T. Mosley, Director, assisted me with the visit. Your program currently operates with a five-star license, issued November 21, 2025, with NAEYC Accreditation, meeting all enhanced requirements. The last annual compliance visit was conducted on May 8, 2025. The sanitation inspection was completed January 8, 2026, with a “Approved” classification. The last fire inspection was conducted on March 10, 2026, and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of April 21, 2026. This program is owned by East Carolina University. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. Four groups of children were observed in the indoor environment. Children enrolled in space #1 received care based on their individual needs including tummy time, feeding, and naps. In space #1, children gathered at the carpet for music time. They sing songs and play maracas as a classroom guest play the guitar. In spaces #2 and #3, children engaged in free play including dramatic play, art and blocks. Teachers were observed moving about the spaces, talking with the children, asking questions, and assisting them with the materials. Morning snack was observed and consisted of cheerios and milk. The following violation was documented. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #2, a written authorization for Albuterol Sulfate expired on 4/8/26 and a written authorization for an Epi Pen expired on 4/15/26. 10A NCAC 09 .0803(1)(a & b) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One of five children's records reviewed did not contain a health assessment. GS110-91(1) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. Two of the five children's records reviewed, did not contain a signed acknowledgement of receiving the discipline policy. .1804(c) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The administrator did not complete the ABCMS Provider Portal training, therefore does not have access to the portal to update the roster. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 7, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations 4were corrected. We walked though how to access the training on DCDEE Moodle. After completing the training you will need to submit the power form to gain access to the portal. There you can find your facility codes to help you and your staff complete Connecting Applications. Once Connecting Applications are completed you can log back into the ABCMS Portal and add everyone to your roster. I have provided the ABCMS Provide Portal Technical Assistance guide with you during the visit. Medication Authorizations- Administering medication requires skill, knowledge and careful attention to detail. Parents must give a caregiver/teacher written authorization to administer medication to the child. There were two expired medication authorization forms in space #2. You shared you have an assigned staff that does monthly checks of the medications and authorizations. Additionally, stated there is supposed to be a log located in the classroom, listing medications and authorization forms and their expirations and noticed they are not where they are supposed to be posted. During the visit, you created a reoccurring event on your calendar to alert you and the assistance director to monitor the medications throughout the facility. You were also able to contact the parents during the visit to have them update the authorization forms at pick up. Have teachers check the medications themselves along with the assigned staff. Additional training may be required. A Medication Administration Poster may also be helpful and can be found at https://healthychildcare.unc.edu/resources/posters/. Children's Records - Maintaining accurate documentation at the child care center is important because it demonstrates compliance with the child care rules and ensures the health and safety of each child. Children's records are required to completed and maintained for each child who attends your child care (full-time, part-time, or drop-in). Children's records must be kept for at least one year after the child no longer attends. A health assessment and immunization record is required within 30 days of the child's first day of enrollment. One of five children’s records did not contain a health assessment. Two of five files reviewed did not contain a signed discipline policy. You emailed all families involved to inform them of the documents that were needed. One family sent you a copy of a recent health assessment, and you file it. You shared you typically have parents bring the health assessment and immunization records on or before the first day. Since you time at the center you have developed an acknowledgement form for parents to sign to reduce the number of forms to track and those families enrolled prior to the form being implemented. T. Mosley, Director, assisted me with the visit. Your program currently operates with a five-star license, issued November 21, 2025, with NAEYC Accreditation, meeting all enhanced requirements. The last annual compliance visit was conducted on May 8, 2025. The sanitation inspection was completed January 8, 2026, with a “Approved” classification. The last fire inspection was conducted on March 10, 2026, and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of April 21, 2026. This program is owned by East Carolina University. Children enrolled in space #1 received care based on their individual needs including tummy time, feeding, and naps. In space #1, children gathered at the carpet for music time. They sing songs and play maracas as a classroom guest play the guitar. In spaces #2 and #3, children engaged in free play including dramatic play, art and blocks. Teachers were observed moving about the spaces, talking with the children, asking questions, and assisting them with the materials. Morning snack was observed and consisted of cheerios and milk. The following violation was documented. You have not completed the ABCMS Provider Portal Training to gain access to the ABCMS Provider Portal. You stated that you did not recall taking the training and it was an oversight. We walked though how to access the training on DCDEE Moodle. After completing the training you will need to submit the power form to gain access to the portal. There you can find your facility codes to help you and your staff complete Connecting Applications. Once Connecting Applications are completed you can log back into the ABCMS Portal and add everyone to your roster. I have provided the ABCMS Provide Portal Technical Assistance guide with you during the visit. Medication Authorizations- Administering medication requires skill, knowledge and careful attention to detail. Parents must give a caregiver/teacher written authorization to administer medication to the child. There were two expired medication authorization forms in space #2. You shared you have an assigned staff that does monthly checks of the medications and authorizations. Additionally, stated there is supposed to be a log located in the classroom, listing medications and authorization forms and their expirations and noticed they are not where they are supposed to be posted. During the visit, you created a reoccurring event on your calendar to alert you and the assistance director to monitor the medications throughout the facility. You were also able to contact the parents during the visit to have them update the authorization forms at pick up. Have teachers check the medications themselves along with the assigned staff. Children's Records - Maintaining accurate documentation at the child care center is important because it demonstrates compliance with the child care rules and ensures the health and safety of each child. Children's records are required to completed and maintained for each child who attends your child care (full-time, part-time, or drop-in). Children's records must be kept for at least one year after the child no longer attends. A health assessment and immunization record is required within 30 days of the child's first day of enrollment. One of five children’s records did not contain a health assessment. Two of five files reviewed did not contain a signed discipline policy. You emailed all families involved to inform them of the documents that were needed. One family sent you a copy of a recent health assessment, and you file it. You shared you typically have parents bring the health assessment and immunization records on or before the first day. Since you time at the center you have developed an acknowledgement form for parents to sign to reduce the number of forms to track and those families enrolled prior to the form being implemented. During a child’s first 30 days of enrollment, check the file frequently to ensure all required documents are on file. Children enrolled in spaces #2, #3, and # 4 played in the outdoor learning environment with tricycles, sand toys, balls, and chalk. Teachers were observed moving about the spaces, talking with the children and assisting them with problem solving. Proper handwashing techniques were observed. Files for new staff were reviewed. Fire inspection remains current. A sanitation inspection was completed on 5/14/25 with a Superior classification. Today, I observed peeling paint near the window and the exterior door of space #1. You stated there was a pull bar and mirror that were removed from that wall, causing it to peel. A work order had been put in on August 12, 2025 .You followed up with facility maintenance during the visit to confirm they will complete the repairs on October 13-14, 2025, when the facility is closed to children. Additionally, you moved a book shelf and added an educational poster to cover the peeling paint and make it inaccessible to the children until repairs can be made. Technical assistance was provided about making hazardous items inaccessible to children and following up with maintenance staff to ensure potential hazards are addressed quickly. Consultation: Outdoor Inspections- Today the mulch near the slide on space #2 was compacted. Technical assistance was provided to create a routine schedule for fluffing mulch. You shared you will work with student workers to have a rotating schedule for them to rake the mulch. After fluffing the mulch, measure the mulch to ensure there is at least six inches of loose surfacing. If it does not meet this requirement, add additional mulch. Infant food/Bottle Labels- To help ensure the food and bottles are fresh and consumed by the correct child, all bottles and food containers must be labeled with the child’s name and the date it is received at the child care facility. Confirm containers are labeled upon arrival to the facility/classroom and prior to feeding the child. Each child had a space in the refrigerator that was labeled with their name. Star Rated License- Your program currently operates with a five-star license, issued 7/29/19. During the visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. You provided the email correspondence of the staff confirming the document was ready and would be turned in on the first day of employment. The chemistry of medication may change over time, causing them to be less effective or potentially cause a reaction. Today we discussed your current practices for monitoring medications. You reported that there had been some recent changes in staff and could not say for sure the new staff had reviewed the medications and authorization form. During the visit you removed the medications and notified the parents that more diaper rash cream and a new authorization form would be required. Review medication administration requirements with your staff and complete regular audits. Storage of Hazardous Items- Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. Today the door to the laundry closet was open, with the key in the lock, and one canister of Zep disinfectant wipes was in an unlocked cabinet of classroom space #1. There is a sign outside the laundry room the says to keep the door locked, however the hook for the lock is broken. Today we discussed reviewing the storage of hazardous items poster with your staff, staff training and the replacement of the lock and/or key storage. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Consultation: Outdoor inspections- The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment. New rubber tile mats have been added to the walkways of the outdoor space. While many of the tile lay flush against on a another, some are uneven and my cause a trip hazard if there is a difference of more than a half inch. When completing the inspections, be sure to pay attention to the rubber tiles as they may shift or settle over time and create trip hazards. Reminders: Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy. T. Mosley, Director, assisted me with today’s visit. Your program currently operates with a five-star license, issued 7/29/19 earning 7 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having 75% of teachers have an AAS or higher in ECE/CD. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled (Creative Curriculum). The center's compliance history was reviewed with the operator. The program’s compliance history was percent as of 10/15/24. The facility is owned by East Carolina University. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. Four groups of children were observed using the indoor and outdoor learning areas. Children participated in group time and indoor free play. Proper handwashing techniques were observed. Teacher sat with the children reading books, playing with them and engaging in conversations with the children. New planter boxes and a rubber tile bike trails have been added to the outdoor learning environment. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. The door was unlocked, with the key in the lock of the laundry closet containing four bottles of bleach, five bottles of GOO Gone, 23 bottles of Connection Impressions Ready to use Sanitizer and canisters of Zep disinfectant wipes. There was one canister of Zep disinfectant wipes in an unlocked cabinet, above five feet, in space #1. .2820(b) 843 A drug or medicine was administered after its expiration date. You reported that there had been some recent changes in staff and could not say for sure the new staff had reviewed the medications and authorization form. During the visit you removed the medications and notified the parents that more diaper rash cream and a new authorization form would be required. Review medication administration requirements with your staff and complete regular audits. Storage of Hazardous Items- Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. Today the door to the laundry closet was open, with the key in the lock, and one canister of Zep disinfectant wipes was in an unlocked cabinet of classroom space #1. There is a sign outside the laundry room the says to keep the door locked, however the hook for the lock is broken. Today we discussed reviewing the storage of hazardous items poster with your staff, staff training and the replacement of the lock and/or key storage. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Consultation: Outdoor inspections- The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment. New rubber tile mats have been added to the walkways of the outdoor space. While many of the tile lay flush against on a another, some are uneven and my cause a trip hazard if there is a difference of more than a half inch. When completing the inspections, be sure to pay attention to the rubber tiles as they may shift or settle over time and create trip hazards. Reminders: Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy. Proper hand washing techniques and diapering procedures were observed. Lunch was observed and consisted of hamburger/veggie burger on a bun, broccoli, peaches, and milk. When medication authorization forms are filled out by parents, you should check that the correct expiration date is noted on the topical ointment permission form and that it matches the expiration date on the medication. If a parent brings in a diaper cream/sunscreen/repellent to replace another, a new form must be filled out for that medication. Thank you for removing these items from the classroom spaces. You shared with me that there are plans for some changes in the outdoor area and the surfacing has been scheduled for delivery in the coming weeks. We discussed ways to ensure the children’s safety during outdoor activities until the surfacing can be delivered, including not using the climber and creating other outdoor interest activities for the children. Additional Comments: Resuming Rated License: During your preparation year, July 1, 2024-June 30, 2025, you may have the Environment Rating Scales completed, review the ITERS-R and ECERS-R by obtaining the manuals for each classroom and looking for assessment related resources at ncrlap.org. Teachers engaged in conversation and play with the children and moving about the outdoor spaces. Upon transitioning into the indoor environment, the children washed hands before engaging in group time and indoor free play. Children’s art work, photos, and educational posters hang on the walls of the classrooms. Children enrolled in space # 4 received care according to individual needs including tummy time, diapering, and bottle feeding. Sleep checks were completed and documented. Proper hand washing techniques and diapering procedures were observed. Lunch was observed and consisted of hamburger/veggie burger on a bun, broccoli, peaches, and milk. The following violation was documented. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #4, authorization for Calmoseptine Diaper Rash Cream expired on 5/14/24. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. In space #1, there was one tube of Eucerin Eczema Relief cream that expired on 3/2024, one tube of Desitin Diaper Rash Cream that expired 1/2024, One tube of Honest Diaper Rash Cream that expired 4/2024, one tube of Boudreaux's Butt Paste that expired on 2/2024 and one tube of Boudreaux's Butt Paste that expired on 3/2024. In space #2, there was one tube of Think Baby Sunscreen that expired on 4/2024 and one tune of Badger Anti Bug Balm that expired 2/2024. 10A NCAC 09 .0803(1)(d) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The depth of surfacing under the climbing structure on space #1, measures one (1) to two (2) inches in the fall zone. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 6/3/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations 4were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: Medication- Administering medication requires skill, knowledge and careful attention to detail. Caregivers/teachers must be diligent in their adherence to the medication administration policy and procedures to prevent any inadvertent medication errors, which may be harmful to the child. Today, there were expired diaper creams in Space #1 and expired sunscreen and bug repellent in Space #2. The permission forms were still in date; however, the creams expiration date stamped on the diaper cream/sunscreen/bug repellent had passed. When medication authorization forms are filled out by parents, you should check that the correct expiration date is noted on the topical ointment permission form and that it matches the expiration date on the medication. If a parent brings in a diaper cream/sunscreen/repellent to replace another, a new form must be filled out for that medication. Thank you for removing these items from the classroom spaces. Return them to the parent or dispose of them within 72 hours. In space #4 the authorization for one diaper rash cream had expired, however the medication has not expired. Thank you for removing the diaper rash cream from the classroom. Please have the parent complete a new form or return the medication to the parent. Today, we discussed your current medication monitoring practices. Complete audits at least monthly to ensure all medications and authorizations are current. Surfacing- Having protective surfacing under and around playground equipment can reduce the risk of serious injury. The surfacing in the fall zone of the stationary climbing structure on outdoor space #1 measures one to two inches. You shared with me that there are plans for some changes in the outdoor area and the surfacing has been scheduled for delivery in the coming weeks. We discussed ways to ensure the children’s safety during outdoor activities until the surfacing can be delivered, including not using the climber and creating other outdoor interest activities for the children. Additional Comments: Resuming Rated License: During your preparation year, July 1, 2024-June 30, 2025, you may have the Environment Rating Scales completed, review the ITERS-R and ECERS-R by obtaining the manuals for each classroom and looking for assessment related resources at ncrlap.org. Request technical assistance and training for your staff from the Martin-Pitt Partnership for Children. You can request and complete an ITERS-R and ECERS-R assessment during your preparation year. If your scores help you meet or exceed your current star rating and you are ready to move forward with your rated license assessment, you may choose to do so during your preparation year. Your preparation year is also the time to work on staff education and ensure that Works accounts are completed and up to date for staff members. Encourage staff to enroll in additional college coursework to meet or increase points in Education. TEACH Scholarships are available to help staff pay for educational expenses. Go to https://www.childcareservices.org/programs/teach-north-carolina/. Staff Education and Works - Formal education (NCECC, college coursework, and Early Educator Certificates) are submitted to DCDEE through WORKS, an on-line portal where you can register, request to be approved for various positions, and access your education information once evaluated and posted. You can now request to have your transcripts sent electronically from colleges attended. Staff education verifying qualification for the position hired, must be submitted to WORKS within six (6) months of hire or change of position. Staff not meeting formal education requirements for specific positions must enroll in required coursework within six (6) months of hire or change of position and complete the NCECC (lead teacher and program coordinator) within 18 months after enrollment or NCECAC (administrator) within 24 months after enrollment. When education is posted, staff need to log back into Works and print a copy of their Education Status Letter for their file. T. Mosley, Director, assisted me with today’s visit. Your program currently operates with a five-star license, issued 7/29/19 earning 7 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having 75% of teachers have an AAS or higher in ECE/CD. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled (Creative Curriculum). The center's compliance history was reviewed with the operator. The program’s compliance history was eighty eight percent as of 12/1/23. The facility is owned by East Carolina University. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Upon arrival, four groups were observed transitioning to do a caroling walk around campus. The groups walked around campus singing Christmas carols and passing out candy canes. Interns and families joined you on your walk. When you returned, groups entered their classroom, washing hands and transitioned to indoor free play. Classrooms were organized in to learning centers including reading, blocks, dramatic play and table-top manipulatives. Low lying shelves were organized with bin to store learning materials and provide easy access for the children. I observed children in both the indoor learning environments and found supervision and staff/child ratios to be in compliance. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One can of Lysol Disinfectant Spray and Zep Disinfectant Wipes were stored in an unlocked cabinet above five feet. .2820(b) 843 A drug or medicine was administered after its expiration date. There was a tube of Aquaphor with an expiration date of 1/2023 located in space #4. 10A NCAC 09 .0803(1)(d) Compliance Verification: Violations cited were corrected during the visit. No further action is needed. Technical Assistance: Medications-Expired medical products can be less effective or risky due to a change in chemical composition or a decrease in strength. Today, I observed one tube Auquaphor, with an expiration date of 1/2023, located in space #4. You stated the child had recently started and the medication must have expired when received. The medication was returned to the parent during the visit. We discussed what it looks like when receiving medication from parents and the way you track and monitor medications. Create a tracking log or schedule to assist you with medications. Storage of Hazardous Items- Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Today I observed one can of Lysol Disinfectant spray and one canister of Zep Disinfectant wipes in an unlock cabinet of space #1. Thank you for locking it during the visit. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. We discussed placing a sign on the outside of the door as a visual reminder to lock the door. Consultation: Transitions-When there are planned events that change the daily routine, it is important to have good planning for the children. Today I observed a group of children having difficulty waiting for their teacher as they transitioned from their morning snack. Teachers can consider transition activities such as putting out a set of toys for the children to play with, books to read or a music and movement activity as they wait for their teacher. This will give them something constructive to do and reduce unwanted behaviors. Reminders: Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy.
