N. Shamble, operator assisted me with today’s visit. Your program currently operates with a four-star license, issued 1/15/2024 earning 7 points in the education component, 4 points in the program standards component (meeting enhanced ratio) and 1 quality point for in a stand-alone school program, 75% of group leaders have at least 5 years’ experience in no more than 2 different school age setting. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety- five percent as of 5/5/26. The NC Secretary of State website was reviewed on 5/5/26 and Precious Moments Afterschool Care, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Observed children arriving from school on the school bus and from facility van arrivals. Upon arrival children were observed putting away their personal belongings, washing hands, then eating snack. Snack included Triscuit crackers, pears, and milk. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was a localized soft spot in the floor in classroom space #1. In classroom space #2 the edges of the rug are in the dramatic playa area was torn. On the playground there was a broken piece of equipment with a sharp edge exposed, a broken planter box, and three large holes creating a tripping hazard. 10A NCAC 09 .0601(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. N. Shamble, administrator assisted me with the visit. Your program currently operates with a four-star license, issued 1/15/24. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The sanitation inspection was completed 9/8/25 with an “Superior” classification. The last fire inspection was conducted 11/18/24 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-seven percent as of 10/22/25. The NC Secretary of State website was reviewed on 10/22/2025 and Precious Moments Afterschool Care, LLC was listed as current- active. I visited indoor and outdoor spaces. Children were observed arriving from school. Staff and children engaged in back-and-forth conversation. Staff asked the children open-ended questions. Children and staff discussed what the children did at school today and an upcoming event at school. The children also engaged in a group game and asked each other math questions. Snack included goldfish, mangos, and milk. The following violations were documented. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. You stated that you scheduled a date already for the next review so that it will be completed annually as required. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety percent as of 3/20/25. The NC Secretary of State website was reviewed on 3/20/25 and Precious Moments Afterschool Care, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children resting on linen covered mats. The outdoor space included monkey bars, an octagon climber, portable playhouse, and space for gross motor play. The snack menu reflected cheese sticks and ritz crackers. The NC Secretary of State website was reviewed on 6/13/24 and Precious Moments Afterschool Care, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor classroom environment. The children sat at a child sized table engaged in free play with age-appropriate materials. Later the children transitioned to outdoor play. There were no new staff at the facility. You informed me that currently to transportation vehicles at the facility were not being used. When I arrived, I rang the doorbell, twice there was no answer. Shortly after a staff member arrived. I spoke with the owner via telephone. I observed children arriving from school and transitioning to their designated classroom spaces. Later the children at snack. Snack included oranges, pretzels, and water. Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Any vehicle approved and used for transportation must have a First Aid kit and fire extinguisher located in the vehicle used to transport children. Today there was no fire extinguisher in the vehicle. A fire extinguisher must be bought and placed inside the vehicle immediately. COMPLIANCE HISTORY A facility's compliance history is assessed by evaluating the violations of requirement that have occurred. A minimum compliance history score of at least seventy-five percent (75%) must be maintained at all times. You, N. Shamble, administrator assisted me with the visit. This facility was issued a temporary license on November 20, 2023 with a restriction of daytime care only. A sampling of classrooms, the outdoor learning environment, sampling of staff and child files, and other required program records were monitored. When I arrived, I rang the doorbell, twice there was no answer. Shortly after a staff member arrived. I spoke with the owner via telephone. I observed children arriving from school and transitioning to their designated classroom spaces. Later the children at snack. Snack included oranges, pretzels, and water. The required posted information and materials were present in each classroom. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Broken hula hoop pieces were stored in a toy bin and on the playground. The bolts of base of the school bus were protruding. 10A NCAC 09 .0601(a) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States.
