A. Harper, lead teacher assisted me with today’s visit. Your program currently operates with a five-star license, issued 10/3/25 earning 5 points in the education component, 7 points in the program standards component (meeting enhanced space and reduced ratio minus one) and 1 quality point for having a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher, and group leader over the past 12 months. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six percent as of 6/4/26. The NC Secretary of State website was reviewed on 6/9/25 and New Beginning Christian Childcare Center, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment. Children enrolled in classroom space #2 were observed running and playing in the train tunnel in the outdoor space. Children in classroom space #1 were observed transitioning to an art activity including paper, glue and orange paper squares. Later, children in classroom space #2 transitioned to the indoor space, handwashing was observed. The children engaged in free play with puzzles, then transitioned to lunch, and read individually. Lunch included peanut butter and jelly sandwiches mixed fruit, string beans with potatoes, and milk. The following violations were documented. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. There was a hole in wall behind the toilet in the children's restroom. 15A NCAC 18A .2825(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the van used for transportation there was a bottle of Parts Master Motor Oil. The storage cabinet in the staff bathroom was unlocked. Stored inside the cabinet were two jugs of Pine ammonia Floor Cleaner, Oxygen Orange Degreaser, jugs of Awesome Bleach, cans of Paint, ZEP floor stripper, and Ajax Dish detergent. .2820(b) 1123 All vehicles used to transport children were not free of hazards. Each child transported must be have on file emergency contact information along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. Your transportation notebook was not available for review today. Staff stated they switched vans after morning pickups. Leaving the notebook at the facility after each van route to ensure when switching vans you have the information of having a copy of the information on each van used. This is a repeat violation from the annual compliance visit on 12/18/25. You stated in your compliance letter received on 12/22/25 that all children's emergency information and pictures are in a binder located on the van for those children being transported daily. As part of your compliance letter submit a plan for complying with transportation requirements. Vehicle Maintenance: Children should be cared for and transported in safe environments. Maintaining the exterior of the vehicle is important to the safety of children. Today in the vehicle used to transport children the front bumper was damaged. Staff stated that a new bumper was ordered on Friday. Inspect vehicles daily prior to driving to ensure they meet state requirements. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. In the van used for transportation there was a bottle of Parts Master Motor Oil. The storage cabinet in the staff bathroom was unlocked. Stored inside the cabinet were two jugs of Pine ammonia Floor Cleaner, Oxygen Orange Degreaser, jugs of Awesome Bleach, cans of Paint, ZEP floor stripper, and Ajax Dish detergent. When asked, staff locked the cabinet. Review storage of hazardous items with staff. Tobacco Free Environment: Children shall be in a smoke free and tobacco free environment. Signage regarding the smoking and tobacco restriction shall be posted at each entrance to the center and in vehicles used to transport children. K. Harper, administrator assisted me with the visit. Your program currently operates with a five star license, issued 5/6/2022 earning 5 points in the education component, 7 points in the program standards component(meeting enhanced space and reduced ratios) and 1 quality point for the facility having a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher and group leader over the last 12 months. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 3/1/2023. The sanitation inspection was completed 1/31/23 with a “Superior” classification. The last fire inspection was conducted 3/13/2023 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of 9/8/24. The NC Secretary of State website was reviewed on 2/8/24 and New Beginning Christian Childcare Center, LLC was listed as current- active. I visited indoor and outdoor spaces. Children under age one were observed resting in approved cribs and during free play. The teacher met children’s individual needs including feeding and diapering. Sleep charts were observed and recorded as required. Children enrolled in classroom space #2 were observed during free play, morning group, transition, and outdoor play. Lunch was observed and consisted of butter beans, sausage, applesauce, a roll, and milk. The following violation was documented. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Six children did not have emergency information with a photograph on file. 10A NCAC 09 .1003(d) You corrected the violation during today’s visit. Therefore, no further action is required. Technical assistance was provided on the following: Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Each child transported must be have on file emergency contact information along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. Six children being transported did not have emergency information along with a photograph on file. You had the children’s emergency information on file, took pictures of the children, and added the information to your transportation information. Reminders: NCDHHS Children’s Environmental Health adopted new Child Care Sanitation Rules, effective July 1, 2023. The rules have been approved but have not yet been updated in the Administrative Code. Once updated, notification will be sent out via listserv.
