Superior rating with 6 demerits. The centers emergency drill log report form was reviewed during the visit. Emergency drills have been documented as required. The last fire drill was conducted on 10/16/25 and the last shelter-in-place drill was conducted on 10/16/25. Playground safety inspections were reviewed during the visit, monthly inspections were documented as required. This facility does not provide transportation. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, medication administration, program records, and management. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, activity plans, menu, infant feeding schedules, applicable staff/child ratio charts, tobacco free signage, First Aid poster and evacuation routes. The new QRIS system and pathways to the stars were discussed during this visit with administrator, C, Callahan. We discussed pathways 1 and 2 in detail. We reviewed the documents listed under the QRIS modernization tab located on the DCDEE website and the application for a rated license. We discussed CEU course availability. A timeframe for completion was discussed and documented on the Pathway discussion form and a signed copy of the form was provided during the visit. Information on the new section .3200 of the child care rules was shared during this visit. Four (4) violations of child care requirements were observed during the visit and documented below. Violation Number Comment Rule 1329 Application for enrollment did not include all required information. Two (2) of seven (7) children's files monitored during the visit did not have the section of the application completed that includes particular fears the child may have, any allergies and symptoms the child may have, unique behavior characteristics, etc. .0801(a)(1-7) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A staff member in space #6 had two (2) cups from Starbucks with beverage that was being consumed by the staff member. .0901(i) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR Plan for the facility was last updated on 10/3/24. .0607(e) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The surfacing under the stationary equipment on the outdoor learning environment was not the correct depth. .0605(k)(1-4) The violations documented during the visit must be corrected immediately. The facility operates with a 3-star license earning 2 points in program standards, 4 points in staff education and 1 quality point. The facility has a current compliance history of 93%. There were 68 children present today. The children were observed during rest time. The last annual compliance visit was conducted on 10/22/24. A superior sanitation inspection was completed on 3/24/25. A fire drill was last conducted on 5/25/25. A shelter in place drill was last conducted on 4/15/25. Playground Inspections have been conducted as required. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, daily schedules, first aid poster, applicable ratio charts, emergency medical care plan, emergency telephone numbers, ITS-SIDS safe sleep policy, evacuation routes, no smoking signage, cot lists, and menus. The following items were monitored today: supervision, staff/child ratios, storage of hazardous products and medications, adequate/approved space, staff files, program files, posted license and license restrictions. I observed children in space #5 resting on their cots. The teachers were supervising the children. All interactions and language observed were positive. I observed children in space #4 sleeping. The staff were supervising the children. I observed children in space #3 sleeping. The teacher was observed holding and talking to one (1) child that was awake. The teacher was nurturing to the child and used a pleasant tone when speaking to the child. There were four (4) violations of child care requirements observed during today’s visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Additional information can be found in the Raise North Carolina Newsletter sent via email on 2/28/25 from DCDEE. As of today, the website indicates that your facility has completed all sections for testing for lead in water, testing of lead-based paint and asbestos. Resources and Reminders: *The North Carolina Child Care Health and Safety Resource Center is a great resource for information and free child care publications/posters. The website is healthychildcare.unc.edu. *Please ensure that all on-going training hours are being completed and documented as required for yourself and all staff members and that any annual documents are being updated and completed. These items will be monitored for completion at your annual compliance visit: - Staff Development Plans - Annual Evaluations for Staff - Annual Medical Forms (Health Questionnaire/Emergency Information) - Polices (Emergency Medical Care Plan, Emergency Preparedness and Response Plan, and any other policies that are amended) *All completed and signed incident reports need to be filed in each child's individual file. *The facility’s Emergency Preparedness and Response Plan (EPR) must reflect all current information regarding enrollment, contact numbers and any updates to your emergency plans. This plan requires at a minimum to be updated annually or when any changes occur. Upon arrival I spoke with Carla Callahan, Owner, about the purpose of my visit. The facility has a compliance history of 84%. 4 I observed the children present engaged in the following activities: indoor free-play, outdoor play, teacher-directed activities, lunch and bathroom/handwashing routines. The program operated with a 3-star license issued January 24, 2020. The program earned four (4) points in education two (2) points in program standards and one (1)quality point. License restriction: Daytime care only; Meets enhanced ratios. The Annual Compliance Monitoring Checklist for Child Care Centers was used to note the requirements monitored today. The staff and training worksheets were submitted prior the visit. Four (4) new staff were hired since the last monitoring visit. During today's visit I monitored the four (4) files of the new staff members and one (1) existing staff file. I selected ten (10) children's records for review. The last fire inspection was conducted October 18, 2023 and the facility was found to be Satisfactory and approved for daytime care only. The last sanitation inspection was conducted on 7/17/23. The facility received a Superior rating with 5 demerits. The centers emergency drill log report form was reviewed during the visit. Emergency drills were completed as required. The last fire drill was conducted on 10/20/23 and the last lockdown drill was conducted on 9/28/23. Playground safety inspections were reviewed during the visit, monthly inspections were documented as required. This facility does not provide transportation. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, medication administration, program records, and management. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, activity plans, menu, infant feeding schedules, applicable staff/child ratio charts, tobacco free signage, First Aid poster and evacuation routes. Twelve (12) violations of child care requirements were observed during the visit and documented below.
