M. Tyson, owner assisted me with today’s visit. Your program currently operates with a five-star license, issued 9/22/24 earning 5 points in the education component, 7 points in the program standards component (meeting enhanced space and reduced ratio) and 1 quality point for the child care administrator having at least 10 years of documented child care administration work experience. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-one percent as of 7/8/26. The NC Secretary of State website was reviewed on 7/8/26 and your facility is not owned by a corporation. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Throughout the classroom spaces children rested on linen covered cots. The lights were dimmed. Staff files for new staff were reviewed. A fire inspection was conducted at the facility on 2/5/26 and your facility was approved for daytime and night care. A sanitation inspection was conducted on 3/5/26 with a “Superior” classification. The following violations were observed. Violation Number Comment Rule 303 Children were not adequately supervised at all times. When I arrived at the facility staff assigned to classroom space #4 was observed outside the facility. Staff stated the staff member assigned to classroom space #1, stood in the hallway to monitor classroom space #4 and classroom space #1 while staff was outside the building. .1801(a)(1-5) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. On the playground, there bench seat was broken, and there were two broken baskets. 15A NCAC 18A .2832(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A Ventolin HFA inhaler for a child in classroom space #3 expired June 2026. .0803(12) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The facilities EPR plan was last reviewed on April 10, 2025. .0607(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. We discussed that staff must be positioned to render assistance at all times. Review supervision with new staff, consider forming a plan for supervision when you are away from the building. Medication: Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. Proper disposal of medications is important to help ensure a healthy environment for children in our communities. A Ventolin HFA inhaler for a child in classroom space #3 expired June 2026. We discussed returning all unused or expired medications to parents within 72 hours. A outdoor environment was not provided for the children with damaged items and equipment that were accessible to the children. On the playground, there bench seat was broken, and there were two broken baskets. Staff should remove broken equipment daily or inform you when equipment is damaged. Prior to using the playground inspect daily for hazards. EPR Plan: Your programs Emergency Preparedness and Response plan was created to assist you with following a plan in the event of an emergency. You must update your plan yearly or update the plan whenever changes occur in your program. Your facilities emergency response plan must be reviewed with staff annually or when changes occur. Your last documented review was April 10, 2026. Calendar and set a reminder for your annual review. Additional Comments: Today we discussed Pathways to the Stars, I reviewed with you the Classroom and Instructional Quality requirements. You will need to select an approved formative assessment tool and complete training on the selected assessment. Sleep checks were completed, recorded, and maintained as required. Fire and sanitation inspections have not occurred since the last visit and remain current. The following violation was observed. This program currently operates with a Temporary License issued March 21, 2024. The temporary license is valid through September 21. This facility is owned and operated by M. Tyson. The children were engaged in morning group activities. Colors in English and Spanish were reviewed, animals were identified using picture cards and children jumped and danced to shake the wiggles out. Children’s artwork was displayed throughout each space at the children’s eye level. The teachers sat on the floor with the children as they engaged in indoor free play, rotating as the children moved from learning center to learning center. The administrator assisted the children with toileting and handwashing routines prior to transitioning to outdoor free play. The fenced outdoor space located at the rear of the facility includes portable play equipment as well as a shaded play space. Staff, children, and program were monitored. Lunch included a chicken sandwich, sweet peas, mixed fruit, and milk. Teachers sat with and engaged in conversation with the children as they ate lunch. After lunch the children washed hands and transitioned to linen covered cots. Soft music played in the background. Teachers cleaned and sanitized the tables used for lunch, sweeping the floor afterwards. COMPLIANCE HISTORY - The compliance history for this facility prior to today’s visit is 100%. The violations documented today may affect the facility’s compliance history score. If a facility’s compliance history falls below 75% over an eighteen-month time frame, some type of administrative action may be issued. The following violations were documented.
