B. Moore, teacher, assisted me with the visit. Your program currently operates with a four-star license, issued 11/2/2022, earning 5 points in the education component, 4 points in the program standards component (meeting enhanced ratios) and 1 quality point for the child care administrator has at least 10 years of documented child care administration work experience in a licensed program. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 2/25/25. The sanitation inspection was completed 12/4/25 with an “Superior” classification. The last fire inspection was completed on 3/7/24 and your facility was approved for day and night time care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-seven percent as of 2/10/26. The NC Secretary of State website was reviewed on 2/5/25 and Abundantly Chozen Learning Center LLC was listed as current-active status. I visited all indoor and outdoor spaces. Children enrolled in classroom space #1 were observed during gross motor activities in classroom space #5. The children rode on push cars, used hula hoops, and tossed balls around the space. In classroom space #2 children were observed during free play in designated learning areas. Lunch was observed and included chicken nuggets, mixed veggies, pears and milk. The activity plan posted in classroom space #2 was dated February 2-6, 2026. The doorknob to the security door to enter the classroom spaces is broken. Conduct regular safety walk-through of the facility to identify any hazards. Vehicle Maintenance: Children should be cared for and transported in safe environments. Maintaining interior of vehicles is important to the safety of children. Today in the vehicle used to transport children two front tires treads were less than 2/32 of an inch. You stated that you would have the driver get new tires today. Have the driver complete regular safety checklist to ensure tires and other safety standards are maintained. Activity Plan: Activity plans help teachers to plan for and implement daily activities for children. They provide information to families on activities taking place during their children’s day. Planning for a week of activities helps teachers to obtain and prepare the materials and equipment needed to successfully carry out the activities on the plan. Staff informed me that the staff files were in the office and no one on site had a key to the office. You arrived during the visit and provided the staff records. We discussed having a locked cabinet space outside of the office that did not include staff confidential information or providing a key to a designated staff member so that information can be accessed by DCDEE staff when you are not on site. Additional Comments: Weather Watch Chart: Today children were observed during gross motor activity in classroom space #5. The temperature outside was 40 degrees, staff stated they had just come from outside. When determining whether temperatures are safe for children to engage in outdoor play, ensure staff are referring to the weather watch chart and adjust outdoor play times as required. Print a copy of the chart for each teacher. Rated License: Today we reviewed the requirements for the continuous quality improvement plan for the staff and the facility, the portfolio requirements for the children enrolled, and using a approved assessment tools. Clean Classrooms for Carolina Kids: Facilities must test all water outlets used for drinking or food preparation, once every three years. Complete the lead in water testing for the facility. Prior to any meal or snack being served, the posted menu must be reviewed for accuracy of items served and if comparable changes occur, then make sure those changes are noted on the posted menu(s). A safe indoor environment was not provided for the children with damaged items and equipment that were accessible to the children. In classroom space #1 a heater fan was operating on a shelf below five feet exposed to children one to two years of age. Vehicle Maintenance: Children should be cared for and transported in safe environments. Maintaining interior of vehicles is important to the safety of children. Today in the vehicle used to transport children two front tires treads were less than 2/32 of an inch. You stated that you would have the driver get new tires today. Have the driver complete regular safety checklist to ensure tires and other safety standards are maintained. Activity Plan: Activity plans help teachers to plan for and implement daily activities for children. They provide information to families on activities taking place during their children’s day. Planning for a week of activities helps teachers to obtain and prepare the materials and equipment needed to successfully carry out the activities on the plan. In classroom space #2 the posted activity plan was dated February 2-6, 2026. The teacher stated that the wrong week was printed, and presented an activity plan for the current week on the teachers’ phone. Staff printed and posted the plan during the visit. Have staff post activity plans for the upcoming week on Fridays to ensure the activity plan for the upcoming week are posted. Record Access: Staff qualifications are an important part of providing quality care to children. Today when I requested to review staff files, the staff on site did not have access to the files. Staff informed me that the staff files were in the office and no one on site had a key to the office. You arrived during the visit and provided the staff records. We discussed having a locked cabinet space outside of the office that did not include staff confidential information or providing a key to a designated staff member so that information can be accessed by DCDEE staff when you are not on site. Additional Comments: Weather Watch Chart: Today children were observed during gross motor activity in classroom space #5. The temperature outside was 40 degrees, staff stated they had just come from outside. When determining whether temperatures are safe for children to engage in outdoor play, ensure staff are referring to the weather watch chart and adjust outdoor play times as required. Print a copy of the chart for each teacher. Rated License: Today we reviewed the requirements for the continuous quality improvement plan for the staff and the facility, the portfolio requirements for the children enrolled, and using a approved assessment tools. The last fire inspection was completed on 3/7/24 and your facility was approved for day and night time care. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-one percent as of 2/25/25. Children were observed resting on linen covered cots. The lights were dimmed and soft music played. Signage regarding the smoking and tobacco restriction shall be posted at each entrance to the center and in vehicles used to transport children. Attendance: The health and safety of individual children requires that information regarding each child in care be kept and available when needed. Attendance records should accurately reflect the number of children in care at all times. Attendance records in each classroom should include each child that is enrolled in the space and documented as present or absent. Attendance records should also include time records of childrens arrival and departure times. In classroom space #2 nine children were present and the attendance indicated only six children was present. When I brought this to the staff’s attention the staff updated the attendance. Arrival times were indicated for two of the nine children. We discussed that staff should complete arrival/departure times immediately to ensure accurate records are maintained. Plastic Bags: Plastic bags are a suffocation hazard for young children and should be made inaccessible to children less than three years of age. To make plastic bags inaccessible, they will need to be stored out of reach of children, which is above five feet from the finished floor. Review this requirement with staff. In classroom space #4b staff read books and spoke to children under age one using low, gentle tones. Following nap time, the children enrolled in space #1 were observed eating snack, at a child sized table. The snack included cheese-its and 100% cranberry and grape juice. All child care facilities shall comply with all State laws and federal laws and local ordinances that pertain to child health, safety, and welfare. State inspections are required for all vehicles. The state inspection for the Chrysler van used to transport children expired August 2024. We discussed creating a safety checklist for the van that staff can complete to ensure the vehicle meets safety requirements. You stated that the van driver was taking the van to get inspected today. Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Prior to being transported, each child transported must be have on file emergency contact information along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. Three school age children did not have a photograph or emergency information on file. You took the children’s pictures during today’s visit. Tobacco Free Environment: Children shall be in a smoke free and tobacco free environment. Signage regarding the smoking and tobacco restriction shall be posted at each entrance to the center and in vehicles used to transport children. Medication: No drug or medication may be administered to any child without specific written instructions from the child's parent/guardian, a physician, or other authorized health professional. You were not aware that the permission forms were expired for the medications. We discussed when the Pitt County Schools medical authorization form is used, the authorization period is six months. In addition, Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. During our observations of the breakfast served to the children, I noted food substitutions were of comparable food value; however, the substitutions were not recorded on the menu. According to the posted menu, the breakfast served consisted of waffles, fruit cocktail, and milk. Today children were served mini pancakes, mixed fruit and milk. Prior to any meal or snack being served, the posted menu must be reviewed for accuracy of items served and if comparable changes occur, then make sure those changes are noted on the posted menu(s). Safe Environment: A safe indoor and outdoor environment was not provided for the children. On four occasions the entry door was observed not closing after entry. A wrench was observed securing the lock on the entry door. A panel was protruding from the fence on the playground creating a space for entrapment. The nails of the panel were exposed. Ants were observed on the border of the sand area. There was two pieces of broken balloons and five pieces of paper and plastic on the playground and the lining under the sand was exposed creating a tripping hazard. We discussed that the entry door should always be secure. Conduct safety checks daily to ensure any potential dangers are addressed immediately. Playground Surfacing: The sand at the base of the slide measured 5 ½ inches. In order to continue providing a safe outdoor environment, be sure to maintain the six-inch depth and frequently fluff the materials if it becomes compact due to routine play or rainy weather. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. In space #3 a can of Lysol Disinfectant spray, Home Disinfectant spray, and ZEP disinfectant wipes were stored on a shelve above five feet but not locked. We discussed reviewing storage of hazardous items with staff. Playground Inspections: A monthly playground inspection shall be conducted by an individual trained in playground safety requirements. Playground inspections reviewed for June 2023 and July 2023 were conducted by staff that did not have documentation of completion of Playground Safety training. Have any staff inspection playgrounds complete the required training. Additional Comments: During todays visit portable cooling devices were in the lobby and in space #1. Monitor temperature in spaces were caring for children to ensure temperatures do not exceed the 85 degrees Fahrenheit. Violations were documented for failure to maintain compliance with nutrition, safe environment, and storage of hazardous items requirements. Violations of this nature directly impact the safety of the children while in your care. During the visit, we discussed that repeated violations of this nature during consecutive visits may lead to an administrative action against your license. The allegations are as follows: There is a concern that the outdoor learning environment is unsafe. There are concerns that the meals provided are not following Meal Patterns for children in childcare. Enrollment: Space #2: Enrolled: 6 children 3 years old, 7 children four years old, 1 child five years old Present: 1 child 3 years old, 2 children four years old Staff: E. Harris Space #3: Enrolled: 1 child 5 years old, 25 children 6-11 years old Present: 1 child 5 years old, 1 – child 6 years old Staff: D. Parker Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Files for selected staff were reviewed. I observed indoor environments including the kitchen. I observed children during free play with age-appropriate materials. Based on the information provided regarding concerns that the that the outdoor learning environment is unsafe. I monitored the outdoor learning environment and observed a panel of the fence is protruding creating a space for entrapment and the nails of the panel were exposed. Additionally, ants were observed along the border of the sand area, the depth of the sand measured 5 ½ inches, the lining of the sand was exposed, there was pieces of plastic and paper debris in the sand. I reviewed monthly playground inspection checklists. Based on observations, this allegation is confirmed. Children in space #2 were served three mini pancakes, mixed fruit and milk, and children in space #3 were served four mini pancakes, mixed fruit and milk. I observed serving utensils used to prepare the mixed fruit, and bags of mini pancakes stored in the freezer. The bags did not have a label with serving sizes. Based on collateral reports and observation the allegation in confirmed. The following violations were observed. In space #3 stored on a shelf above five feet but not locked was a can of Lysol spray, Home disinfectant spray and a container of ZEP disinfectant wipes. .2820(b) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Playground inspections on file for June 2023 and July 2023 were completed by staff that did not have documentation of Playground Safety training received. .0605(q) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The surfacing at the base of the slide measured 5 1/2 inches. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 8, 2023 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected.
