Your program currently operates with a three-star license, issued 11/2/22, earning 4 points in the education component, 2 points in the program standards component (meeting enhanced ratios) and 1 quality point for the programmatic option of staff benefits package and infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six as of 7/3/25. The NC Secretary of State website was reviewed on 7/3/25 and Rising Stars of North Carolina, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. Today, I completed a walk-through of the facility, monitoring all indoor and outdoor areas. Six groups of children were observed in the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children enrolled in space #1, napped as soft music play. Safe sleep checks were completed and documented as required. Children enrolled in spaces #2, #3, #4, #5, and #6 had free play in the outdoor learning environment. Staff moved about the spaces, talking with the children and assisting them with the materials. Handwashing and diapering procedures were observed. The classrooms are arranged in learning centers with materials on low lying shelves for easy access. Spaces #2, #4, and #6 have new room arrangements since my last visit. Files for new staff were reviewed. Fire and sanitation inspections remain current. The following violations were observed. Grass was growing in the fall zone under the stationary structures on outdoor spaces #2 and #4. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One canister of Disinfectant Wipes was stored on a shelf, above five feet, over the hand sink, and one canister of Disinfectant Wipes was stored in an unlocked closet in space #4. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. One tube of prescription Nystatin cream was stored on a shelf, above five feet, in space #1. 15A NCAC 18A .2820(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 7/23/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Today, I observed grass growing in the containment area under the stationary structures in outdoor spaces #2 and #4. You shared with me that you had spoken with the landscaper about addressing the grass in the mulch, but he did not come at the designated date and you do not have a sure date of when he will return. As discussed, fall zones should be weeded regularly and loose parts including an pine cones, gumballs, and sticks should be collected daily to ensure fall zones are kept free of hazards. For the safety of the children, you must ensure weeds and debris are removed and the mulch tilled or added to the appropriate depth to avoid potential injuries to children. I also urged you again to create a regular weeding and tilling schedule for the mulch. To verify compliance, please submit photos and a copy of a July playground inspection showing these hazards have been addressed. Medication Storage- Medicines can be crucial to the health and wellness of children. All prescription medication must be stored behind lock and key. Today, prescription Nystatin cream was stored on a shelf above five feet in space #1. Locked storage was available in the room. Thank you for relocating the medication during the visit. Counsel staff on medication storage. You may provide them with a visual reminder as well. Consultation: Staff Training/Orientation- When bringing on new staff, it is very important to thoroughly train them on your programs policies, procedures, and daily practices. Today, we discussed recent staff changes and the steps you are taking to ensure they have strong foundational skills. Continue to complete classroom observations and coaching with the staff. Completing hands on activities and having one-on-one demonstrations may be beneficial to their learning style. Make sure to provide the staff with opportunities to ask questions and provide feedback. Staff/Child Interactions- Providing positive guidance through nurturing and responsive relationships and supportive environments to promote children's self-control, teach responsibility and help children make thoughtful choices. Today we discussed having staff to use positive, encouraging language. You shared this was something you have been addressing in staff training. The more effective caregivers are at encouraging appropriate behavior, the less time and effort caregivers have to spend correcting children's behavior. Today we discussed your current practices for ensuring all medications are accompanied by written authorizations from the parent. It was reported the classroom staff review the medications and authorizations weekly. Today we discussed training the staff on how to properly audit medications and authorization forms. Review medication requirements with staff and periodically check behind them to ensure all requirements are met. There was a new prepackaged first aid kit in the van used to transport children. These kits sometime come with medications an ointments in them. When you purchase or received new first aid kits, review the label and content and removed any prohibited items. Consultation: Mulch-When completing outdoor inspections be sure to check that grass is not growing in the mulch under the stationary climbing structures as well as the surfacing has not become compacted. Today, I observed mulch that has been compacted over time under the monkey bars and at the base of the slides and stairs. When fluffed the area measured at 6 inches in depth as required. Have the mulch under and around the stationary structures fluffed and reassess to see if additional mulch needs to be added. Required Water, Lead Paint, and Asbestos Testing - New child care rules require child care facilities to test for lead in drinking water, lead paint, and asbestos. Testing is required by 1/1/2025. Check out the FAQ in the updated Clean Classrooms for Carolina Kids. Currently there is funding to pay for the testing and to mitigate lead paint or asbestos hazards. Depending on the age of your home or building and the information you provide in the enrollment process, testing for lead paint and asbestos may not be required. Per the Clean Classrooms for Carolina Kids website you have completed all required tests and are waiting for your Lead Based Paint and Asbestos results. An invoice will be emailed to the facility’s contact email after October 1st and will include the invoice number and total amount due. All payments are due no later than December 31, 2024, and must be submitted online only through the Division’s website using a Mastercard or Visa debit or credit card, or by e-check. These kits sometime come with medications an ointments in them. When you purchase or received new first aid kits, review the label and content and removed any prohibited items. Consultation: Mulch-When completing outdoor inspections be sure to check that grass is not growing in the mulch under the stationary climbing structures as well as the surfacing has not become compacted. Today, I observed mulch that has been compacted over time under the monkey bars and at the base of the slides and stairs. When fluffed the area measured at 6 inches in depth as required. Have the mulch under and around the stationary structures fluffed and reassess to see if additional mulch needs to be added.
