C. Rouse, director, and A. Chalmers, administrative assistant, assisted me with the visit. Your program currently operates with a three-star license, issued on January 10, 2026, using the Creative Curriculum, meeting two additional Family and Community Engagement Standards and having a CQI Plan. The last temporary time period visit was conducted on December 17, 2025. The sanitation inspection was completed on February 11, 2026 with a “Superior” classification. The last fire inspection was conducted on May 27, 2026 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-nine percent as of June 10, 2026. The NC Secretary of State website was reviewed on June 10, 2026, and Moms Tiny Treasures (MTT) Academy, LLC was listed as current- active. You were emailed a facility application and appendices to update the Owner information. The contact information for the facility was reviewed with Ms. Rouse and all information was verified. I visited both indoor and outdoor spaces. Children enrolled in classroom space #3 were observed transitioning to an activity with paper and crayons. In classroom space #4 children were observed during free play, as the teacher sat with one child completing a math activity. Lunch was observed and consisted of corndogs, baked beans, and applesauce. Children with nutrition opt out form on file, ate meals from home. The following violations were documented. Violation Number Comment Rule 501 Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. There are active ant hills in the grass area, and the mulch was a light covering and mixed with sand in the fall zones. We discussed that one type of surfacing should be used and must be at least 6 inches depending on height of the stationary equipment. There was a large root from a tree between the stationary structure and fence that created a tripping hazard. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Cavi Wipes were stored in the cabinet below the sink not locked. These wipes contain additional warnings and must be stored in locked storage. Diapers in plastic bags and children’s clothes in a plastic bag were stored inside children’s cubbies below five feet in the unlocked closet in classroom space #3. Plastic bags are a suffocation hazard for young children and should be made inaccessible to children less than three years of age. To make plastic bags inaccessible, they will need to be stored out of reach of children, which is above five feet from the finished floor. Incident Reports and Log : You must now document every injury incurred while a child is in your care, including cuts, scrapes, bumps, and bitemarks. These incidents must be recorded on the incident report form supplied by the Division. The report must include all required information and be signed by the parent. The incident must then be logged on the facility incident report log and the completed report placed in the child’s file. The facilities incident logs were reviewed and eighteen incident forms on file were not included on the log. Designate a staff to maintain incident reports and the incident log. Staff Records: The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation to include emergency contact information. During today’s visit a therapist was on site working with a child in classroom space #1, an emergency information form was not on file. Have the therapist complete the form and store with your staff files. Additional Comments Transportation was monitored, the facility van was not onsite, staff stated the van was receiving maintenance. A rental SUV was being used to transport children. Your facility was approved for transportation during an other visit conducted at the facility 9/2/25. Transportation was added to the facilities information during today’s visit. Reminders Sex Offender Registry: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. According to the posted menu, the lunch served consisted of meatballs, beans, and apples. Today, children were served corndogs, baked beans and applesauce. Prior to any meal or snack being served, the posted menu must be reviewed for accuracy of items served and if comparable changes occur, then make sure those changes are noted on the posted menu(s). Nutrition Opt-Out: When a child's parent opts out of the supplemental food provided by the center, the parent shall sign a statement acknowledging the parental decision shall be kept in the child's file at the center and a copy provided to the parent. A child enrolled on 10/20/25 during lunch was eating food provided by the parent. We discussed that the child’s infant feeding plan did not serve as the child’s opt out form. You stated that you would contact the parent to complete the form today. Nutrition: Nourishing and attractive food is the cornerstone for children’s health, growth, and development as well as developmentally appropriate learning experiences. Nutrition and feeding are essential and required in every facility. Meals and snacks served to children in a child care center shall comply with the Meal Patterns for Children in Child Care Programs from the United States Department of Agriculture (USDA) which are based on the recommended nutrient intake judged by the National Research Council to be adequate for maintaining good nutrition. Lunch should include all five of the five meal components. Today children were served corndogs, baked beans and applesauce. Milk was not served. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. There are active ant hills in the grass area, and the mulch was a light covering and mixed with sand in the fall zones. We discussed that one type of surfacing should be used and must be at least 6 inches depending on height of the stationary equipment. There was a large root from a tree between the stationary structure and fence that created a tripping hazard. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Cavi Wipes were stored in the cabinet below the sink not locked. These wipes contain additional warnings and must be stored in locked storage. Diapers in plastic bags and children’s clothes in a plastic bag were stored inside children’s cubbies below five feet in the unlocked closet in classroom space #3. Plastic bags are a suffocation hazard for young children and should be made inaccessible to children less than three years of age. To make plastic bags inaccessible, they will need to be stored out of reach of children, which is above five feet from the finished floor. Incident Reports and Log : You must now document every injury incurred while a child is in your care, including cuts, scrapes, bumps, and bitemarks. These incidents must be recorded on the incident report form supplied by the Division. The report must include all required information and be signed by the parent. The incident must then be logged on the facility incident report log and the completed report placed in the child’s file. The facilities incident logs were reviewed and eighteen incident forms on file were not included on the log. Designate a staff to maintain incident reports and the incident log. Staff Records: The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation to include emergency contact information. During today’s visit a therapist was on site working with a child in classroom space #1, an emergency information form was not on file. Have the therapist complete the form and store with your staff files. Additional Comments Transportation was monitored, the facility van was not onsite, staff stated the van was receiving maintenance. A rental SUV was being used to transport children. Your facility was approved for transportation during an other visit conducted at the facility 9/2/25. Transportation was added to the facilities information during today’s visit. The allegations are as follows: There is a concern regarding outdoor safety and nutrition. Regarding the allegation of outdoor safety, I reviewed outdoor inspections, and I interviewed staff members and administrators. You and the staff indicated a sweep of the outdoor occurs each morning prior to the arrival of the children. You implement a daily inspection checklist and inspect the outdoor space three times per day because the children go outside twice a day. I reviewed the inspection checklist which lists items found that you feel pose a threat like mushrooms, cobwebs and limbs. I also monitored your playground inspection checklist and you have identified hazards and removed them. Based on the review of documents, outdoor monitoring and interviews the allegation of unsafe outdoor is "unconfirmed." Regarding the allegation of nutrition, the facility does not prepare meals. All children’s files were monitored and nutrition-opt out forms are completed. Based on the review of children's records and interviews the allegation is "unconfirmed." I completed limited monitoring of child care requirements during the visit. Space 1; Ratio 1:5; (4 present; with 1 teacher) COMPLIANCE HISTORY - A violation was documented. Nutrition Opt-Out – “Opting-out" of the meals served in facilities is a parent choice. Your facility does not provide meals currently and therefore; opting-out, is not an option. Each child enrolled must have an opt-out form signed by parents agreeing to bring in all meals, beverages and snacks for their children. One child enrolled requiring food other than formula, did not have an opt-out form available. You indicated the form was in the enrollment packet but was not returned by the parent. You requested the completed packet. It is important to complete the enrollment process with each family. You will then be available to answer all questions they may have and ensure a complete enrollment packet is received. Orientation- Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. Documentation of 6 hours of orientation within the first 2 weeks of employment was not on file for the individual hired on July 21, 2025. You indicated the orientation process is completed by the other administrator. Because there are two administrators, it is important for both of you to review all files as they are received. Medications - Administering medication requires skill, knowledge and careful attention to detail. A jar of A & D Ointment was present for the enrolled infant in space 1; however, permission to apply the ointment was not on file. You indicated the enrollment process has been in the hands of multiple individuals, and you are unsure of the whereabouts of the form. It is important as the administrator to review files during the enrollment process to ensure the child has everything needed. ITS-SIDS – Early childhood teachers touch the lives of young children and their families in many important ways. One of the most important ways to reduce infant deaths in child care settings is for teachers to implement (and model for parents/guardians and families) safe sleep practices in their programs. Safe sleep practices include placing infants on their backs to sleep, using safe cribs and equipment for sleeping, prohibiting smoking, and training teachers on appropriate safe sleep practices. Assigned staff who work alone in the infant space must have a current ITS-SIDS training certificate. The individual in space 1 caring for the infant today does not have a current ITS-SIDS certificate on file. You indicated that the file checklist allows for the completion of the training within 2 months. You are correct; however, someone must have the training in the space while other staff work towards completion. You reassigned the space to a staff person on site today that does have a current ITS-SIDS certification. Health Care Needs - Emergency information is the key to obtaining needed care in emergency situations. In the event of an emergency, child care providers must know about a child’s special health care needs, so they can offer protection for that child. This requires you to obtain the health and safety needs of individual children prior to enrollment. The health care needs section of the enrollment application for 2 out of the 2 children enrolled was blank. We reviewed the applications together and you took notes of the requirements for the completion of each section. A nutrition opt out form was unavailable for one child present with lunch, a beverage and snack brought from home. .0901(d) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. The only teacher present in space 1 designated for the care of infants did not have a current ITS-SIDS training certificate. .01102 (f) 1832 Application did not include health care needs or concerns, symptoms of and the type of response required for the health care needs or concerns. Two out of two children's files did not have the health care needs section of the application for enrollment completed. .0801 (a)(5) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Twelve month standing authorization was not obtained for the application of A & D Ointment observed in space 1. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) * The violation(s) documented must be corrected immediately. On or before September 3, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: Record Keeping - Operational control to accommodate the health and safety of individual children and staff requires that information be kept and made available on a need-to-know basis. These records and reports are necessary to protect the health and safety of all in individuals in the facility. Staff files for both administrators were not available in the facility. You contacted the owner, who is also an administrator for the facility, who indicated the files were with her. Additionally, one individual hired on 7/21/25 did not have a medical report on file. Medical reports must be on file prior to employment for all staff. All staff, children and program files must be kept in the facility and made available for review. Nutrition Opt-Out – “Opting-out" of the meals served in facilities is a parent choice. Your facility does not provide meals currently and therefore; opting-out, is not an option. Each child enrolled must have an opt-out form signed by parents agreeing to bring in all meals, beverages and snacks for their children. One child enrolled requiring food other than formula, did not have an opt-out form available. You indicated the form was in the enrollment packet but was not returned by the parent. You requested the completed packet. It is important to complete the enrollment process with each family. You will then be available to answer all questions they may have and ensure a complete enrollment packet is received. Orientation- Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. Documentation of 6 hours of orientation within the first 2 weeks of employment was not on file for the individual hired on July 21, 2025. You indicated the orientation process is completed by the other administrator. Because there are two administrators, it is important for both of you to review all files as they are received. Medications - Administering medication requires skill, knowledge and careful attention to detail. A jar of A & D Ointment was present for the enrolled infant in space 1; however, permission to apply the ointment was not on file. You indicated the enrollment process has been in the hands of multiple individuals, and you are unsure of the whereabouts of the form. It is important as the administrator to review files during the enrollment process to ensure the child has everything needed. ITS-SIDS – Early childhood teachers touch the lives of young children and their families in many important ways. One of the most important ways to reduce infant deaths in child care settings is for teachers to implement (and model for parents/guardians and families) safe sleep practices in their programs. Safe sleep practices include placing infants on their backs to sleep, using safe cribs and equipment for sleeping, prohibiting smoking, and training teachers on appropriate safe sleep practices. Assigned staff who work alone in the infant space must have a current ITS-SIDS training certificate. The individual in space 1 caring for the infant today does not have a current ITS-SIDS certificate on file. You indicated that the file checklist allows for the completion of the training within 2 months. You are correct; however, someone must have the training in the space while other staff work towards completion. You reassigned the space to a staff person on site today that does have a current ITS-SIDS certification. Health Care Needs - Emergency information is the key to obtaining needed care in emergency situations. In the event of an emergency, child care providers must know about a child’s special health care needs, so they can offer protection for that child. This requires you to obtain the health and safety needs of individual children prior to enrollment. The health care needs section of the enrollment application for 2 out of the 2 children enrolled was blank. We reviewed the applications together and you took notes of the requirements for the completion of each section. Activity Plans – Preschool lesson plans lay the groundwork for a child's initial entry into the world of learning. They act as roadmaps, guiding young learners through the various stages of early academic growth. An activity plan for Space 4 was not available. You indicated the teacher was hired on Monday August 18th and started in the classroom on Wednesday August 20th. You indicated that because the individual is still in training, the activity plan has not yet been completed. Children are enrolled; therefore, you must complete weekly activity plans that are current and include developmentally appropriate activities for enrolled children. Activity plans for all children must include daily activities to stimulate the 4 of the 4 developmental domains, as listed in your NC Foundations for Early Learning and Development book: (social & emotional development, health & physical development, approaches to play and learning, language development/communication, and cognitive development). It must also include a daily gross motor activity which may occur indoors or outdoors. The activity plan must include both free choice and adult-directed activities, which allow children to choose to participate with the whole group, in a small group or independently.
