A. Gaddy, operator assisted me with the visit. Your program currently operates with a five-star license, issued 3/25/2024, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for having infant capacity restricted to no more than two children under one year of age. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 6/13/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of 5/26/26. The NC Secretary of State website was reviewed on 5/26/25 and your facility is not owned by a corporation. You cared for three children during today’s visit. The children engaged in free play with music items including drums and ribbons, as music played. Later the children engaged in indoor gross motor activities including riding on tricycles, seated spinners, and throwing and rolling a large ball to one another. Following indoor gross motor, you and the children took a walk outside the home in the front yard. Upon return you and the children transitioned to prepare for lunch. Lunch was observed and included chicken nuggets, potato tots, mandarin oranges, sliced wheat bread and milk. The following violations were documented. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A. Gaddy, operator assisted me with the visit. Your program currently operates with a five-star license, issued 3/25/2024, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for having infant capacity restricted to no more than two children under one year of age. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 7/2/24. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 6/9/25. The NC Secretary of State website was reviewed on 6/9/25 and your facility is not owned by a corporation. You cared for four children during today’s visit. The children engaged in an art activity using paint and paint brushed to paint picture frames, following art the children engaged in free play with age-appropriate materials. Later you and the children transitioned to the outdoor space. Upon transitioning back into the home, lunch was served and the children rested on cots and playpens. Lunch included chicken nuggets, French fries, mandarin oranges, and milk. The following violations were documented. Violation Number Comment Rule 908 Health questionnaire was not completed annually.
