You, Sharon Crank, Owner/Administrator, and Crystal Thurman, Assistant Director, assisted me with the visit today. The Secretary of State website was checked on January 21, 2026, and your business Unique Kids Childcare Academy Inc., is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The facility currently operates with a 5-star rated license issued on March 14, 2022, earning six (6) points in staff education, six (6) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, 2nd shift, overnight, meets enhanced space, meets enhanced ratios, and maximum of 15 children on 2nd shift, max 7 children on 3rd shift, and max 12 workers per septic tank restrictions. No children were enrolled in second and/or third shift. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of January 22, 2026. The last annual compliance visit was conducted on January 28, 2025. Your program was also monitored for compliance with implementing an approved curriculum Creative Curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Fire Inspection: The last fire inspection was completed on March 10, 2025. Sanitation Inspection: The last sanitation inspection was completed on April 8, 2025. A “superior” classification was issued with eight (8) demerits noted on the grade card. The last lockdown emergency drill was completed on October 31, 2025, at 11:55am and recorded. The last playground inspection was completed on January 5, 2026. The last fire drill was completed on January 5, 2026, at 8:45am and recorded. Each indoor and outdoor space was monitored. Children enrolled were observed during naptime, snack time, indoor free play, art activities, indoor free play, and bathroom and hand washing routines. Snack time was observed during today’s visit and included American cheese slices, round crackers, and milk. Files Reviewed: Four (4) children’s files were reviewed during today’s visit. One (1) new staff file was reviewed, and one (1) existing staff file was reviewed during today’s visit. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. A checklist was used to note the requirements I monitored today. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. In Space #4, one child twelve years of age did not have available for review a current travel authorization form. The travel authorization form had expired on June 2, 2024. .1003(i)(j) Technical Assistance Provided: Travel Authorization: I suggested that you review all children’s files who are currently being transported in the facility’s van for accuracy and that the travel authorization is current. I also suggested that you check children’s travel authorization forms prior to summer and when school returns in the Fall. The violation was corrected during the visit. Compliance History Information: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Consultation: -Provider Access to ABCMS I reviewed the ABCMS Portal Roster for your child care program on January 22, 2025, and you have linked staff to the facility’s ABCMS Portal page. Lead Testing: Child Care Rule .0601(f)- Safe Environments We discussed the test results that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your facility. The lead water testing results were completed on September 19, 2024. On January 21, 2026, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had the following results for lead-based paint testing, and it was determined that Unique Kids Child Care Academy Inc. was exempt from the lead-based paint assessment requirements. The facility provided the appropriate documentation to show all buildings were built after 1978 and no lead-based paint was used in order to meet the building age exemption criteria per 10A NCAC 41C.1004. The Clean Classrooms for Carolina Kids™ team determined that Unique Kids Child Care Academy Inc. was exempt from asbestos inspection requirements. The facility provided the appropriate documentation to show all buildings were built after 1988 to meet the building age exemption criteria per 10A NCAC 41C .1003. No additional action is required by the facility. Lead water testing must be completed every three years. I discussed with you the three different pathways available through the new QRIS System and the resources available for review on the DCDEE website. Information was discussed included the following: the Continuous Quality Improvement Plan for each staff and the program, Enhanced Education standards for the different staff positions, Family and Community Engagement Standards, curriculum, and approved Early Childhood Formative Assessments. I reviewed this information with you and Crystal Thurman and completed a QRIS Modernization: Pathways to the Stars document to determine which QRIS Pathway you chose for your facility. At this time the facility has determined that the QRIS Pathway#1: Program Assessment is what they will complete for their re-assessment. They plan to apply for the rated license by the end of Summer 2026. An updated DCDEE WORKS account is required for all staff to process the education standard for your program. Education will be evaluated for administrators and staff who provide direct care to children. Safe Environments/Supervision During Naptime: I discussed with you the child care requirements about providing a safe environment and supervision during naptime. I observed in Space #1 and Space #2 the naptime environment and found that a safe environment and supervision met current child care requirements -Moodle Support: The Division offers early childhood professionals a wide range of professional development opportunities through our online learning platform Moodle. As we continue expanding our training offerings, DCDEE has established a new email address and phone number for Moodle support. Transportation of children will begin on8/15/2025. Inspections/Drills: Fire Inspection: The last fire inspection was completed on March 10, 2025. Sanitation Inspection: The last sanitation inspection was completed on April 8, 2025. A “superior” classification was issued with eight (8) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill was completed on July 31, 2025, at 3:36pm and recorded. The last playground inspection was completed on August 8, 2025. The last fire drill was completed on August 4, 2025, at 9:17am and recorded. You stated two new staff has been hired since your most recent annual compliance visit conducted on January 28, 2025. Their file was reviewed and verified with the staff and training worksheet. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violation of child care requirements was observed and documented during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current fire inspection available for review was dated March 10, 2025. The previous fire inspection was completed on January 31, 2024. 10A NCAC 09 .0304(a) Technical Assistance regarding the violations observed during today’s visit: Fire Inspections: I discussed with you that fire inspections must be conducted prior to date of the last fire inspection. I suggested contacting your Fire Marshall’s /Fire Inspector’s office at least four to six weeks before your next fire inspection is due. I also suggested placing a reminder on a calendar of the due date. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-nine percent (99%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance letter by August 28, 2025. I discussed with you and staff during the visit today that the naptime classrooms are not allowed to be pitch dark and that lighting should be provided in the classroom to allow staff to supervise and/or offer assistance to a child if needed. I also discussed that naptime music should not be so loud that staff are unable to hear children in case assistance is needed. I reviewed with you the child care rules concerning safe environment and supervision. Child Care Rule References: 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS Children shall be adequately supervised at all times in child care centers. You, Sharon Crank, Owner/Administrator, assisted me with the visit today. The Secretary of State website was checked on July 24, 2025, and your business Unique Kids Childcare Academy Inc., is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The facility currently operates with a 5-star rated license issued on March 14, 2022, earning six (6) points in staff education, six (6) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, 2nd shift, overnight, meets enhanced space, meets enhanced ratios, and maximum of 15 children on 2nd shift, max 7 children on 3rd shift, and max 12 workers per septic tank restrictions. No children were enrolled on third shift. You stated that you have four children enrolled on second shift. Staff and children were observed during indoor free play, table toys, lunch, and naptime. A complete walkthrough of licensed indoor and outdoor spaces was conducted. I monitored the van for transportation and reviewed with you that you needed to updated the emergency information and transportation for each child transported. Transportation of children will begin on8/15/2025. Inspections/Drills: Fire Inspection: The last fire inspection was completed on March 10, 2025. Sanitation Inspection: The last sanitation inspection was completed on April 8, 2025. A “superior” classification was issued with eight (8) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill was completed on July 31, 2025, at 3:36pm and recorded. The last playground inspection was completed on August 8, 2025. The last fire drill was completed on August 4, 2025, at 9:17am and recorded. You stated two new staff has been hired since your most recent annual compliance visit conducted on January 28, 2025. Their file was reviewed and verified with the staff and training worksheet. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violation of child care requirements was observed and documented during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current fire inspection available for review was dated March 10, 2025. The previous fire inspection was completed on January 31, 2024. 10A NCAC 09 .0304(a) Technical Assistance regarding the violations observed during today’s visit: Fire Inspections: I discussed with you that fire inspections must be conducted prior to date of the last fire inspection. I suggested contacting your Fire Marshall’s /Fire Inspector’s office at least four to six weeks before your next fire inspection is due. I also suggested placing a reminder on a calendar of the due date. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-nine percent (99%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance letter by August 28, 2025. Consultation: -Resuming Star Rated License Reassessment Information For more information on the new QRIS system, please visit the DCDEE website, homepage click the What’s new Tab and click onto the QRIS System link. I discussed with you discuss the three different pathways available through the new QRIS System and the resources available for review on the DCDEE website. -Safe Environment/Supervision During Naptime: We discussed naptime classroom environment. I discussed with you and staff during the visit today that the naptime classrooms are not allowed to be pitch dark and that lighting should be provided in the classroom to allow staff to supervise and/or offer assistance to a child if needed. I also discussed that naptime music should not be so loud that staff are unable to hear children in case assistance is needed. I reviewed with you the child care rules concerning safe environment and supervision. Child Care Rule References: 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS Children shall be adequately supervised at all times in child care centers. Adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; 10A NCAC 09 .0601 SAFE ENVIRONMENT All child care centers shall provide a safe indoor and outdoor environment for the children in care. -Provider Access to ABCMS On August 14, 2025, I reviewed the information available on the ABCMS Portal System for your facility. The information available for review showed that you created a business account for your facility and had linked current staff to your ABCMS Portal account. I reviewed the roster to the staff and training worksheet you had available for review for accuracy, and it was correct. I reminded you to update the ABCMS Portal if new staff are hired or if staff are terminated. Lead Testing: Child Care Rule .0601(f)- Safe Environments We discussed the test results that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your facility. The lead water testing results were completed on September 19, 2024. On July 24, 2025, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had the following results for lead-based paint testing, and it was determined that Unique Kids Child Care Academy Inc. was exempt from the lead-based paint assessment requirements. The facility provided the appropriate documentation to show all buildings were built after 1978 and no lead-based paint was used in order to meet the building age exemption criteria per 10A NCAC 41C.1004. The Clean Classrooms for Carolina Kids™ team determined that Unique Kids Child Care Academy Inc. was exempt from asbestos inspection requirements. The facility provided the appropriate documentation to show all buildings were built after 1988 to meet the building age exemption criteria per 10A NCAC 41C .1003. No additional action is required by the facility. Lead water testing must be completed every three years. The sanitation inspection was not completed within one year of the last sanitation inspection. 10A NCAC 09 .0304(b) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #1 on September 19, 2024, the sleep position was not documented for one child enrolled on December 14, 2023, at 10:45am, 11:00am, 11:15am, and 11:30am. .0606(g) The following Technical Assistance regarding the violations observed during today’s visit was provided: -Safe Sleep Checks: As a reminder, review with staff on how to complete the safe sleep check chart and how to record the safe sleep positions. I suggested creating a system where the safe sleep documentation is reviewed and checked for accuracy daily as a closing activity of the classroom by staff. I also suggested that you check the safe sleep documentation at the end of each week to ensure that accurate records are kept. Sanitation Inspections: We discussed that Sanitation Inspection are due annually from when the last sanitation inspection had been completed. I suggested contacting your Environmental Health Inspector at least six weeks prior to when the last sanitation inspection occurred to ensure that the sanitation inspection does not go over one year. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by October 9, 2024. Please include in that letter each violation number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again.
