M. Cherry assisted me with today’s visit. Your program currently operates with a notice of compliance issued 11/16/2018. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five percent as of 2/19/25. The NC Secretary of State website was reviewed on 4/8/26 and Koinonia Christian Center Childcare Services, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. You informed me today that Dr. Boykin is assisting with the facility. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children throughout the classroom spaces engaged in free play with age appropriate materials. In classroom space #3 children under one year of age received care according to their needs including diapering. Child rested in cribs, sleep charts were observed. Children enrolled in classroom space #7 were observed in the church sanctuary practicing graduation songs, scripture and prayer. A sanitation inspection was conducted at your facility on 12/29/25 with a Superior classification and a fire inspection was completed on 9/19/25 and your facility is approved for day time and night care. The violations are as follows: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. The allegations are as follows: There is a concern that a child was handled roughly. Enrollment: Space #5 Ten children three years of age enrolled: nine children three years of age present. Staff: L. Jordan and K. Brown B. Peterson, administrative assistant and D. Harris- Mackey, Parent Consultant, assisted me with today’s visit. Staff/child ratio, group size, supervision, use of licensed space, space capacity, and license restrictions were monitored. Children were observed sitting at child sized tables finishing lunch, handwashing, completing bathroom routines and resting on linen cover cots. Staff assisted children with handwashing and bathroom routines. Based on the information provided regarding concerns that a child was handled roughly, I observed video recordings of classroom space #5 from November 24, 2025. A self-report was received from your facility on December 9, 2025. Staff assigned to classroom space #5 were interviewed along with office staff. In the video recordings the staff member and child are observed standing in front of the bathroom doorway in classroom space #5. The staff member’s body language indicates that the staff member was speaking to the child while gesturing towards the bathroom three times. The staff member then steps toward the child placing a hand on the child’s back, below the neck, between the child’s shoulders and pushed the child toward the bathroom. The child takes a couple of steps then falls to the floor. Based on collateral reports and observations the allegation is confirmed. Violation Number Comment Rule 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. On November 24, 2025 a staff member pushed a child three years of age in the back in classroom space #5. .1803(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Discipline is an ongoing process to help children develop inner control so they can manage their own behavior in a socially approved manner. Train staff on using positive redirection techniques. You informed the staff during today’s visit that their letter was expired, and a new letter was required. Submit a copy of the staff’s qualification letter upon receipt. We discussed that this staff member will have fifteen days (August 8, 2025), to receive an updated qualifying letter, or she will not be able to return to work until the letter is received. Reminders: DCDEE has launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to, see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff members, and run a printable report of the staff roster to assist with compliance visits. You completed the ABCMS Provider Portal training. Continue to create your facility roster.
