Your program currently operates with a three-star license, issued 9/28/2025 earning 4 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under age one. The last temporary time period visit was conducted 9/2/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent as of 3/2/26. The NC Secretary of State website was reviewed on 3/2/26 and your facility is not currently owned by a corporation. Your facility is co-owned by I. Ward-Frazier and L. Ward. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website. The information is current. Contact me anytime you need to make any changes to your contact information, e.g., phone numbers, mailing address. I visited indoor and outdoor spaces. You were caring for three children zero to two years of age. C. Ward was also present in the home and assisted you with caring for the children and preparing lunch. The children engaged in free play with age-appropriate materials, in the room designated as the child care space. You engaged in conversation with the children singing songs, and comforting children. Lunch was observed and included spaghetti, corn, apples and milk. The following violations were documented. On the kitchen counter top there were two boxes of Parent's Choice Gas Relief Drops. .1719(a)(5) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A can of Wizard Double Action 2n1 spray, Febreze Air, True Living Coastal Fog Air freshener spray, and Febreze Fabric spray were stored in an unlock drawer of the diaper changing table. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. The family child care home was issued a temporary license on March 27, 2025, with a restriction of first shift care and serving a maximum of 5 preschool children at any time. I walked through the entire facility and observed the indoor and outdoor environment. Violations observed during the April 29, 2025 visit have been confirmed corrected. Locked Storage for Hazardous Products - Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Two (2) bottles of BIC Cover-it Correction Fluid were observed in a basket on the table in the dining room. The warning label contains more than two warnings and require locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. You stored the product in a locked box during the visit. You, I. Ward Frasier and L. Ward, Owners assisted me with the visit. This facility was issued a temporary license on March 27, 2025. During this visit, we walked through the entire facility, observed the indoor and outdoor environment, and completed a full assessment of the licensed space. Children were observed engaged in indoor and outdoor free play activities, personal care routines, and group time. The operator records were reviewed to ensure compliance. Lunch included a ham and cheese sandwich, green peas, peaches and milk. The following violations were documented. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. A monthly check for outdoor hazards was not on file for the month of March 2025. 10A NCAC .1721(e)(5)(A-F) 1741 Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. One out of the three children enrolled did not have a signed animal acknowledgement statement from the parent on file. 10 NCAC 09 .1719(b)(3) 1853 The operator did not conduct a monthly fire drill. Documentation of a monthly fire drill was not on file for the month of March 2025. .1719(a)(15) & .1721( e)(2) 1979 Unflavored skim milk and/or unflavored low-fat milk, was served to children under the age of 24 months. Whole milk was served to a three year old child, .1706(u)(5) * Your temporary license expires September 27, 2025. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 13, 2025, I must receive written documentation of how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance with the Documented Violation (s): Pet Acknowledgement - Prior to enrollment of children in a family child care home, and before new animals that will be in the home come into the family child care home, a parent of each child must sign a form acknowledging the type of animal and where the animal will be during operating hours. A pet acknowledgement was not available for one out of three children’s files reviewed. You indicated, it did not register that the acknowledgement was not in the file. Because this is your child care business, it is important that you review all files received to ensure accuracy. Outdoor Inspections - Outdoor inspections must be completed and documented monthly on the Outdoor Inspection Checklist for Family Child Care Homes form. Documentation of the completion of a monthly outdoor inspection was not on file for the month of March 2025. The monthly checklist must be maintained for 12 months in your files. You indicated you had just opened and it was only a day or two left in the month you could complete the inspection in April. Completing your fire drill and monthly playground inspections on the same day is a great way to ensure compliance with this requirement. Fire Drills - Facilities must develop a plan for responding in the event of a fire in or near the facility that includes your responsibilities and protocols regarding evacuation and notifying emergency personnel and families. Documentation of the completion of a fire drill for the month of March 2025 was not available for review. You indicated you had just opened and it was only a day or two left in the month you could complete the inspection in April. Planning drills on your activity plan each month will help to ensure they are completed within required timeframes. Nutrition Requirements – Nourishing meals are the cornerstone of children’s health, growth, and development, as well as developmentally appropriate learning experiences. Nutrition and feeding are fundamental and required in every facility. A three-year-old child, was served whole milk. You did not indicate why. Because children grow and develop more rapidly during the first few years after birth than at any other time, a child’s facility must provide food and beverages that are adequate in amount and type to meet each child’s growth and nutritional needs. You must serve unflavored skim or lowfat milk for children 24 months through five years. Additional Comments/ Reminders: Be knowledgeable of the child care laws and rules, review the with your center staff, and assist your staff with maintaining compliance with the laws and rules.
