Your program currently operates with a four-star license, issued July 8, 2024, earning 3 points in the education component, 6 points in the program standards component, and 1 quality point for education option of infant capacity restricted to no more than two children under one year of age. The family child care home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent as of June 9, 2026. The NC Secretary of State's website was reviewed on June 9, 2026, and Nanna’s Family Day Care, LLC was listed current-active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. Upon arrival, you greeted me at the door. Three children were present with you, engaging in indoor free play. The group later transitioned to whole group where you did a morning greeting and reviewed colors, shapes, and read stories. Proper handwashing techniques were observed. The childcare space has been arranged since the last visit, adding a larger child sized table and increasing the floor space for group time. Learning material were stored on low lying shelves for easy access. The room was organized in learning centers, including dramatic play, blocks, art, science, and reading. Lunch was observed and consisted of pasta, peas, carrots, bread, and milk. The following violations were documented. Violation Number Comment Rule 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The ITS-SIDS certification on file expired on 2/26/25. .1703(a)(4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. The ITS-SIDS training on file expired on 2/26/25. You stated that you thought it was still current. You were able to contact a training and schedule a training during today’s visit. Although you do not have infants enrolled, the program is license to serve infants which required the ITS-SIDS certification to remain current. Use the FCCH and Center Located in a Residence Verification of Required Information for Operator and Additional caregivers form as a tracking tool. Monitor the form with your files at least quarterly to ensure all deadlines and requirements are met. Rated License: Your program currently operates with a four-star license, issued July 8, 2024, earning 3 points in the education component, 6 points in the program standards component, and 1 quality point for education option of infant capacity restricted to no more than two children under one year of age. You submitted your application for rated license on June 10, 2026, pursing Classroom and Instructional Quality Pathway. Program Standards: You are using Gee Whiz curriculum and have acquired the Desired Results Developmental Profile assessment tool. I reviewed the Family and Community Engagement Standards with you. You are meeting all the required standards and two additional options. You also submitted the Family Child Care Home Operator Continuous Quality Improvement Plan and Professional Development Plan. The program is eligible for the four-star level in this component. Corporation Status: Per your statements, you created an LLC in September 2022. The NC Secretary of State's website was reviewed during the visit and Nanna’s Family Day Care, LLC was listed current-active. A new facility application and Articles of Incoporation were received during the visit. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Two children were present in the childcare room with the additional caregiver. The two children were seated at the table engaging with paper, crayons, puzzles. In the dining area, you sat with one child completing math problems and reading a story. Later all three children transitioned to dramatic play. Proper handwashing techniques were observed. Learning material were stored on low lying shelves for easy access. The room was organized in learning centers, including dramatic play, blocks, art, science, and reading. The outside area offers a new swing set and shade structure. Lunch was observed and consisted of hamburgers, potatoes, apple juice and milk. The following violations were documented. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. One riding lawn mower, one weed eater, a bottle of Spectracide Bug Stop, one bottle of Martin's Mosquito Pro, and one container of Tom Cat Rodent Killer were stored in the unlock storage building in the back yard near the children's play area. .1719 (a) (1) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. One bottle of Fabuloso and one can of Homeline Scents air freshener was stored on top of a shelf, above five feet, in the bathroom used by children. One can of Lysol Disinfectant spray was stored in the window seal of the kitchen and on top of a frame above the changing table of in the living room. .1719 (a)(7) 1891 Individuals providing care to children did not receive 16 hours of orientation prior to beginning caregiving activities. The additional caregiver, S. Mayo, completed 14 hours of orientation in November 2023. .1729( c) 2050 The ownership of the family child care home changed without notifying the Division thirty days prior to the change occurring. A new LLC was created for the facility on 9/21/22, without notifying a representative of the Division. .1702(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. You share with me that you had done some cleaning yesterday following having guest to prepare for the operating day and in your haste, you did not put the cleaners back in the locked cabinet. Today I suggested you complete a walkthrough of your home prior to providing care for children to ensure all cleaning supplies are properly stored. A “Hazardous Items Storage” poster is on the Division’s website that can be posted as a visual reminder. Storage Building- All storage buildings must be locked. Today I observed the door of the storage building was unlocked. You stated that it had recently broken. You were able to call a repair man during the visit. Complete daily checks of the outdoor area prior to providing care to ensure all hazards are not accessible to children. Staff Orientation- Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. Effective employee orientation makes employees aware of company policies and expectations, handles essential paperwork, and answers any questions or concerns they may have before they transition into their new positions. Staff employed 11/3/23 completed 14 hours of orientation. You stated that since you were not administering medication, you did not review that section of the orientation. Orientation is important because it signifies the beginning of the relationship between employee and employer. Create a plan for orientation of future staff and substitute caregivers. Ownership change- When you wish to change the owner of you family child care home, you must notify the Division 30 days prior to the change. You stated you were informed me a member of the community that you needed to create an LLC. You reported that you did not recall notifying the consultant you were working with at the time of the change. Today, I reviewed childcare rule .1702(e) and section .2200. Failure to notify the Division of a change of ownership warrants an Administrative Action. Reminders: FCCH Verification Form and Statements of Responsibilities– Your FCCH Verification Form is updated and provided to you as a part of your visit summary and is an excellent tool to help you stay current with items that expire, i.e., Criminal Record Checks, well water tests, pet vaccinations, CPR, First Aid and ITS-SIDS trainings, on-going and H&S training requirements, annual health questionnaire, household members, pet vaccinations, and vehicle information. The Operator’s Statement of Responsibilities was reviewed during the visit. Required Water, Lead Paint, and Asbestos Testing - New child care rules require child care facilities to test for lead in drinking water, lead paint, and asbestos. Testing is required by 1/1/2025, but enrollment was required by 5/1/2024. Check out the FAQ in the updated Clean Classrooms for Carolina Kids. Currently there is funding to pay for the testing and to mitigate lead paint or asbestos hazards. Depending on the age of your home or building and the information you provide in the enrollment process, testing for lead paint and asbestos may not be required. (Lead paint was not used in homes built after 1978. Asbestos was not used in homes after 1988.) Water testing will only be required once for family child care homes. The program’s compliance history was ninety-three percent as of 1/22/24. Corporation Status: Per your statements and according to the NC Secretary of State's website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. The license was posted, and the restrictions were in compliance. Today I completed a walk-through of the family child care home, monitoring all the indoor and outdoor areas. I observed children in child care room, engaging with learning materials including cars, blocks and toy food. An additional caregiver was present and engaged in play and conversations with the children. Later the group had story time. Children identified items on the pages as the caregiver read the story. Proper handwashing techniques were observed. File for the new caregiver was reviewed. The violations are as follows: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The last documented playground inspection on file was dated 9/5/13, therefore was not completed monthly. 10A NCAC .1721(e)(5)(A-F) 935 Individuals who provide care for 5 hours or more a week, during planned absences of the operator, did not submit criminal background check forms as required in Rule .2703. Additional caregiver, employed on 11/3/23, had a Qualifying letter dated 11/8/23. "G.S. 110-90.2(b) & .1729(a)(6) 1101 Operator or other transportation provider did not have written permission from each child's parents or guardian to transport the child. Transportation authorization forms for three of four children enrolled, expired 12/13/23. .1723(5) 2010 Orientation received did not include the required topics as listed in rule. Documentation of orientation was not on file for additional caregiver employed on 11/3/23. .1729(c)(1-16) 2015 Operator did not review the abusive head trauma and shaken baby syndrome policy with staff and/or the acknowledgement of the review did not contain required items or was not available for review. These requirements protect children in family child care home by ensuring these family child care home provide a physically safe and healthy environment where the developmental needs of children are being met and where children are cared for by qualified staff. Operator and additional care givers must receive a minimum of 16 hours of orientation within the first 6 weeks of employment. Documentation of orientation was not on file for S. Neal-Mayo during today’s visit. You stated you had completed orientation with Ms. Neal-Mayo, but it had not been documented on the orientation log form. Document orientation as it occurs to maintain current and accurate records. Provide documentation of orientation with your compliance letter. Prevention of Shaken Baby and Head Trauma Policy- Each child care program licensed to care for children up to five years of age shall develop and adopt policies to prevent shaken baby syndrome and abusive head trauma. This policy must be reviewed and signed prior to caring for children ages 0-5. Caregiver S. Neal-Mayo did not have a documentation of acknowledgement of the Prevention of Shaken Baby and Head Trauma Policy on file for review during the visit. You stated you reviewed it during orientation, but the paper must have gotten misplaced. Thank you for reviewing the policy with her during today’s visit. Criminal Background Check- All owners of licensed child care facilities and employees working in a licensed family child care home are required to submit a Criminal Background Check (CBC) and receive their Qualifying Letter prior to their first day of employment and complete a five-year requalification (on-line application and fingerprints) so that the new Qualifying Letter is on file before the expiration date listed on their current Qualifying Letter. Caregiver, S. Neal-Mayo, employed on 11/3/23 had a Qualifying Letter dated 11/8/23, therefore was employed prior to obtaining a qualified status. You stated she had come in on 11/3/23 to begin paperwork and reviewing policies. I advised you that orientation should not begin until the potential staff is qualified and proper documentation is on file. Multiple instances of failing to complete Criminal Background Checks may result in an Administrative Action. Review the section in the NC Child Care Requirements on Criminal Background Checks: 10A NCAC 09 .2700. Transportation Permission Forms- Transporting children in child care is a high risk activity. Parents/guardians must provide written permission to transport children and be notified when and where the child is to be transported. Parents/guardians may give standing permission, valid for up to 12 months. Transportation permission forms had expired on 12/13/23 for three children enroll. Please have the parents complete a new form. Technical assistance was provided to today regarding monitored/auditing files. Review the files monthly and set reminders for documents with approaching expiration/due dates. Outdoor Inspections- The outdoor learning environment offers a sense of freedom for children. Since children are exploring it is very important to provide a safe environment. Documentation reflects the last outdoor inspection was 9/5/23. You stated you did not use the climber and sandbox area which you considered the playground. You also stated you did inspect the other area of the yard used but did not document it as you did not consider it the playground. We discussed the importance of checking the entire outdoor play area as children have access to the entire yard. Reminders: Maintaining Records- Technical assistance was also provided today regarding file organization. I suggested that you use the revised file checklists under “Provider Documents and Forms” on the DCDEE website to assist with organization and ensure all requirements are met. I suggested you create a system to maintain your records. Consider using a binder for program records as you are currently doing to maintain children’s and caregivers’ records. I strongly urge you to review Child Care Rule 10A NCAC 09 .1721 regarding requirements for records. Please pay close attention to the charts located in 10A NCAC 09 .1721(f)(4) which indicate how long records must be kept on file. Three Month Self-Study- You recently attended a Three Month Self-Study training session. We discussed the things you learned from the session. You shared with me that you care currently working to improve you outdoor learning area. I suggested you review that Family Child Care Environmental Rating Scale subsection “Activity Physical Play” as well as “Nature and Science.” Also, review the most recent assessment results to determine areas of improvement. You served breakfast and stood beside them, engaging in conversation about the meal and how milk helps make strong bones. The group transitioned to handwashing and moved to the child care space. Learning material were stored on low lying shelves for easy access. The room was organized in learning centers, including dramatic play, blocks, art, science, and reading. Educational posters and children’s artwork hang on the walls of the space. You then had group time, singing the alphabet song, weather observation, and reviewing colors by identifying the color of objects in the room. Outside, children play with balls and push and ride toys. You visited to garden and observed how big the watermelon had gotten. Proper handwashing techniques were observed. The health questionnaire on file for two of two caregivers was expired. .1703(a)(1) 1891 Individuals providing care to children did not receive 16 hours of orientation prior to beginning caregiving activities. Additional caregiver employed on 9/16/22 did not have documentation on orientation on file for review. .1729( c) 2015 Operator did not review the abusive head trauma and shaken baby syndrome policy with staff and/or the acknowledgement of the review did not contain required items or was not available for review. Two of two caregiver files did not contain documentation of review of the abusive head trauma and shaken baby syndrome policy. .1726(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 8/16/23, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. A copy of the form is posted on the Division’s website, www.ncchildcare.nc.gov, under the provider’s tab. We discussed using caregiver file checklist. You stated you will be adding it to each caregiver’s file and used to make sure all forms are on file by day one of hire. Prevention of Shaken Baby Syndrome & Abusive Head Trauma Policy - Each child care program licensed to care for children up to five years of age must develop and adopt policies to prevent shaken baby syndrome and abusive head trauma. The policy must be reviewed with additional caregivers prior to them providing care to the children. Documentation of this review was not on file for the additional caregiver hired in 9/16/22. You stated you had reviewed this policy with your additional caregiver and the documents must have been misplaced. Review the policy with all caregivers and file in her personnel file. Orientation- Orientation provides the basic information caregivers need to feel prepared for their new role within the family child care home. Effective orientation makes caregivers aware of policies and expectations, handles essential paperwork, and answers any questions or concerns they may have before they transition into their new positions. Each caregiver must receive 16 hours of orientation within the first 6 weeks of employment. The caregiver employed on 9/16/22 did not have 16 hours of documented orientation for review during the visit. You stated you had completed orientation with her, and the documents must have been miss placed. When bringing in new care givers, complete orientation, and document it immediately in the personnel file to ensure proper documentation and storage. Pet Vaccines- Vaccinations prevent diseases that can be passed between animals and from animals to people. Diseases prevalent in wildlife, such as rabies and distemper, can infect unvaccinated pets. Vaccines for a canine in the home expired 5/1/23. You stated the due date had slipped by you. We discussed documenting the due date of vaccines on your phone or wall calendar to help you track them. Please see you vet to have vaccines updated and provide proof of vaccination with you compliance letter. Reminders: Be knowledgeable of the child care laws and rules, review the with your center staff, and assist your staff with maintaining compliance with the laws and rules.
