A. McClain, operator, assisted me with the visit. Your program currently operates with a five-star license, issued 12/12/2025 earning 7 points in the education component, 5 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under age one. The last temporary time period visit was conducted 10/30/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent as of 4/28/26. The NC Secretary of State website was reviewed on April 28, 2026, and Little Explores Daycare LLC was listed as current-active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website. The information is current. You were caring for three children one to four years of age. You sat with the children at a child sized table as the children played with magnetic shape tiles. You engaged in conversation and assisted the children as needed. Later the children were observed during outdoor play. Lunch was observed and included chicken alfredo, sweet peas, apple sauce and milk. Following lunch the children rested on linen covered cots. The following violations were documented. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. There were dog feces in the outdoor play space. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. We discussed cleaning the outdoor space daily. I walked through the entire facility and observed the indoor and outdoor environment. The children cleaned the indoor space and transitioned to outdoor play. The fenced outdoor space located at the rear of the facility included portable toys and materials. Lunch included corndogs, peas & carrots, peaches and milk. After the lunch, diaper checks were completed, hands were washed and the children rested on linen covered cots. Music played softly in the background. The following violations were documented. On or before November 13, 2025, you must provide written documentation indicating how and when the violations were corrected. In the event of an emergency, child care providers must know about a child’s special health care needs, so they can offer protection for that child. This requires you to obtain the health and safety needs of individual children prior to enrollment. The health care needs section of the enrollment application for 2 out of the 6 children enrolled was not complete. You indicated did not indicate why. Ensuring the application is complete during enrollment allows time for parents to complete required information. Storage of Hazardous Items - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. All children must be cared for in safe environments and protected from the risk of potentially dangerous products. The cabinet underneath the sink in the kitchen was unlocked. It contained bleach and dish detergent. You indicated the cabinet does have a magnetic lock on it but appeared not strong enough to remain locked. A can of Axe Dark Temptation body spray was stored on an open shelf in in the laundry. You indicated you thought it had to be stored at five feet. All aerosol cans are highly flammable and present a hazard because of the compressed gas used as a propellant. If punctured, the contents may be released so forcefully that injuries can result. All aerosols must be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. Rated License – You signed the Initial Application for Assessment for a Two Component Star Rated License and completed the Rated License Assessment Request Review form on June 11, 2025. The facility will be assessed based on requirements for a two-component rated license. You submitted official transcripts to WORKS on 10.29.25. Once your education has been evaluated your star rating will be determined. We reviewed the Facility Summary Report for your FCCERS-3 Assessment that occurred on October 7, 2025. The total score was 5.19. Items scoring less than a three could constitute a violation of child care requirements and therefore were monitored and noted below. These items are noted below: Item 7: Indicator 1.3 (Score 1pt) Assessor Comments: There were animal droppings on the playground. Substances or materials that could be unsafe or unhealthy for the children should not be in their play area. Observations today: I did not observe animal droppings on the outdoor learning environment. You indicated you inspect the outdoor space daily and somehow missed this on the day of the assessment. Discussion: We discussed at the issuance of your star rated license you will be licensed for ten children as there are no zoning ordinances to prohibit. A family child care home may provide care for one of the following groups of children, including the operator's own preschool age children and excluding the operator's own school-age children up to 13 years of age: A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. Mixed age groups consisting of children aged zero-twenty four months, children aged 2-5 years, and school-aged children will be monitored as follows: A maximum of nine children, with no more than three children aged zero to twenty-four months and of the six additional children, three must be school age. A maximum of 10 children if all children are older than 24 months of age. You must complete all of the health and safety training within one year of employment and as part of on-going training so that every five years, all the topics have been covered. After the first year of employment, you must complete at least 8 on-going training hours. Emergency Preparedness and Response - Within one year of the effective date of a new license, must have completed the Emergency Preparedness and Response in Child Care training. Upon completion of the Emergency Preparedness and Response in Child Care training, you must develop the Emergency Preparedness and Response Plan. The Emergency Preparedness and Response Plan means a written plan that addresses how a child care facility will respond to both natural and man-made disasters, such as fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disaster, or a dangerous person in the vicinity, to ensure the safety and protection of the children and additional caregivers. This Plan must be on a template provided by the Division available at https://rmp.nc.gov/portal/# completed within four months of completion of the Emergency Preparedness and Response in Child Care training, and available for review. In the event of an emergency, child care providers must know about a child’s special health care needs, so they can offer protection for that child. This requires you to obtain the health and safety needs of individual children prior to enrollment. The health care needs section of the enrollment application for 2 out of the 6 children enrolled was not complete. You indicated did not indicate why. Ensuring the application is complete during enrollment allows time for parents to complete required information. Storage of Hazardous Items - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. All children must be cared for in safe environments and protected from the risk of potentially dangerous products. The cabinet underneath the sink in the kitchen was unlocked. It contained bleach and dish detergent. You indicated the cabinet does have a magnetic lock on it but appeared not strong enough to remain locked. A can of Axe Dark Temptation body spray was stored on an open shelf in in the laundry. You indicated you thought it had to be stored at five feet. All aerosol cans are highly flammable and present a hazard because of the compressed gas used as a propellant. If punctured, the contents may be released so forcefully that injuries can result. All aerosols must be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. Rated License – You signed the Initial Application for Assessment for a Two Component Star Rated License and completed the Rated License Assessment Request Review form on June 11, 2025. The facility will be assessed based on requirements for a two-component rated license. You submitted official transcripts to WORKS on 10.29.25. Once your education has been evaluated your star rating will be determined. We reviewed the Facility Summary Report for your FCCERS-3 Assessment that occurred on October 7, 2025. The total score was 5.19. Items scoring less than a three could constitute a violation of child care requirements and therefore were monitored and noted below. Substances or materials that could be unsafe or unhealthy for the children should not be in their play area. Observations today: I did not observe animal droppings on the outdoor learning environment. You indicated you inspect the outdoor space daily and somehow missed this on the day of the assessment. Discussion: We discussed at the issuance of your star rated license you will be licensed for ten children as there are no zoning ordinances to prohibit. A family child care home may provide care for one of the following groups of children, including the operator's own preschool age children and excluding the operator's own school-age children up to 13 years of age: A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. Mixed age groups consisting of children aged zero-twenty four months, children aged 2-5 years, and school-aged children will be monitored as follows: A maximum of nine children, with no more than three children aged zero to twenty-four months and of the six additional children, three must be school age. A maximum of 10 children if all children are older than 24 months of age. Annual Licensing Fees - The Division of Child Development and Early Education assesses an Annual License Fee to active child care providers in accordance with North Carolina General Statute § 110-90(1a). This fee applies to the current calendar year (January - December 2025). Payments must be submitted online only no later than December 31, 2025. The current Annual License Fee Invoice Amount for family child care homes is $52.00. Moodle Support – The Division offers early childhood professionals a wide range of professional development opportunities through our online learning platform Moodle. You must complete all of the health and safety training within one year of employment and as part of on-going training so that every five years, all the topics have been covered. After the first year of employment, you must complete at least 8 on-going training hours. Emergency Preparedness and Response - Within one year of the effective date of a new license, must have completed the Emergency Preparedness and Response in Child Care training. Upon completion of the Emergency Preparedness and Response in Child Care training, you must develop the Emergency Preparedness and Response Plan. The Emergency Preparedness and Response Plan means a written plan that addresses how a child care facility will respond to both natural and man-made disasters, such as fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disaster, or a dangerous person in the vicinity, to ensure the safety and protection of the children and additional caregivers. This Plan must be on a template provided by the Division available at https://rmp.nc.gov/portal/# completed within four months of completion of the Emergency Preparedness and Response in Child Care training, and available for review. ABCMS – As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired within five business days. The process of notifying the Division is captured in ABCMS. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days.
