M. Bass, director and K. Doughtie, assistant director, assisted me with the visit. Your program currently operates with a five star license, issued 9/29/2017. The last annual compliance visit was conducted 4/16/25. The sanitation inspection was completed 6/24/25 with a “Superior” classification. The last fire inspection was conducted 1/10/25 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety- seven percent as of 9/15/25. The NC Secretary of State website was reviewed on 9/15/25 and Aspiring Minds, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. You visited indoor spaces with me. Children enrolled in classroom spaces #1 and #5 were observed during morning group time. In classroom space #1 a child was observed sharing items from an all about me bag. The teachers assisted the child with sharing with the class. In classroom space #5 children, one year of age sat with the teacher on the carpet. As the teacher displayed pictures for the children, the teacher encouraged the children to repeat after her naming the items. Throughout the remainder of the classroom spaces children engaged in free play with age-appropriate materials. Children in classrooms spaces designated for infants received care based on their individual needs. Breakfast was observed and included egg and cheese omelets, orange slices, blueberries and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The children were observed playing in designated learning areas and small group art with the letter A. Materials were observed for the beginning of the year assessments. Planning for a week of activities helps teachers to obtain and prepare the materials and equipment needed to successfully carry out the activities on the plan. The activity plan posted in the school age classroom was dated 8/11/25-8/15/25. You looked for the activity plan but could not locate it. You stated that the assistant director checks activity plans weekly on Monday mornings. We discussed having staff submit activity plans earlier and making a copy of the activity plan for your records. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. Within in 30 days of enrollment a child must have a health assessment on file. Two of seventeen children’s records reviewed did not have a valid medical report on file. We discussed that the cover sheet to the child’s medical report does not include all of the required items. You stated that you would review all the children’s files and request updated medical reports if needed. Request the medical report prior to the child’s first day on their first day to ensure you receive the medical report within the required timeframe and it meets the requirements. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 11/8/2023. The sanitation inspection was completed 6/28/24 with a “Superior” classification. The last fire inspection was conducted 1/10/24 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight percent as of 10/16/24. The NC Secretary of State website was reviewed on 10/14/24 and Aspiring Minds, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Throughout the classroom spaces children were observed during free play with a variety of age-appropriate materials. Children enrolled in classrooms spaces designated for infants received care according to their individual needs. Children’s arrival was observed. Today’s lunch menu reflected tomato soup, grilled cheese sandwich, peas, apple slices, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The teacher was observed completing assessments on letters with the children individually. Children enrolled in space #1 were sitting at sitting on the carpet with books and sitting at a child sized table eating breakfast. Children under twelve months old received care according to their individual needs including feeding. Children were observed during free play with age-appropriate materials. In the outdoor space children played on stationary equipment and on the swings. A fire drill was observed during the visit. During the fire drill teachers were observed having conversation with children about nature, children were observed exploring the leaves, teachers played music for the children. Breakfast was observed and consisted of grits, peaches and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Student portfolios and assessment materials were reviewed. Family communication logs were reviewed. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The teacher stated that she was working on the checkpoint assessments. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In outdoor space #3 the chains to the swings were rusted and there was hole with tree root in front of the metal playhouse creating a tripping hazard. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A jug of windshield wiper fluid was stored near the entry door on the minibus used to transport children. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. The authorization form for Blue Lizard Sunscreen in space #1 expired on 7/1/2023. .0803(12) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Staff hired on 9/6/23 and 11/6/23 did not have an Emergency information form on file on or before the first day of work. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff hired on 9/6/23 did not have documentation of 16 hours of orientation of file. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff hired on 8/26/2020 First Aid training expired on October 31, 2023. A new first aid training was not on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff hired on 8/26/2020 CPR training expired on October 31, 2023. A new CPR training was not on file. .1102(d) 1123 All vehicles used to transport children were not free of hazards. Tire threads on the front driver and passenger side of minibus #8 measured less than 2/32 of an inch. 10A NCAC 09 .1002(a) 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. One of seventeen children's record reviewed discipline policy did not include the child's date of enrollment. .1804(b) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. A smoking and tobacco restriction sign was not posted on in the minibus for transportation. .0604(i) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Staff hired on 9/6/23 did not have documentation of review of the facilities Shaken baby Syndrome and Abusive Head Trauma policy on file. .0608(d)(1-4) 1898 Staff did not complete the health and safety training within one year of employment. One staff hired on 8/26/22 did not have documentation of completed health and safety trainings on file. .1102(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 22, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The chains of the swings in outdoor space #3 were rusted and there was hole in front of the metal playhouse with an exposed tree root creating a tripping hazard. You contacted maintenance during the visit. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. A jug of windshield wiper fluid was located near the entry door to a minibus used to transport children. You removed this from the minibus during the visit. Medication: Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. In space #1 the authorization for a bottle of Blue Lizard sunscreen expired on 7/1/23. We discussed returning all unused or expired medications to parents within 72 hours. Vehicle Maintenance: Children should be cared for and transported in safe environments. Maintaining interior of vehicles is important to the safety of children. Tire treads on the front driver and passenger side of minibus #8 were less than 2/32 of an inch. Conduct safety checks regularly to ensure vehicle safety requirements are met. Tobacco Free Environment: Children shall be in a smoke free and tobacco free environment. Signage regarding the smoking and tobacco restriction shall be posted at each entrance to the center and in vehicles used to transport children. A no smoking/tobacco restriction was not posted on the minibus used to transport children. You replaced the sign during today’s visit. Health & Safety Trainings: Child care operators, additional caregivers, and substitute providers were required to complete health and safety training offered by the Division no later than June 30, 2018. During a review of staff training hours, staff hired on 8/26/22 did not complete health and safety trainings within in the one-year requirement. When a new staff member is hired, they must complete the trainings within the first year of hire. Require new staff to complete the health & Safety trainings as part of your new hire orientation, to ensure completion in the required timeframe. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. One of seventeen children’s records reviewed discipline policy did not include the child’s enrollment date. Review children’s files often and use the Center children’s checklist to ensure you have all required documents within the specified timeframe. Orientation: Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. Effective employee orientation makes employees aware of company policies and expectations, handles essential paperwork, and answers any questions or concerns they may have before they transition into their new positions. Each staff person shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. A staff employed on 9/6/23 did not have documentation of orientation on file for review. You stated you had completed orientation with the staff member, but were unable to document it at the time due to being out of the office. You had staff complete the documentation during today’s visit. Document orientation as it is completed to ensure accurate records. Prevention of Shaken Baby Syndrome & Abusive Head Trauma Policy: Each child care center licensed to care for children up to five years of age must develop and adopt policies to prevent shaken baby syndrome and abusive head trauma. Staff employed on 9/6/23 did not have documentation of review of the facility’s Prevention of Shaken Baby Syndrome & Abusive Head Trauma Policy. You stated the you reviewed it with the staff during orientation. You had staff complete the form during today's visit. When reviewing policies, sign and file the forms upon completion to ensure accurate and complete records. Emergency Information: The Emergency Information and Health Questionnaire form must be completed for all staff by day one of hire and updated as changes occur or at least annually. Staff hired on 9/6/23 and 11/6/23 did not have and emergency information form on file. You had staff complete the forms during todays visit. Have staff complete forms during their first day of orientation to ensure they are completed on the staff's first day. First Aid/CPR: To ensure the health and safety of children in a child care setting. Each staff member providing direct care for children are required to complete First Aid and CPR training and to recertify every two years thereafter. One staff hired on 8/26/2020 CPR and First Aid training expired on 10/31/2023. A new CPR and first aid training had expired. Have staff to complete training and provide a copy of the certification/card. Review your files regularly to help track due dates. This is a repeat violation form the routine unannounced visit conducted on 6/15/2023 and the annual compliance visit conducted on 12/6/2023. Repeat violations may lead to the issuance of an administrative action. LeShaunda McCotter, Child Care Consultant assisted me with todays visit. M. Bass, director assisted me with the visit. Your program currently operates with a five star license, issued 9/29/2017, earning 6 points in the education component, 6 points in the program standards component(meeting enhanced ratios and space) and 1 quality point for having enhanced policies and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 12/6/2022. The sanitation inspection was completed 6/6/23 with a “Superior” classification. The last fire inspection was conducted 1/18/23 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-seven percent as of 11/3/23. The NC Secretary of State website was reviewed on 11/3/23 and Aspiring Minds, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Children enrolled in space #1 were sitting at sitting on the carpet with books and sitting at a child sized table eating breakfast. Children under twelve months old received care according to their individual needs including feeding. Children were observed during free play with age-appropriate materials. In the outdoor space children played on stationary equipment and on the swings. A fire drill was observed during the visit. During the fire drill teachers were observed having conversation with children about nature, children were observed exploring the leaves, teachers played music for the children. Breakfast was observed and consisted of grits, peaches and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Student portfolios and assessment materials were reviewed. Family communication logs were reviewed. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The teacher stated that she was working on the checkpoint assessments. The following violations were documented. LeShaunda McCotter, Child Care Consultant assisted me with todays visit. M. Bass, director assisted me with the visit. Your program currently operates with a five star license, issued 9/29/2017, earning 6 points in the education component, 6 points in the program standards component(meeting enhanced ratios and space) and 1 quality point for having enhanced policies and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 12/6/2022. The sanitation inspection was completed 6/6/23 with a “Superior” classification. The last fire inspection was conducted 1/18/23 and your facility was approved for day time and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-seven percent as of 11/3/23. The NC Secretary of State website was reviewed on 11/3/23 and Aspiring Minds, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. You visited indoor and outdoor spaces with me. Children enrolled in space #1 were sitting at sitting on the carpet with books and sitting at a child sized table eating breakfast. Children under twelve months old received care according to their individual needs including feeding. Children were observed during free play with age-appropriate materials. In the outdoor space children played on stationary equipment and on the swings. A fire drill was observed during the visit. During the fire drill teachers were observed having conversation with children about nature, children were observed exploring the leaves, teachers played music for the children. Breakfast was observed and consisted of grits, peaches and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Student portfolios and assessment materials were reviewed. Family communication logs were reviewed. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The teacher stated that she was working on the checkpoint assessments. The following violations were documented. In outdoor space #3 the chains to the swings were rusted and there was hole with tree root in front of the metal playhouse creating a tripping hazard. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A jug of windshield wiper fluid was stored near the entry door on the minibus used to transport children. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. The authorization form for Blue Lizard Sunscreen in space #1 expired on 7/1/2023. .0803(12) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Staff hired on 9/6/23 and 11/6/23 did not have an Emergency information form on file on or before the first day of work. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff hired on 9/6/23 did not have documentation of 16 hours of orientation of file. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff hired on 8/26/2020 First Aid training expired on October 31, 2023. A new first aid training was not on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. A new CPR training was not on file. .1102(d) 1123 All vehicles used to transport children were not free of hazards. Tire threads on the front driver and passenger side of minibus #8 measured less than 2/32 of an inch. 10A NCAC 09 .1002(a) 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. One of seventeen children's record reviewed discipline policy did not include the child's date of enrollment. .1804(b) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. A smoking and tobacco restriction sign was not posted on in the minibus for transportation. .0604(i) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Staff hired on 9/6/23 did not have documentation of review of the facilities Shaken baby Syndrome and Abusive Head Trauma policy on file. .0608(d)(1-4) 1898 Staff did not complete the health and safety training within one year of employment. One staff hired on 8/26/22 did not have documentation of completed health and safety trainings on file. .1102(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 22, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The chains of the swings in outdoor space #3 were rusted and there was hole in front of the metal playhouse with an exposed tree root creating a tripping hazard. You contacted maintenance during the visit. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. A jug of windshield wiper fluid was located near the entry door to a minibus used to transport children. You removed this from the minibus during the visit. Medication: Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. In space #1 the authorization for a bottle of Blue Lizard sunscreen expired on 7/1/23. We discussed returning all unused or expired medications to parents within 72 hours. Vehicle Maintenance: Children should be cared for and transported in safe environments. Maintaining interior of vehicles is important to the safety of children. Tire treads on the front driver and passenger side of minibus #8 were less than 2/32 of an inch. Conduct safety checks regularly to ensure vehicle safety requirements are met. Tobacco Free Environment: Children shall be in a smoke free and tobacco free environment. Signage regarding the smoking and tobacco restriction shall be posted at each entrance to the center and in vehicles used to transport children. A no smoking/tobacco restriction was not posted on the minibus used to transport children. You replaced the sign during today’s visit. Health & Safety Trainings: Child care operators, additional caregivers, and substitute providers were required to complete health and safety training offered by the Division no later than June 30, 2018. During a review of staff training hours, staff hired on 8/26/22 did not complete health and safety trainings within in the one-year requirement. When a new staff member is hired, they must complete the trainings within the first year of hire. Require new staff to complete the health & Safety trainings as part of your new hire orientation, to ensure completion in the required timeframe. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. One of seventeen children’s records reviewed discipline policy did not include the child’s enrollment date. Review children’s files often and use the Center children’s checklist to ensure you have all required documents within the specified timeframe. Orientation: Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. Effective employee orientation makes employees aware of company policies and expectations, handles essential paperwork, and answers any questions or concerns they may have before they transition into their new positions. Each staff person shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. You stated you had completed orientation with the staff member, but were unable to document it at the time due to being out of the office. You stated the you reviewed it with the staff during orientation. You had staff complete the form during today's visit. When reviewing policies, sign and file the forms upon completion to ensure accurate and complete records. Emergency Information: The Emergency Information and Health Questionnaire form must be completed for all staff by day one of hire and updated as changes occur or at least annually.
