Children throughout the classroom spaces were observed eating lunch. Lunch included chicken alfredo, broccoli, mixed fruit and milk. Later children were observed resting on linen covered cots. The following violations documented during the 3/18/26 visit were monitored for compliance during this visit: 1756- Enhanced staff/child ratios and group sizes were not met. In classroom space #2 three staff cared for fifteen children one to two years of age. This is a violation of a requirement in 10A NCAC 09 .2818. During today’s visit in classroom space #2 two staff members cared for fourteen children one to two years of age. As stated in the 8/19/2024 visit summary, all violations must be corrected immediately and a compliance letter verifying this must be received by the established due date. I received a compliance letter from you during today’s visit, you stated the violation was corrected by separating the classroom spaces. Today a repeat violation of staff/child ratio was documented. Repeat violations demonstrate a pattern of noncompliance and may result in an administrative action per Child Care Rule 10A NCAC 09 .2204(6). You will be notified in writing of any action taken. The following violations were documented. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. Jennifer Bradshaw Garrett, Child Care Consultant, accompanied me during today’s visit. You, D. Ward Administrator, assisted me with the visit. This facility was issued a temporary license on October 27, 2025. The NC Secretary of State website was reviewed on March 16, 2026, and Dawnsha's Loving Childcare Center LLC was listed as current-active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed and the information is current. It is important to frequently check your email to remain informed about any new changes that have or may occur. During this visit, I visited indoor and outdoor learning environments , but a full assessment was not completed for each space. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. Children were observed engaged in personal care routines, free play in designated learning areas, and transitioning from outdoor play. Lunch was observed and included beef macaroni and cheese, corn, fruit cocktail and milk. Program records, including fire drills and monthly outdoor area inspections were complete and current. Compliance History - The compliance history for this facility prior to today’s visit is 89%. The violation(s) documented may impact the compliance history score and must be corrected immediately. Items that require locked storage were improperly stored and are as follows: In classroom space #1 there was Chanel #5 perfume stored in a container with food on top of the children’s cubbies. When this was brought to staff attention they discarded the perfume. Items that require storage at five feet above the finished floor were improperly stored and are as follows: In a child’s backpack stored in a cubby below five feet in the hallway was tube of Remedy Skin Protectant Ointment. You removed the ointment and placed it in locked storage in the office. You inquired about adding a locked box on the wall in classroom spaces #1 and #2. We discussed that the lock box would need to be placed above five feet so that it is inaccessible to the children. Staff Records - The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation to include health information. During the visit today one staff hired on 3/6/26 did not have medical report on file or available for review. You shared that the staff stated the doctor was supposed to be emailing the staff a copy of the report. It is important that this information is received at or prior to hire. Require the documents prior to proceeding with the hiring process. Staff/Child Ratio- Research has verified that staff-child ratios and group sizes are important quality indicators. You have the required number of staff to meet staff/child ratio for each classroom space. You stated that one staff is a new teacher and you did not want to place that teacher in the space alone with children. Pair new staff with experienced staff. Rated License: You submitted an Application for Assessment for a Rated License you submitted on March 12, 2026. Today you provided me with an updated Application for a Rated License Assessment. You indicated you are applying for the 4-Star level of Pathway 2- Classroom and Instruction Quality. I reviewed your family and community engagement standards and verified you are meeting minimum requirements and have a plan to meet three additional choices, one from each category of engagement. I reviewed the facility continuous quality improvement plan, and you have identified a goal for the facility. I reviewed and verified the Individual CQI's for staff. Complete CQI’s for J. Hines, I. Langley and J. Barrett. Curriculum: During today’s visit you purchased the Experience Early Learning Curriculum and the Experience Early Learning Assessment. You identified Funny Daffer as the curriculum for school age. I reviewed portfolios set up for each child enrolled in the facility. Education: Today we reviewed the 4 Star Education Standards for Lead, Teachers, and Group Leader. The following staff did not have DCDEE WORKS accounts: M. Artis M. Holmes A. Cox S. Ross J. Hines S. Epheim You stated that WORKS accounts have been created for each of the staff members. Education has been submitted to WORKS for the following staff: D. Ward S. Harris M. Lawerence N. Additional Dirt was delivered during today’s visit. You stated the company that will distribute the dirt and on the hill is scheduled to arrive this afternoon. You also shared that you will be purchasing sod to prevent the erosion of the sand. You shared that you would use the Early Learning Matters Curriculum and we reviewed the requirements for approved assessment tools. You informed me that the staff stated they forgot to put the laminating machine back in the storage closet, staff returned the laminating machine to the office during the visit. Children enrolled in classroom space #1 were observed resting in approved cribs while staff completed diapering routines and comforted children under age one. In classroom space #6 children were observed sitting at child sized tables as the teacher read a story. School age children were observed during free play with dramatic play items, completing floor puzzle with the teacher assisting as needed and children engaged in art activities. During the visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. You informed me that you will pursue the Classroom and Instructional Quality Pathway. Enhanced ratio requirements were reviewed. You shared that you would use the Early Learning Matters Curriculum and we reviewed the requirements for approved assessment tools. We discussed that an approved curriculum is needed for your school age children. We reviewed education requirements for your staff including CEU requirements and additional training requirements. Submit your staff information worksheet by February 16, 2026. Submit your rated license application by March 2026. The following violations were documented. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. In classroom space #1 a laminator was stored ina unlocked cabinet below the diaper changing table. .0604(e) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff hired on 1/20/2026 did not have a TB test or screening on file. .0701(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. One staff hired on 1/20/2026 did not have a medical statement on file. .0701(a) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. Two of nine children did not have medical exam or health assessment on file. Remind parents that bottles must have lids. Activity plans: Activity plans help teachers to plan for and implement daily activities for children. They provide information to families on activities taking place during their children’s day. Planning for a week of activities helps teachers to obtain and prepare the materials and equipment needed to successfully carry out the activities on the plan. A current activity plan was not available in classroom space #5 and #6. We discussed a current activity plan should be posted in each classroom space. You had staff complete an activity plan during the visit. Safe Environment: Building and physical premises safety refers to actively identifying and mitigating potential hazards within a building or outdoor space that could cause bodily harm, including risks like electrical shock, drowning hazards from bodies of water, and injuries from vehicular traffic, by implementing preventative measures and safety protocols to protect individuals on the premises. In classroom space #1 a laminating machine was stored in an unlocked cabinet below the diaper table. You informed me that the staff stated they forgot to put the laminating machine back in the storage closet, staff returned the laminating machine to the office during the visit. Staff Records: The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation such as employment application, medical report, and TB test results. One staff hired 1/20/26 did not have a TB skin test/screening or medical report on file. You stated that you did not review the file, but you inquired about the forms with he staff and the staff indicated they had the forms at home and would bring them in. Two new staff N. Cox and S. Harris did not have a signed acknowledgement of the Prevention of Shaken Baby Policy on file. You had both staff complete the form during the visit. We discussed that prior to staff first day at the facility, have staff submit the forms and then discuss a start date. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency Two children of nine children’s records reviewed did not have a medical report on file. You immediately contacted both parents informing them the child needed a medial report. When enrolling children require the medical report prior to the child’s first start date.
