B. Weaver, the administrator, assisted me with the visit. Your program currently operates with a notice of compliance, issued 9/17/2011. The last annual compliance visit was conducted 3/12/2025. The sanitation inspection was completed 4/11/25 with a “Superior” classification. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight percent as of 2/25/26. The NC Secretary of State website was reviewed on 2/25/2026 and Mount Calvary F. W. B. Church, Inc. was listed as current- active. I visited indoor and outdoor spaces. Children enrolled in classroom space #1 were engaged in free play. In classroom spaces #3 and 4 children were observed during group time. Teachers asked the children open ended questions, named words that start with the letter T, named colors and shapes. Lunch was observed and included peas, pears, macaroni and cheese, sliced wheat bread and milk. The following violations were documented. Teachers sat with children that were awake, comforting them and talking softly. Staff were observed preparing the classroom with materials for an art activity to be completed after rest time. The following violations were observed Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There were three active wasp nest on the side of the building on outdoor space #1. On outdoor space #2, there was one large active ant hill near the fence, at the rear of the playground. There were 5 vines, one with thorns, along the fence, near the slide. There is a cactus on the outside of the fence of outdoor space #2 that has thorns that reach through the fence. 10A NCAC 09 .0601(a) 828 Air conditioning units were accessible or did not have a guard to keep objects from being thrown into the unit. Since children are exploring it is very important to provide a safe environment. Children had access to unlocked gated areas that surrounded the HVAC units. The play areas contained an active ant hill and there active wasps nest. Harmful pants were also growing along the fence of outdoor space #2. We discussed the importance of checking the outdoor play areas every morning prior to the children arriving at the facility. We also discussed that staff report any harmful items to the administrator to have it addressed immediately. Emergency Drills- Emergencies occur without warning. B. Weaver, administrator assisted me with today’s visit. Your program currently operates with a notice of compliance issued on 9/17/2011. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of 12/4/23. The NC Secretary of State website was reviewed on 12/4/23 and Mount Calvary F.W.B Church, INC. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor classroom spaces. Children rested in approved cribs and on linen covered mats. Sleep checks were completed, recorded, and maintained as required. Files for new staff were reviewed. A fire inspection have not occurred since the last visit and remain current. A sanitation inspection was conducted at the facility on 11/29/23 with a "Superior" classification. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the infant and toddler playground there was portable climber with a broken slide. On the preschool playground the top of the water car of the train was broken. The metal bolts of the airplane were protruding. 10A NCAC 09 .0601(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space #1 a tube of Nystatin was not in it's original packaging with the prescription label. .0803(2)(a) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Playground inspections were not provided, the administrator stated they were not updated. .0605(q) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Staff R.
