S. Davender-Tyson, director assisted during the visit. Your program currently operates with a five star license, issued 1/16/2023 earning 6 points in the education component, 6 points in the program standards component (meeting enhanced space and ratios) and 1 quality point for having a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 1/30/25. The sanitation inspection was completed 10/31/25 with a “Superior” classification. The fire inspection was conducted on 4/25/25 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of 1/2/26. The NC Secretary of State website was reviewed on 1/2/2026 and Follow The Son of Greenville, LLC. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. We discussed that your facility is approved for second shift care, you are currently not operating second shift. You indicated that second shift could be removed from the facilities license. Children enrolled in classroom space #1 were observed during free play. The staff sat with children on the carpet as they explored. Handwashing was observed as a child arrived in classroom space #1. In classroom space #4 children sat with the teacher on the carpet during morning group. The children named colors and shapes as the staff member held up each shape and color. Following group the staff and children transitioned to the outdoor space. Lunch was observed and consisted of wheat crackers, peaches, and grilled chicken and salad. Department of Information Technology will be changing the minimum length of NCID passwords from eight to 14 characters. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children enrolled in classroom space #1 were observed during free play and sitting at a child sized table eating breakfast. Breakfast included pancakes, applesauce and milk. Sleep checks were completed, recorded, and maintained as required. Files for new staff were reviewed. In classroom space #5 children were sitting on the carpet with the teachers for morning group. The children named shapes as the teacher pointed to different shapes on the board. A sanitation inspection was conducted at the facility on 3/5/25 with a “Superior” classification and a fire inspection was conducted at the facility on 4/25/25 and your facility was approved for day time and night care. The violations are as follows: Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. On the preschool playground the tray on the paint easel was cracked and broken, the platform to the stationary climber was cracked, and the storage container with the waffle blocks was broken. 15A NCAC 18A .2832(a) 843 A drug or medicine was administered after its expiration date. S. Davender-Tyson, director, assisted me with the visit. T. Tyson administrator arrived during the visit. Your program currently operates with a five-star license, issued 1/16/2023, earning 6 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having staff benefit package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 2/22/24. The sanitation inspection was completed 6/11/2024 with a “Superior” classification. The last fire inspection was conducted 5/29/24 and your facility was approved for day time care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-five percent as of 1/30/25. The NC Secretary of State website was reviewed on 1/30/25 and Follow The Son Child Care Center, LLC was listed as current- active. I visited indoor and outdoor spaces. In classroom space #1 children were moving freely around the classroom space, staff comforted crying children. In classroom space #4 and #2B children engaged in free play with a variety of age-appropriate activities. Lunch was observed and included baked ziti, garden salad, peaches, sliced wheat bread and milk. The following violations were documented. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. The slide on a portable climber was cracked and broken. 10A NCAC 09 .0604(p) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Staff did not document visual checks of an a child under age one who was resting during the visit. .0606(g) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The use of sleep charts is identified in your center’s ITS-SIDS policy. Sleep charts are part of the center’s program records and must be kept on file and available for review. A child under age one was observed resting in an approved crib. When asked staff reported that the child fell asleep at 9:00am, the child woke up at 9:16am. This was not documented on the child’s sleep chart. The director reviewed the use of sleep charts with the staff, and instructed the staff to add a note to the child’s chart as documentation. Check sleep charts weekly to ensure they are completed daily for infants in care. Health & Safety Trainings: Child care operators, additional caregivers, and substitute providers were required to complete health and safety training offered by the Division no later than June 30, 2018. Staff T. Staton did not complete the health and safety trainings within one year of hire. You stated that the staff member is the cook but also drives the vans. We discussed that van drivers are included in caregiver duties. You stated that you would have the staff complete the training. A safe outdoor environment was not provided for the children, damaged equipment were accessible to the children. On the playground, the slide to a portable climber on the toddler playground was cracked and broken. You immediately had the slide removed during the visit. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Your program currently operates with a five-star license, issued 1/16/2023, earning 6 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having staff benefit package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 2/22/24. The sanitation inspection was completed 6/11/2024 with a “Superior” classification. The last fire inspection was conducted 5/29/24 and your facility was approved for day time care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-five percent as of 1/30/25. The NC Secretary of State website was reviewed on 1/30/25 and Follow The Son Child Care Center, LLC was listed as current- active. I visited indoor and outdoor spaces. In classroom space #1 children were moving freely around the classroom space, staff comforted crying children. In classroom space #4 and #2B children engaged in free play with a variety of age-appropriate activities. Lunch was observed and included baked ziti, garden salad, peaches, sliced wheat bread and milk. The following violations were documented. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 3/15/2023. The sanitation inspection was completed 12/11/2023 with a “Superior” classification. The last fire inspection was conducted 5/24/23. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-one percent as of 3/10/23. The NC Secretary of State website was reviewed on 3/10/23 and Follow The Son Child Care Center, LLC was listed as current- active. I visited indoor and outdoor spaces. Throughout the classroom spaces children were engaged in free play with age appropriate materials including blocks and dramatic play items. Children were observed coloring on construction paper with crayons. Infant children enrolled in space #1 received care to meet their individual needs including diapering and feeding. The required staff/child ratio for this group would be 1:5. In any multi age group situation, the staff/child ratio for the youngest child in the group shall be maintained for the entire group. The teacher stated the children were combined in one room while a staff member used the restroom. Consider hiring part-time staff to provide extra coverage during certain hours of the day to assist primary caregivers with carrying out daily activities and routines. The NC Secretary of State website was reviewed on 11/9/2022 and Follow The Son Child Care Center, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were engaged free play, feeding and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. Six violations were observed today. Two were corrected during the visit and the others must be corrected immediately. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was 6 places on along the fence of the playground area where there was a gap between the fence and the ground. This created spaces of entrapment and the sharp edges of the fence wires was exposed. There was a gap in the containment border around the stationary climbers creating a tripping hazard. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. There was 6 places on along the fence of the playground area where there was a gap between the fence and the ground. This created spaces of entrapment and the sharp edges of the fence wires was exposed. There was a gap in the containment border around the stationary climbers creating a tripping hazard. The concrete base to poles of the shelter were exposed. During the visit the maintenance staff arrived to work on the fence. You, the center supervisor corrected the piping to the containment border. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. In space # 4 a container of ZEP wipes were stored in a cabinet above five feet but not locked. In space #2B a container of ZEP wipes were stored above the diaper table in a cabinet but not locked. When I brought this to your attention, you immediately removed the Zep wipes and properly stored the items. Medication Disposal: Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. In space #4 a tube of Walgreens Diaper Rash Paste expired on 8/2023. You stated that the child was no longer enrolled, and you removed the medication from the classroom. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. Assign a staff member to check for expired medications and medication authorization forms monthly. No drug or medication may be administered to any child without specific written instructions from the child's parent/guardian, a physician, or other authorized health professional. The medication authorization form identified for a Gentle Steps Diaper Rash paste did not include the name of the diaper paste on the form. We reviewed that authorization should include the name of the medication and the amount. Vehicle Maintenance: Children should be cared for and transported in safe environments. Maintaining the interior and exterior of vehicles is important to the safety of children. The front tires on the Honda Odyssey van used to transport children measured less than 2/32 of an inch. The two front tires on the 350 Transit van used to transport children measured less than 2/32 of an inch. We discussed having daily transportations checks prior to using vehicles. Playground Surfacing: The surfacing used underneath one stationary f equipment was a light covering of mulch material that measured less than an inch. In order to continue providing a safe outdoor environment, be sure to maintain the six inch depth and frequently fluff the materials if it becomes compact due to routine play or rainy weather. The period for background check qualification renewals was changed from every 3 years to every 5 years. The provisional qualification period was reduced to 45 days The Division intends to implement these changes as soon as possible. An individual’s current Qualification Letter (QL) will be extended 2 years for a total of 5 years. Updated letters will not be generated. Individuals with a Provisional Qualification Letter due to an out-of-state check will received a qualifying letter on the 45th day. Letters are generated in the CBC Portal. COVID Signage – On August 15, 2022, the governor lifted the “State of Emergency” status for North Carolina. The period for background check qualification renewals was changed from every 3 years to every 5 years. R. Johnson, center supervisor assisted me with today’s visit. Your program currently operates with a five-star license, issued 1/16/23 earning 6 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having a infrastructure of family involvement and staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty four percent as of 11/9/22. The NC Secretary of State website was reviewed on 11/9/2022 and Follow The Son Child Care Center, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were engaged free play, feeding and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. Six violations were observed today. Two were corrected during the visit and the others must be corrected immediately. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was 6 places on along the fence of the playground area where there was a gap between the fence and the ground. This created spaces of entrapment and the sharp edges of the fence wires was exposed. There was a gap in the containment border around the stationary climbers creating a tripping hazard. The concrete base on three of the poles to the covered area were exposed. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space # 4 a container of ZEP wipes were stored in a cabinet above five feet but not locked. In space #2B a container of ZEP wipes were stored above the diaper table in a cabinet but not locked. .2820(b) 843 A drug or medicine was administered after its expiration date. In space #4 a tube of Walgreens Diaper Rash Paste expired on 8/2023. 10A NCAC 09 .0803(1)(d) 847 Parent's medication authorization did not include required information. The medication authorization form identified for a Gentle Steps Diaper Rash paste did not include the name of the diaper paste on the form. 10A NCAC 09 .0803(4)(6-9) 1123 All vehicles used to transport children were not free of hazards. The front tires on the Honda Odyssey van used to transport children measured less than 2/32 of an inch. The two front tires on the 350 Transit van used to transport children measured less than 2/32 of an inch. 10A NCAC 09 .1002(a) 1867 The depth of the loose surfacing was not based on critical height of the equipment. Mulch around and the fall zones of the stationary climber measured less than an inch. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 21, 2023 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. In space # 4 a container of ZEP wipes were stored in a cabinet above five feet but not locked. In space #2B a container of ZEP wipes were stored above the diaper table in a cabinet but not locked. When I brought this to your attention, you immediately removed the Zep wipes and properly stored the items. Medication Disposal: Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. In space #4 a tube of Walgreens Diaper Rash Paste expired on 8/2023. You stated that the child was no longer enrolled, and you removed the medication from the classroom. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. Assign a staff member to check for expired medications and medication authorization forms monthly. No drug or medication may be administered to any child without specific written instructions from the child's parent/guardian, a physician, or other authorized health professional. The medication authorization form identified for a Gentle Steps Diaper Rash paste did not include the name of the diaper paste on the form. We reviewed that authorization should include the name of the medication and the amount. Vehicle Maintenance: Children should be cared for and transported in safe environments. In order to continue providing a safe outdoor environment, be sure to maintain the six inch depth and frequently fluff the materials if it becomes compact due to routine play or rainy weather.
