S. Harris, center supervisor, assisted me with the visit. Your program currently operates with a five-star license, issued 5/16/23 earning 7 points in the education component, 5 points in the program standards component (meeting enhanced ratio) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The sanitation inspection was completed 12/23/2025 with a “Superior” classification. The last fire inspection was conducted 12/23/25 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent as of 1/26/26. The NC Secretary of State website was reviewed on 1/26/26 and Precious Moments Learning Center, LLC was listed as current- active. Children in classroom space #1 were observed during free play, the teacher was observed reading a story to some children in the block area. In classroom spaces #4 and #5 children one to two years of age were observed dancing to music. Lunch was observed and consisted of chicken nuggets, broccoli, mixed fruit and milk. The allegations are as follows: There is a concern that documentation was not provided when a child was injured in care. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions were monitored. The license and emergency care plan were posted. Children were observed during free play with blocks and dramatic play items. Staff engaged in free play with the children, by asking them questions and pretending to cook food and talk on the phone with the children. Lunch was observed and included meatballs, wheat bread, mixed vegetables, applesauce and milk. Brown Regarding the allegation that documentation was not provided when a child was injured in care, observations and selected interviews were conducted. S. Harris, center supervisor assisted me with today’s visit. I reviewed the facilities incident log for 2024. I reviewed thirteen (13) incident reports from August 2024, September 2024 and for the current month, October 2024. All thirteen (13) incident reports were documented on the incident log, indicators were selected, on how parents were notified, and parents’ signature and date of receipt were indicated on the incident reports. Five (5) incident reports observed indicated a date when the parent was notified but did not include the time the parent was notified. Two (2) reports did not include the date the parent was notified but included the time the parent was notified. It was reported that incident reports are completed for all incidents even when there is not a visible injury, parents are contacted by text or phone call when an injury occurs. Collateral reports were received that included photographs and documentation of a child enrolled in your facility. You informed me that the Department of Social Services visited the facility on August 20, 2024, to investigate concerns of child injury. Today you stated you were not aware of any incidents that occurred that were not reported to you by parents or staff. Based on observation and collateral reports the finding regarding the allegation of documentation not being provided when a child was injured was not confirmed. The following violation was observed: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Michelle O’Kelley, Child Care Consultant accompanied me during today’s visit. S. Harris, Center Supervisor assisted me with today’s visit. Your program currently operates with a five-star license, issued 5/16/23 earning 7 points in the education component, 5 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having a infrastructure of family involvement and staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of 9/3/24. The NC Secretary of State website was reviewed on 11/9/2022 and Precious Moments Learning Center, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children enrolled in classroom space #1 were engaged in free play in the dramatic play and block areas. In classroom space #3 children sat with teachers on the carpet engaged in play with a variety of age-appropriate toys. Children under one years of age received care according to their individual needs including diapering. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In classroom space #1 the shelf for the cots was broken, in classroom space #2 the linoleum was lifting at the door of the bathroom creating a trip hazard, in classroom space #4 the vent covers on the floor were rusted and peeling, the linoleum was peeling, and in classroom space #5 the carpet in the block area was lifting creating a trip hazard. On playground #3 there were vines with thorns growing near the fence, the base of the school bus was exposed creating a tripping hazard, the concrete base of the basketball goal was exposed and the concrete base the black rubber tiles was exposed. 10A NCAC 09 .0601(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Check sleep charts weekly to ensure they are completed daily for infants in care. Additional Comments: Teachers should provide consistent, continuous and inviting opportunities to talk, listen to, and otherwise interact with young infants and toddlers throughout the day including feeding, changing, playing with, and comforting them. Language increases children’s learning and helps them learn about the world in which they live. We discussed increasing language in the infant classroom, encourage staff to get down on the floor with infants and consistently talk to the children. Reminders: The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Lunch was observed and included cheeseburgers, corn, oranges and milk. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. Prescribed medications should be in the original container (with a label that includes the child’s name, date filled, prescribing clinician’s name, pharmacy name and phone number, dosage/instructions, and relevant warnings). In space# 1 there was a ventolin inhaler stored in the cabinet that was not in it’s original box and was not accompanied with the prescription label. We discussed having the parent bring the medication in the original box with prescription label. Playground Surfacing: The surfacing used underneath two stationary pieces of equipment was a light covering of mulch material that measured less than 1 inch under and around the monkey bars and climbing octagon. In order to continue providing a safe outdoor environment, be sure to maintain the six-inch depth and frequently fluff the materials if it becomes compact due to routine play or rainy weather. Additional Comments: Guidance is about building an encouraging setting for every person in the group. It means helping young children understand they can learn from their mistakes, and it starts with showing them how. To give this help successfully, we need to build relationships with every child—especially with the children we find difficult to connect with and understand. We build these relationships from day one, outside of conflict situations. It is only when children know and trust us in day-to-day interactions that they will listen to us when conflicts happen (after we have helped everyone calm down). Use positive guidance and redirections to help child navigate the classroom space. Reminders: A new July 2023 New Rule/Rule Amendments Overview module has been added to the Child Care Rules Training on DCDEE Moodle.
