S. Smith and T. Tyson, administrators, assisted me with the visit. Your program currently operates with a three-star license, issued on January 3, 2026, using the Smart Moves and Triple Play Curriculum, meeting two additional Family and Community Engagement Standards and having a CQI Plan. The last temporary time period visit was conducted on December 2, 2025. The sanitation inspection was completed on May 25, 2026 with a “Superior” classification. The last fire inspection was conducted on August 20, 2024 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five percent as of June 9, 2026. The NC Secretary of State website was reviewed on June 9, 2026, and Boys & Girls Clubs of the Coastal Plain was listed as current- active. You were provided with a facility application to update the Owner Name. You shared there was a new board member, please submit an updated Appendix F with your facility application. The contact information for the facility was reviewed with Ms. Smith and Ms. Tyson and all information was verified. The facility address was updated from 475 Belvoir Road to 475 Belvoir Hwy. No children were present during today’s visit. A return visit will be made to observe children in care and to monitor children’s records. The following violations were documented. Being trained in CPR and first aid can be invaluable when someone is in serious medical distress. Therefore, it is so important that all staff working directly with children have current CPR and First Aid Certification. Staff have 90 days from the day of hire to complete the required trainings. A staff member was hired on 1/26/26 and completed CPR and First Aid training on 5/14/26. We discussed having staff complete the training as part of orientation to ensure the requirement is met. Emergency Drills: Caregivers and teachers should teach children the skills necessary to respond to a need to shelter in- place and should teach those skills before children participate in drills. You must conduct either a shelter-in-place or a lockdown drill every three months. Consider marking your calendar as a reminder of when drills are due. The last emergency drill was completed on 12/19/25 based on the emergency documentation log. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked regularly and proper upkeep is conducted to ensure the environment is safe for use. A monthly inspection was available for May 2026. You stated that the playground was closed during May 2026 because a shade structure was being installed. We discussed that even when the playground is closed it should be inspected for safety hazards. Fire Inspection: Facility safeguarding is not achieved by one agency carrying out a single regulatory program. Total safeguarding is achieved through a multiplicity of regulatory programs and agencies including sanitation and fire inspectors. Fire inspections should be completed annually and within 12 months of the previous inspection. The current inspection on file was completed on 3/4/25 and an updated fire inspection was not on file. You contacted facility maintenance during the visit to request an inspection. You stated that typically you would get a call, and a call was not received so the inspection was missed. Schedule a reminder for the month that the fire inspection is due annually. Additional Comments Today we discussed meals for the facility. Lunch meals will be provided by Pitt County Schools Monday- Thursday. We reviewed the nutrition rules regarding lunches being brought from home. We also discussed storage of children's personal belongings. Reminders Sex Offender Registry: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. During this visit, we walked through the entire facility observing the indoor environment, but a full assessment was not completed for each space. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. Children were observed engaged in tutoring, personal care routines, and group time. Program records, including fire drills and monthly outdoor area inspections were complete and current. The afternoon meal included sunflower seeds, sun butter, vegetable juice, craisins, honey graham crackers and milk. Compliance History - The compliance history for this facility prior to today’s visit is 97%. The violation(s) documented may impact the compliance history score and must be corrected immediately. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The medications stored at your facility must be reviewed regularly to ensure they have not expired. Administering a medication that has expired could cause harm to a child. One (1) inhaler expired on 8/22/25. The expired medication must be returned to the parent or discarded immediately. Per our discussion, you must create a plan to review medications, permission forms, and medical action plans to ensure they are current and accurate. Orientation- Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. One employee hired on 10/27/25 did not have documentation of 6 hours of orientation within the first 2 weeks of employment. Effective employee orientation makes employees aware of company policies and expectations, handles essential paperwork, and answers any questions or concerns they may have before they transition into their new positions. Orientation is important because it signifies the beginning of the relationship between employee and employer. Rated License – You the Application for Assessment for a Rated License today. We reviewed your family and community engagement standards and the facility continuous quality improvement plan. You indicated you are applying for the 3-Star level of Pathway 2- Classroom and Instruction Quality until you receive a determination of the approval of the curriculum utilized by the Boys and Girls Clubs of the Coastal Plains. Education: You have three staff that you are completing WORKS accounts for tomorrow to get them group leader qualified. A new license effective January 3, 2026 will be mailed directly to you and must be posted upon receipt. Health & Safety Training - All staff must have completed the required applicable Health and Safety Training (s) and have documentation on file for review within 1 year of employment and every five years thereafter. On-Going Training requirements must be completed and documentation of each training in file for review after the first year of employment and annually thereafter. ABCMS – As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired within five business days. The process of notifying the Division has changed and is now captured in ABCMS. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. During this visit, we walked through the entire facility observing the indoor environment, but a full assessment was not completed for each space. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. Children were observed engaged in tutoring, personal care routines, and group time. Program records, including fire drills and monthly outdoor area inspections were complete and current. The afternoon meal included sunflower seeds, sun butter, vegetable juice, craisins, honey graham crackers and milk. Compliance History - The compliance history for this facility prior to today’s visit is 97%. The violation(s) documented may impact the compliance history score and must be corrected immediately. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The following violations were documented. Medication that expired on 8/22/25 was on site. .0803(12) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. An individual hired 10/27/25 did not have documentation of orientation within the first two weeks of employment. .1101(a)(b) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. The medications stored at your facility must be reviewed regularly to ensure they have not expired. Administering a medication that has expired could cause harm to a child. One (1) inhaler expired on 8/22/25. The expired medication must be returned to the parent or discarded immediately. Per our discussion, you must create a plan to review medications, permission forms, and medical action plans to ensure they are current and accurate. Orientation- Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. One employee hired on 10/27/25 did not have documentation of 6 hours of orientation within the first 2 weeks of employment. Effective employee orientation makes employees aware of company policies and expectations, handles essential paperwork, and answers any questions or concerns they may have before they transition into their new positions. Orientation is important because it signifies the beginning of the relationship between employee and employer. Rated License – You the Application for Assessment for a Rated License today. We reviewed your family and community engagement standards and the facility continuous quality improvement plan. You indicated you are applying for the 3-Star level of Pathway 2- Classroom and Instruction Quality until you receive a determination of the approval of the curriculum utilized by the Boys and Girls Clubs of the Coastal Plains. Education: You have three staff that you are completing WORKS accounts for tomorrow to get them group leader qualified. A new license effective January 3, 2026 will be mailed directly to you and must be posted upon receipt. On-Going Training requirements must be completed and documentation of each training in file for review after the first year of employment and annually thereafter. ABCMS – As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired within five business days. The process of notifying the Division has changed and is now captured in ABCMS. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Emergency Preparedness and Response - Within one year of the effective date of a new license, must have completed the Emergency Preparedness and Response in Child Care training. The Emergency Preparedness and Response Plan means a written plan that addresses how a child care facility will respond to both natural and man-made disasters, such as fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disaster, or a dangerous person in the vicinity, to ensure the safety and protection of the children and additional caregivers. This Plan must be on a template provided by the Division available at https://rmp.nc.gov/portal/# completed within four months of completion of the Emergency Preparedness and Response in Child Care training, and available for review. The NC Secretary of State website was reviewed on August 25, 2025, and Boys and Girls Club of the Coastal Plain was listed as current- active. We discussed the following: Recognizing and Responding to Suspicions of Child Maltreatment – Childcare Staff are mandatory reporters of child abuse and neglect. Education is important in identifying manifestations of child maltreatment that can increase the likelihood of appropriate reports to child protection and law enforcement agencies. Within 90 days of employment staff must complete Recognizing and Responding to Suspicions of Child Maltreatment training. Knowing the signs of child maltreatment and responding accordingly is key to creating a nurturing child care environment and could potentially save a child's life.
