The sanitation inspection was completed 10/10/25 with a “Superior” classification. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of 2/25/26. The NC Secretary of State website was reviewed on 4/15/24 and Mount Calvary F. W. B. Church, Inc. was listed as current- active. Children were observed arriving at the facility from school. Upon arrival children washed their hands and then picked up their snack from the counter. The children engaged in conversation as they sat at the table eating snack. Snack included pretzels and orange juice. The following violation was documented. The following violation was documented. Staff members must be positioned to see and hear children, render immediate assistance and provide for their needs, especially infants and toddlers who depend solely on caregivers. Today, I observed one staff member leave the classroom and enter the restroom, leaving eight children unattended. When asked through the door who was with the children, the staff member exited the restroom and called for another staff member, then returned to the restroom before the other staff member could enter the classroom space. Technical assistance was provided about giving staff regular break times, using a walkie-talkie or intercom system to call for additional support and having staff to remain with the children until someone is present to relieve them. Due to the nature of this violation, a follow up visit will be completed to verify compliance. Medication Authorization- All medications must be accompanied by written authorizations from parents with complete instructions for administration. Today, I observed Eucerin cream with an authorization that was not completed correctly. You stated that medications are received by staff and the administrator. We discussed completing a review medication requirement with staff and periodically check behind them to ensure all requirements are met. Electrical Outlets- Children are curious about their environment and explore by touching the items within their reach. Providers are responsible for ensuring that the spaces used by children are safe and ready for this type of exploration every day care is provided. Today, I observed one broken and one missing wall plate from two electrical outlets in the classroom space. When teachers notice items that are broken or damaged, they need to report them immediately and take precautions to ensure the children’s safety. During the visit, you moved shelves in front of the outlets to make them inaccessible as you wait for maintenance to replace the missing and broken wall plates. Emergency Drills- Emergencies occur without warning. Drills are conducted to ensure everyone knows what to do and where to go in the event of an emergency. You must conduct either a shelter-in-place or a lockdown drill every three months as well as a monthly fire drill. The log reflects an lock down drill was completed on 6/4/24 and a shelter-in-place drill was completed on 10/11/24. You stated that not completing a drill in September was an oversite and when it was brought to your attention you immediately completed a drill. Setting reminders on your calendar or having a digital reminder can help you track and complete drills as required. Additional information: Activity Plans- Activity plans are detailed plans which outline activities that will encourage children’s growth and development. The activities outlined on the activity plan will typically occur during the “Activity Center” times indicated on the daily schedule. While you are not required to have an activity plan posted, you have opted to have them. Today I observed an activity plan posted that was dated 10/7/24-10/11/24. It was reported that an activity plan was not available for this current week. Have staff plan ahead and have activity plans written out at least one week in advance. Staying consistent with posting plans will provided children and families the opportunity to know what is coming and build joy and excitement. They must be posted for easy reference by caregivers and parents. Parents/guardians and prescribing health professionals must give a caregiver/teacher written authorization to administer medication to the child. Caregivers/teachers must be diligent in their adherence to the medication administration policy and procedures to prevent any inadvertent medication errors, which may be harmful to the child. There is always a risk that a child may have a negative reaction to a medication, and children should be monitored for serious side effects that may require an emergency response. We discussed that when determining medication expiration, you must review the discard date on the prescription label as well as the manufacture date, the expiration date is determined by which date in chronologically first. You reviewed this with staff during the visit. Staff also placed a name on the jar of Eucerin during the visit. Outdoor Environment- The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. You stated that you would contact maintenance to have the grass cut and fix the fence and that you would remove the net. Incident Log - You must document every injury incurred while a child is in your care, including cuts, scrapes, bumps, and bitemarks. These incidents must be recorded on the incident report form supplied by the Division. The report must include all required information and be signed by the parent. The incident must then be logged on the facility incident report log and the completed report placed in the child’s file. The incident log should be used to track patterns of injuries and adjust routines or the learning environment to prevent further injuries. It must be kept with other program records for review during monitoring and complaint visits. Check incident logs regularly to ensure staff are documenting incidents as required. B. Weaver, administrator assisted me with today’s visit. Your program currently operates with a GS 110 Notice of Compliance, issued 7/15/19. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-four percent as of 11/22/23. The NC Secretary of State website was reviewed on 11/22/2023 and Mount Calvary F. W. B. Church Incwas listed as current- active. A walk-through of the facility was completed today, all indoor areas were monitored. Children were observed during free play with age -appropriate materials. School age children were observed using writing materials. Teachers were engaged in activities with the children and redirecting children when needed.
