The allegations are as follows: There are concerns regarding the use of inappropriate discipline. Enrollment: Space #10 Staff-K. Williams, K. Pitts 11 children four years of age enrolled; 7 children four years of age present 13 children five years of age enrolled; 10 children five years of age present Children were engaged in interest areas including science, art, reading, blocks, and table top manipulatives. Teachers rotated around the classroom, asking children questions about the materials they engaged with and assisting children with problem solving and classroom materials. Regarding the allegation regarding the use of inappropriate discipling, I completed classroom observations, and interviewed classroom and administrative staff. Video footage of previously recorded dates was requested, but were unavailable during the visit. It was reported that on January 23, 2026, a child was removed from the classroom and placed in the outdoor space following behavioral issues including hitting and kicking the staff. The recorded temperature for that date and time was 48 degrees. The child was not wearing a coat. Based on the above information the allegation was confirmed. The following violation was observed. Violation Number Comment Rule 908 Discipline was not appropriate for the child's age and development. It was reported that on January 23, 2026, a child was taken outside, without a coat after the child dumped his lunch and was under the table and hit and kicked a staff member. The recorded temperature for that date and time frame was fourth-eight (48) degrees. .1803(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It was reported that on January 23, 2026, a child was taken outside, without a coat after the child dumped his lunch and was under the table and hit and kicked a staff member. You stated that you sometimes removed children from the classroom environment to help with de-escalation and give the child a change of environment, once the child is calm, the staff or administrator talk with the child about what happened and how things could be handled differently. While the method of changing the environment may be helpful, we discussed ways to ensure children’s safety during those changes. Ensuring the outdoor environment is safe also includes ensuring children are properly dressed. Weather conditions may require a coat, hat, or even a raincoat. You reported that you have been seeing an increase in behavior issues recently. Technical assistance for the Healthy Social Behaviors (HSB) Project was provided to you during the visit on September 4, 2025. You reported that you had not reached out to them prior to this incident. A review of the child care rule section .1803, your facility discipline policy, and additional training is needed. The Southwester Child Development Commission offers Managing Emotions & Self-Regulation, a self-paced/on demand training. Participants in this training will be given the skills needed for self-regulation in children, steps to supporting self-regulation for children, and an action plan template. We reviewed the resource library on the National Center for Pyramid Model Innovations (NCPMI) website. Additionally, we looked at classroom tools that can be printed and reviewed with staff. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that confirmed complaints and repeated violations of this nature during consecutive visits may lead to an administrative action. A follow up visit may be conducted in the near future to monitor compliance with child care requirements. Your program currently operates with a five-star license, issued 9/6/22, earning 7 points in the education component, 6 points in the program standards component (meeting all enhanced requirements) and 1 quality point for the programmatic option of staff benefits package and infrastructure of parent involvement. The last annual compliance visit was conducted 10/26/22. The sanitation inspection was completed 6/30/23 with a “Superior” classification. The last fire inspection was conducted 6/2/23 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of 10/10/23. The NC Secretary of State website was reviewed on 10/10/23 and Children’s Campus of Greenville, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Children enrolled in spaces #1 and #2 received care according to individual needs including napping, diapering, and bottle feeding. Safe sleep checks were conducted and recorded as required. Teachers sat near the children, talking to them as assisting them with rolling over, walking, and play. Children’s art work were displayed on the walls of each classrooms. Teachers moved about the spaces, supervising the children and engaging in conversation play with the children. Teachers were observed comforting children that were upset, assisting with problem solving and asking open ended questions to encourage reasoning skills. The outdoor area offered station and portable climbers, balls, and push and ride toys to encourage vigorous gross motor play. Lunch was observed and consisted of turkey and cheese sandwiches, green beans, tater tots, and milk. The following violations were documented.
