T. Pollard, operator assisted me with today’s visit. Your program currently operates with a four-star license, issued 6/27/2022. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-nine percent as of 10/29/25. The NC Secretary of State website was reviewed on 2/3/25 and Creative Learning Childcare Center, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today. I observed children in the indoor learning environments. In classroom spaces #2, 5 and 6 observed children sitting at child sized tables eating lunch. Lunch included chicken wings, peas, peaches, sliced wheat bread, and milk. In classroom space #1 children rested on linen covered cots and approved cribs. The violations are as follows: Violation Number Comment Rule 303 Children were not adequately supervised at all times. Children in classroom space #5 were observed sitting at a child sized table with an activity, unattended. .1801(a)(1-5) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. The were holes in the underpinning of the building on the school age playground. 15A NCAC 18A .2832(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. When I entered classroom space #5 the children were seated at child sized tables engaged in an activity unattended. The teacher assigned to the classroom space was standing in the doorway of classroom space #6. When prompted the teacher returned to classroom space #5. You immediately addressed the staff member. Review supervision with classroom staff and the use of your facilities walkie-talkies. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The were holes in the underpinning of the building on the school age playground. We discussed replacing the underpinning. Additional Comments: During the visit an overview of section .3200 of the child care rules was provided. We discussed the following pathway: Classroom and Instructional Quality Pathway. You informed me that your facility will pursue Pathway #2: Classroom and Instructional Quality. We reviewed the Experience Early Learning Curriculum. You plan to use the Experience Early Learning Curriculum through Brightwheel. We discussed that you would need an approved curriculum for you School Age program. Education requirements were shared with you for both 4 Stars and 5 Stars. Submit all education to DCDEE Works by May, 2026. Rated License Timeline: Submit your Rated License Application by June 2026. Your program currently operates with a four-star license, issued 6/27/2022, earning 5 points in the education component, 6 points in the program standards component (meeting enhanced ratio and space) and 1 quality point for having an infrastructure of parent involvement and staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled). The sanitation inspection was completed 6/23/25 with a “Superior” classification. The last fire inspection was conducted 4/22/25 and your facility was approved for day time care only. The center's compliance history was reviewed with the administrator. The program’s compliance history was ninety-four percent as of 7/3/25. The NC Secretary of State website was reviewed on 7/3/2025 and Creative Learning Childcare Center LLC was listed as current- active. We reviewed the facility contact information. I visited indoor and outdoor spaces. Children enrolled in classroom space #1 engaged in free play as the teachers sat with the children on the floor. Children in classroom space #5 were observed making suncatchers. Transitions from outdoor play and routines were observed. Lunch was observed and included beef steak and gravy, wheat bread, broccoli mix with carrots and cauliflower, pineapples and milk. The following violations were documented. Being trained in CPR and first aid can be invaluable when someone is in serious medical distress. Therefore, it is so important that all staff working directly with children have current CPR and First Aid Certification. Staff have 90 days from the day of hire to complete the required trainings. One staff hired on 6/18/24 CPR and First Aid certification expired on 8/1/24, and a new card was on file dated 6/27/25. We discussed creating a tracking system to ensure CPR/FA certifications remain current. Medication: Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. An epi-pen stored in the office expired 5/2025. We discussed returning all unused or expired medications to parents within 72 hours. Additional Comments: Today we reviewed child care rules section .0703 and .0710 regarding lead teacher requirements. We also reviewed the QRIS Modernization pathways 1, 2, and 3. DCDEE has added a new training module about Child Development on the DCDEE Moodle Learning Platform. This is the first of several planned modules to support orientation and the health and safety training requirements. This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Upon completion, participants will receive one (1) contact hour credit (CHC). The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Each staff member shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. The sanitation inspection was completed February 9, 2023 with a “Superior” classification. The last fire inspection was conducted 4/24/23 and your facility was approved for day time and night care. The center's compliance history was reviewed with the administrator. The program’s compliance history was ninety-four percent as of 7/18/23. Lunch was observed and consisted of ham sandwich, french fries, fruit cocktail, and milk. The following violations were documented.
