Jennifer Garner, lead child care consultant, assisted with the visit. Your program currently operates with a Five Star Center License issued April 26, 2022. Restrictions on the license include a first shift capacity of fifty-four (54) children ages three through 6, meets enhanced ratios, meets enhanced space, and meets reduced enhanced ratios (minus one). Restrictions were in compliance today. The last annual compliance visit was completed on February 17, 2025. The sanitation inspection was completed on November 19, 2025, with a “Superior” classification. The fire inspection was completed on August 21, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. Prior to the visit the program’s eighteen-month compliance history was ninety-five percent (95%). Administrator K. Hill and teachers K. Maynor and T. Mathis were present and available for consultation during the visit. There were thirty-nine (39) children enrolled and thirty-six (36) children present. There are five (5) indoor spaces and one (1) outdoor space approved for use. Two (2) of the five (5) approved indoor spaces were currently being used. Age-appropriate materials were accessible to all children and spaces were arranged into activity areas. Group 2 was observed in group time with color and letter sound activities, then transitioned to indoor free play using name tags, during which they used a variety of materials including a large dollhouse, books, puzzles, and music instruments. Group 5 was observed during outdoor play using gross motor materials such as wheeled toys and the slide. They also searched for examples of frost and ice throughout the playground. Supervision and staff-child ratios were observed and found to be in compliance. Lunch consisted of hot dog with chili, fish sticks, corn muffin, baked beans, corn, peaches, and milk. Staff and Training worksheet: A current and updated worksheet was not available. All staff files were reviewed and all documents and related dates were found to be in compliance. No later than Friday, January 23rd, 2026, you must complete, sign, and email me a copy of an updated and completed staff and training worksheet. Failure to do so may result in a follow up visit to ensure compliance. The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes were verified in compliance. Children attend the program from (7:45am-2:45pm), meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Brigance Screening, and TS Gold for their ongoing assessments. Developmental Screenings completed and Health Assessments were on file. A Site Monitoring visit has been conducted by Columbus County Schools, the contracting agency, during a site visit on November 14th, 2025. It was noted that all requirements were in compliance. Families are invited to an open house and ongoing communication occurs through regular newsletters, as well as a daily folder. In addition to regular communication, families are invited to special events including a Christmas program and a Guardian Gala. The following violation(s) were documented. Next, children in Space #100a traveled to the fenced in playground, where they used the stationary climbing structure, tricycles, scooters and push toys. Children in Space #100b were observed in a group time activity during which they listened to a book and did letter and color recognition activities. After group time, these children transitioned back to free choice activities. Lunch served in the classrooms consisted a choice of steak with gravy or chicken nuggets, whole grain roll, mashed potatoes, green beans, fresh oranges, and milk. Today it was observed that a new climber/slide combination shaped like a truck and a new rock climbing wall have been placed on the grassy area of the playground. Installation instructions could not be located, however stickers containing some instructions for installation and use were observed on the pieces of equipment themselves. Stickers on the truck shaped piece of equipment indicated that it should not be placed over hard ground such as grass. The last annual compliance visit was conducted March 7, 2023. A Superior sanitation rating was obtained January 30, 2024, and a satisfactory fire inspection was conducted September 27, 2023. Prior to today’s visit, the facility had an eighteen-month compliance history score of 97%. Administrator, Dr. K. Hill, Lead Teacher, J. Floyd and Teacher, C. Coleman, were present and available for consultation today. Space #100 is a large room divided into two separate and distinct spaces which are approved for use by children and used as NC Pre-K classrooms. The spaces are separated by a partial wall and conference/storage room in the middle. Upon arrival, children in Space #100b were observed in free choice activities including building buildings with blocks, playing in the dramatic play kitchen and working with playdough. In Space #100a, children were observed in a group time activity during which they listened to a book, discussed the letter “R,” and did a color recognition activity. At 9:36 A.M., the fire alarm went off and both groups evacuated the classrooms to the field behind the building. The staff and children appeared familiar with fire drill procedures. After the fire drill, children enrolled in Space #100a went to the fenced in playground at the other end of the school. They played in the sand table, and using riding toys and the climbing structure. Children in Space #100b continued free choice activities. At 10:40 A.M., staff members in each space read books to children and completed transition activities as they waited for lunch. The equipment on the grass outside of the enclosed area, such as the swings, slide and dome climber, could not be used by preschool children unless fall zones with resilient surfacing were created underneath and around the equipment. If the equipment is made inaccessible or unavailable to preschool children, the grassy field could be used for gross motor play with portable equipment such as riding toys, balls, and hula hoops. Upon arrival, children in Space #100b were observed in free choice activities including building buildings with blocks, playing in the dramatic play kitchen and working with playdough. In Space #100a, children were observed in a group time activity during which they listened to a book, discussed the letter “R,” and did a color recognition activity. At 9:36 A.M., the fire alarm went off and both groups evacuated the classrooms to the field behind the building. The staff and children appeared familiar with fire drill procedures. After the fire drill, children enrolled in Space #100a went to the fenced in playground at the other end of the school. They played in the sand table, and using riding toys and the climbing structure. Children in Space #100b continued free choice activities. Restrictions on the permit include a first shift capacity of 54 children 3-6 years of age, meets enhanced space and meets reduced enhanced ratios (minus one). Restrictions were in compliance today. The program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled; this program uses The Creative Curriculum, Guided Edition. The last annual compliance visit was conducted March 7, 2023. A Superior sanitation rating was obtained June 7, 2023, and a satisfactory fire inspection was conducted September 27, 2023. Prior to today’s visit, the facility had an eighteen-month compliance history score of 98%. Lead Teachers, J. Floyd and T. Mathis, were present and available for consultation today. Space #100 is a large room divided into two separate and distinct spaces which are approved for use by children and used as NC Pre-K classrooms. The spaces are separated by a partial wall and conference/storage room in the middle. Upon arrival, children in Space #100b were observed eating a special snack to celebrate a child’s birthday. Children sang “Happy Birthday” to the child and ate cupcakes with juice and cheese balls at the child-sized tables. In Space #100a, children were observed in toileting and hand washing routines. Next, both groups of children transitioned to rest time routines. They rested on appropriately spaced cots with individual linens. Lights were dimmed and soft music played. Two new staff members have been hired since the Annual Compliance visit. Files were monitored and in compliance today. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. One board on each of the wooden platforms of the climbing structure was completely detached from the rest of the platform. .0601(c) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. Action plans were not on file for one child with an allergy and another child with specialized health concerns. .0801(b) A compliance letter including detailed information about how all violations have been corrected must be received by October 12, 2023. Include any supporting documentation with your response (if applicable). Your letter will be used as verification that violations have been corrected and compliance is maintained; your written response is considered a legal document so must be accurate and truthful. If you email the compliance letter, it must be sent using the email address registered with DCDEE and include your facility name and ID number. If sufficient information is not received by the due date, a follow-up visit will be conducted. Technical Assistance Regarding Violations Cited For the safety of the children, all outdoor equipment must be sturdy and stable. Today I urged you to request repair of the two loose boards on the climbing structure as soon as possible. You may have to keep the equipment inaccessible until repairs can be made. Please describe how repairs were made or how equipment was made inaccessible in your compliance letter. As discussed today, for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional, and the purpose is to ensure you have all necessary information to meet the child’s health care needs. You may find sample copies of allergy action plans and medical action plans on the DCDEE website under Provider Documents and Forms. Please send a copy of the completed action plans for A. Patrick and M. Mabe to verify compliance. Technical Assistance Technical assistance was also provided today regarding topical ointments such as sunscreen. As discussed, written authorization is required from parents/guardians prior to administering any topical ointments, even when a doctor’s note is on file. You may find a blank permission to administer topical ointment form on the DCDEE website under Provider Documents and Forms as well. To help prevent potential injuries from tripping, I encouraged you today to spray paint the large, exposed roots on the playground as well as the concrete “outcrop” next to the slab where the picnic tables are located. You may wish to use a bright colored paint to help alert children to potential tripping hazards. Rated License Information Your facility’s three-year rated license reassessment was voluntarily conducted in April 2022, which means that your program has been assigned to “Cohort 3” in the recently announced “cohort model” of resuming reassessments. As discussed today, 7/1/25-6/30/26 will be your “planning/preparation” year, and your next reassessment will be required between 7/1/26-6/30/27. As you typically have the Early Childhood Environment Rating Scales- Revised (ECERS-R) conducted, we discussed that you will have the option to request an ECERS-R assessment during your planning year if you wish, or you may wait until your reassessment year. You may also wish to request an outreach assessment from NCRLAP this year. I will be in touch regarding timelines for reassessment as your planning year approaches. It was verified today that the facility is maintaining compliance with Program Standards points, Education points and the Quality point. Regarding DCDEE WORKS, new administrator K. Hill must register for a WORKS account, apply for a DPI Administrator position, and upload the appropriate form with her Principal’s License to complete evaluation. Please refer to the email sent August 1, 2023 with WORKS account registration instructions. Reminders On August 1, 2023 I emailed Dr. Hill to introduce myself and offer technical assistance and/or training on child care requirements. Please let me know if would like to meet virtually or in-person for an overview of Pre-K requirements as offered! Child Care Rules were revised July 1, 2023, and a new rule book was posted to the DCDEE website. As discussed, you should discard any old versions of the rules and ensure you are referring to the most current rule book when reviewing or researching requirements.
