J. Branch Jr. , operator assisted me with the visit. Your program currently operates with a five-star license, issued 11/24/2025 having family and community engagement foundational practices with three additional choices, a continuous quality improvement plan on file, using an approved curriculum (Ghee Whiz) when four years old are enrolled, scoring 4.97 on the FCCERS, and meeting five -star education requirements. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 4/21/26. The NC Secretary of State website was reviewed on 4/21/26 and Brilliant Minds Enrichment Childcare, LLC was listed as current-active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website. The facility name listed did not include LLC, you informed me that the name of the facility is Brillant Minds Enrichment Center, LLC. We discussed you will receive a new license in the mail. Contact me anytime you need to make any changes to your contact information, e.g., phone numbers, mailing address. I visited indoor and outdoor spaces of your family child care home. You and an additional caregiver were present in the designated child care space. Children were observed during free play. The additional caregiver used flashcards to assist children in counting and played games with the children. Later the children transitioned to the outdoor space, followed by lunch. Lunch was observed and included chicken nuggets, greenbeans, pineapples, wheat bread and milk. Following lunch the children rested on linen covered cots. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Stored in a drawer below five feet in the kitchen were three screwdrivers. .1719 (a) (1) 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. Two travel packs of Tylenol were stored in a First Aid Kit on a shelf below five feet. .1719(a)(5) 908 Health questionnaire was not completed annually. The operators health questionnaire expired 11/25/25. .1703(a)(1) 1891 Individuals providing care to children did not receive 16 hours of orientation prior to beginning caregiving activities. Orientation documentation was not completed for an additional caregiver hired on 2/25/26. .1729( c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance regarding violations: Staff Records: The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation to include emergency contact information. The operator’s health questionnaire expired on 11/25/26. I shared with you the health questionnaire must be updated annually. Safe Environment: Building and physical premises safety refers to actively identifying and mitigating potential hazards within a building or outdoor space that could cause bodily harm. By implementing preventative measures and safety protocols to protect individuals on the premises the risk of injury is reduced. Today there were two travel packs of Tylenol stored in a first aid kit on a shelf below five feet. There were screwdrivers stored in the bottom drawer of the kitchen counter. You removed the Tylenol during the visit. A storage of hazardous items poster was shared with you during the visit. Orientation: Staff qualifications and training requirements are essential for adults who interact with and teach children. These requirements protect children in child care facilities by ensuring these facilities provide a physically safe and healthy environment where the developmental needs of children are being met and where children are cared for by qualified staff. Each staff person shall receive 16 hours of orientation prior to caring for children. An orientation form was on file but completed for an additional caregiver hired on 2/25/26. We reviewed the importance of documenting as things occur. Additional Comments: You may begin on-going training hours May 23, 2026. You also shared that you will complete EPR training on May 19, 2026. We also discussed you are required to complete 2.5 CEUs annually for your education standards, we discussed using your on-going training log to document this requirement. ABCMS- You have completed the ABCMS provider portal training. We discussed submitting your powerform to DCDEE _ABCMS_Provider@dhhs.nc.gov. Once you have access, creating connecting applications for all caregivers. Reminders: The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. The team is made up of specialists with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children.
