LeShaunda McCotter, Child Care Consultant, accompanied me during today’s visit. C. Lakhram, NC Pre-K Program Director, assisted me with the visit. Your program currently operates with a five-star license, issued 8/17/20, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced requirements) and 1 quality point for the programmatic option of staff benefits and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum, High Scope, as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 1/21/25. The sanitation inspection was completed 11/12/25 with a “Superior” classification. The last fire inspection was conducted 9/26/25 and your facility was approved for daytime care only. This facility is currently owned by Pitt County Schools. The program’s compliance history was ninety-two percent as of 12/15/25. Nine groups were observed in the indoor and outdoor spaces. The children were engaged in breakfast upon arrival. The children transitioned to the outdoor courtyard for free play. Children throughout the facility were engaged in free play, including interest areas: art, blocks, library, music, and discovery areas. Teachers rotated throughout the indoor and outdoor spaces, getting down on the children’s level when talking to them. The outdoor fenced play spaces included stationery and portable toys and materials. I observed proper handwashing procedures. Breakfast was observed during today’s visit and consisted of mini bagels, cranberries, grape juice, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files were monitored for completed health assessments and developmental screenings. The program uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 9-3-25. The classroom operates from 7:40 am to 2:20 pm. You stated you had reviewed the policy with the staff during onboarding and discovered the documentation was missing while reviewing the files and gave the staff a second copy. The staff dated the policy for the date they received the second copy. Best practice is to sign and date the documents as trainings and the review of policies are completed and file them to reduce the risk of misdating and misplacing documents. Child care rules state all staff providing direct care for children must successfully complete a basic first aid and CPR course for the age of children in care, within the last 2 years, or prior to training expiration date, whichever is less. Administrator shared that the staff was out on leave during the training held in August 2025 and had returned in the recent weeks. You were able to contact a training and registered the staff for a training on December 19, 2025. Review staff files regularly to ensure all documents and trainings are acquired in the required time frame. This is especially important when there are irregularities such as extended leave or scarcity in training opportunities in the area. There was an active fire ant hill on the playground near the edging of the stationary equipment. You stated you did not know and called for maintenance to spray the area. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. Prevention of Shaken Baby Syndrome & Abusive Head Trauma Policy - Each child care center licensed to care for children up to five years of age must develop and adopt policies to prevent shaken baby syndrome and abusive head trauma. The policy must be reviewed with additional all caregivers prior to them providing care to the children. Documentation of this review for staff employed on 8/15/25 was dated for 9/5/25. It was reported that children began attending the facility on 9/3/25. You stated you had reviewed the policy with the staff during onboarding and discovered the documentation was missing while reviewing the files and gave the staff a second copy. The staff dated the policy for the date they received the second copy. Best practice is to sign and date the documents as trainings and the review of policies are completed and file them to reduce the risk of misdating and misplacing documents. Child care rules state all staff providing direct care for children must successfully complete a basic first aid and CPR course for the age of children in care, within the last 2 years, or prior to training expiration date, whichever is less. Administrator shared that the staff was out on leave during the training held in August 2025 and had returned in the recent weeks. You were able to contact a training and registered the staff for a training on December 19, 2025. Review staff files regularly to ensure all documents and trainings are acquired in the required time frame. This is especially important when there are irregularities such as extended leave or scarcity in training opportunities in the area. Original container-Prescription medication such as Albuterol must be in the original container with the prescription label from the pharmacy attached or written physician's instructions. Medication cannot be administered in any manner different from the pharmacy label instructions and/or the manufacturers labeled directions/dosage. There were two (2) inhalers (one (1) inhaler of alburetrol in space #112, and one (1) inhaler of Ventolin in space # 105) not in original boxes with prescribing information attached. We discussed completing medication audits at least monthly. Be sure the person that is completing the audit has been trained on what to look for: the medication in original packaging, the expiration date of the medication, the authorization form is complete with all required information, the instructions match the physician’s instructions on the pharmacy label and medical action plan, and the expiration of the authorization form. Teachers used the smart board to read “Pumpkin Growing” story to the children. Teachers rotated throughout the indoor and outdoor spaces, getting down on the children’s level when talking to them. The outdoor fenced play spaces included stationery and portable toys and materials. I observed proper handwashing procedures. Lunch was observed and consisted of General Tso’s chicken, rice, green beans, bananas, and milk. The following violations were documented. These medications may be stored out of reach, above five feet from the floor. Other medication that are part of an emergency medical care plan, such as an antihistamine, must be kept in locked storage. In the red bookbag hanging on the wall in space #112 there was one bottle of Benadryl and two bottles of Allergy Relief medications. Staff in space #112 stated they did not know that Benadryl had to be locked up since it was noted as a rescue medicine on the children’s medical forms. During the visit, the classroom was provided a lock box for the antihistamine and an order was placed for additional locking medication bags. Today TA was provided to the administrator and nurse staff on the requirements of medication storage. Storage of Hazards Items - All aerosol cans are highly flammable and present a hazard because of the compressed gas used as a propellant. If punctured, the contents may be released so forcefully that injuries can result. On top of a shelf located in a shared bathroom for space # 105 and #107 was an aerosol can of Lysol disinfectant spray. A staff member in space #107 locked the aerosol can in a closet during the visit. All aerosols must be kept in locked storage which is unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. Consultation: We reviewed the DCDEE Medication Authorization form and the information included on the form. Some classrooms were using the Pitt County Schools Medication Authorization forms. This form does not have the required end date. Emergency Medication authorization form are valid for up to six (6) months. By using the DCDEE provided form, you can ensure all medications are not authorized and used beyond the six (6) month time frame. C. Lakhram, NC Pre-K Program Director, assisted me with the visit. Your program currently operates with a five-star license, issued 8/17/20, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced requirements) and 1 quality point for the programmatic option of staff benefits and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum, High Scope, as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 3/2/23. The sanitation inspection was completed 5/22/23 with a “Superior” classification. The last fire inspection was conducted 10/25/23 and your facility was approved for daytime care only. Eleven groups of children were observed during today’s visit. Each classroom was equipped with age-appropriate materials including books, child size furniture, puzzles, manipulatives, blocks, and art materials. In the indoor learning environment, children engaged large and small group activities and free play. Children’s artwork was displayed on the walls of the classrooms. Teachers were observed interacting with the children with positive language, problem solving, and open-ended questions. The outdoor space offered stationary structures, tricycles, hula-hoops. Lunch was observed during today’s visit and included cheese sticks with marinara sauce, beans, peaches and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files were monitored for completed health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 9-5-23. The classroom operates from 7:40 am to 2:20 pm. Parent conferences are held three times per year. It was reported that teachers use the Class DoJo app, children’s daily communication folders, and a monthly newsletter to communicate with parents any information about the child’s day and any reminders. The environment rating scale score received on 3/22/23 was 6. The following violations were documented.
