T. Pratt, administrator assisted me with today’s visit. Your program currently operates with a five-star license, issued 6/27/22 earning 7 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having a infrastructure of family involvement and staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of 6/11/2026. The NC Secretary of State website was reviewed on 6/1/2026 and Walton Companies, INC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. The facility fax number was removed. You stated that your facility no longer has a fax machine. Contact me if you need to make any other changes to your contact information, e.g., phone numbers, mailing address. I provided you with a facility application to update the facilities mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today. Children were observed via monitors in the lobby and in the indoor learning environments. Throughout the classroom spaces children were observed eating lunch and transitioning to rest time. Lunch included greenbeans, sunbutter sandwiches, pears and milk. Files for new staff were reviewed. A sanitation inspection was conducted at your facility on 6/4/25 and a “superior” rating was achieved. The fire inspection remains current. The following violation was documented. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In classroom space #16 there was an epi-pen that expired in March 2026. 10A NCAC 09 .0803(1)(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Staff contacted the parent and the authorization form was updated during the visit, therefore there is no further action is required. Additional Comments: Today we reviewed the Divison’s Website “Whats New” QRIS Modernization and the NCRLAP website “Get Ready for the 3’s”. NCRLAP Outreach Assessment- An Outreach Assessment offered by NCRLAP is a fantastic opportunity for programs to get a head start on understanding the Environment Rating Scales (ECERS-3, ITERS-3, SACERS-U, and FCCERS-3) without the stress of it impacting their Star Rated License. Think of it as a friendly, no-pressure practice run that helps you pinpoint your program’s strengths and areas for improvement. Here’s how it works: 1. Tamaria Williams, Child Care Consultant accompanied me during today’s visit. T. Pratt, administrator assisted me with the visit. Your program currently operates with a five-star license, issued 6/27/2022, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for having an infrastructure of parent involvement and staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 11/21/2023. The sanitation inspection was completed 6/20/2024 with a “Superior” classification. The last fire inspection was conducted 11/20/23 and your facility was approved for day time and night care. The center's compliance history was reviewed with the administrator. The program’s compliance history was eighty-eight percent as of 10/29/24. The NC Secretary of State website was reviewed on 10/28/2024 and Walton Companies, INC. was listed as current- active. You visited indoor and outdoor spaces with me. Children in classroom space #9 were observed sitting on the carpet as the teacher read a book about a dragon, the teacher asked the children opened questions about what was happening in the story. Children one year of age were observed during outdoor play. The children were playing in a play house and riding on push toys. Children’s fall themed art activities were posted on classroom walls. Today’s lunch included chicken nuggets, peas, strawberries, whole grain rolls, and milk. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In outdoor spaces #3 and #4 the black lining and plastic netting was exposed. 10A NCAC 09 .0601(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. On child being transported did not have a photograph attached to their emergency information. 10A NCAC 09 .1003(d) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One of nineteen children's records reviewed did not have a immunization record on file within 30 days of enrollment. 10A NCAC 09 .0302(d)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical assistance was provided on the following: Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Each child transported must be have on file emergency contact information along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. One school age child did not have a photograph attached to their emergency information. You stated that the child’s first day attending the facility was yesterday. K. Justice created a reminder to take a picture of the child this when they arrived at the facility this afternoon. During enrollment have parents supply a recent picture of their child, so that the required information is on file the child’s first day. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The black lining and plastic webbing was exposed in the sand box area in outdoor space #3 and 4. You had staff remove the exposed pieces during the visit. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. Within in 30 days of enrollment a child must have a health assessment on file and each child must also have an immunization record on file. One child did not have immunization records on file within 30 days of enrollment. Review children’s files often and use the Center children’s checklist to ensure you have all required documents within the specified timeframe. Additional Comments: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Each child transported must be have on file emergency contact information along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. One school age child did not have a photograph attached to their emergency information. You stated that the child’s first day attending the facility was yesterday. K. Justice created a reminder to take a picture of the child this when they arrived at the facility this afternoon. During enrollment have parents supply a recent picture of their child, so that the required information is on file the child’s first day. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The black lining and plastic webbing was exposed in the sand box area in outdoor space #3 and 4. You had staff remove the exposed pieces during the visit. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. Within in 30 days of enrollment a child must have a health assessment on file and each child must also have an immunization record on file. One child did not have immunization records on file within 30 days of enrollment. Review children’s files often and use the Center children’s checklist to ensure you have all required documents within the specified timeframe. Additional Comments: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. The last annual compliance visit was conducted 12/8/2022. The sanitation inspection was completed 6/13/23 with a “Superior” classification. The last fire inspection was conducted 11/20/23 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight percent as of 11/2/23. The NC Secretary of State website was reviewed on 11/20/23 and Walton Companies, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. I observed arrival of children at the facility. Staff and children were observed in transitions between classrooms throughout the facility. Staff were observed serving breakfast. Breakfast included waffles, peaches, and milk. Children were observed during outdoor play on stationary equipment and swings. Lunch was observed and consisted of cheese pizza, lima beans, strawberries and milk. The following violations were documented. Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. The director was observed pushing four children in a bye-bye buggy and two additional children were walking with the director. Staff-child ratio refers to the maximum number of children who may be supervised by one staff member. In any multi age group situation, the staff/child ratio for the youngest child in the group shall be maintained for the entire group. Children under one year of age shall be kept separate from children two years of age and over. Children between the ages of 12 months and 24 months shall not be grouped with older children unless all children in the group are less than three years old. Today, when we arrived you were pushing four children in a bye-bye buggy and had two additional children walking with you. Three children were under 12 months old, one child was one years of age and two children three years of age. You stated that you were in the infant classroom and there were staff that had called out this morning. The children where in your care until you were able to transition the children when staff arrived. We discussed the hours your staff are scheduled to report to work and you also had additional staff arrive from other facilities within the company to assist. Medication: No drug or medication may be administered to any child without specific written instructions from the child's parent/guardian, a physician, or other authorized health professional. In space #17 the written authorization for Boudreaux Buttpaste expired on 8/30/23 and 9/13/23 for PC diaper rash cream. You removed the creams from the classroom and placed them in your office to be returned to the parent. Incident Reports and Log: You must now document every injury incurred while a child is in your care, including cuts, scrapes, bumps, and bitemarks. These incidents must be recorded on the incident report form supplied by the Division. The incident must then be logged on the facility incident report log and the completed report placed in the child’s file. Nine incident reports from October and November were not documented in the incident log. You updated the log during today’s visit. The incident log should be used to track patterns of injuries and adjust routines or the learning environment to prevent further injuries. It must be kept with other program records for review during monitoring and complaint visits. Plastic Bags: Plastic bags are a suffocation hazard for young children and should be made inaccessible to children less than three years of age. To make plastic bags inaccessible, they will need to be stored out of reach of children, which is above five feet from the finished floor. In space #16 a zip loc bag with children’s clothes was stored in a child’s cubby below five feet. In space #17 plastic bags with diapers were stored below five feet under the diaper table in an unlocked cabinet. When this was brought to your attention, you immediately removed the plastic bags. Your program currently operates with a five star license, issued 6/27/2022, earning 7 points in the education component, 6 points in the program standards component(meeting enhanced ratios) and 1 quality point for having staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 12/8/2022. The sanitation inspection was completed 6/13/23 with a “Superior” classification. The last fire inspection was conducted 11/20/23 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight percent as of 11/2/23. The NC Secretary of State website was reviewed on 11/20/23 and Walton Companies, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current.
