Name of Operation: YESS Learning Center - Vandalia Facility ID: 41002038 Consultant: CHERISSA ALSTON Operation Type: Center Case Number: Visit Date: 12/5/2024 Number Present: 94 Completed Date: 12/5/2024 Age: From 0 To 11 Total Minutes: 405 Time In: 10:00 AM Time Out: 04:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance of child care rules during an annual compliance visit. The facility has an 18-month compliance history of 96%. The last annual compliance visit was documented on 12/12/23. The facility’s corporation status was verified current on the NC Department of the Secretary of State on December 5, 2024. Upon arrival, I was greeted by Kimberley Dymond, Assistant Director. Jiandaa Turner, Director, accompanied me on the walkthrough of the facility. 94 children were present today. Children were observed playing with toys in the various centers and playing outside. Infants were observed eating, being fed, and sitting in bouncer chairs near the teacher. Emergency medications, topical creams, and permissions were monitored today. Outdoor spaces were monitored, and some areas of non-compliance were observed today. Outdoor inspections were documented monthly. I also monitored two buses used to transport school-age children. There were binders with children’s identifying information and bus driver information. There was a first aid kit and a fire extinguisher on the bus. Seats were in good repair on both buses. The registration and proof of insurance were monitored and found to be current. Ms. Turner assisted me with staff and children’s files and other required documentation. Nine randomly selected children’s files and staff files were monitored during today’s visit. A satisfactory fire inspection was completed on 1/25/24 and the facility was approved for daytime care only. Emergency drills were documented monthly. A sanitation inspection was completed on 8/9/24 and the facility received a Superior rating. Required items were posted and current. There were four (4) items of non-compliance were observed today, and the four (4 ) corresponding violations were cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The swing structures and ant structure all had chipping paint. There were several tree roots and concrete areas exposed on the two's and preschool playgrounds. There was also grass growing through the mulched area. 10A NCAC 09 .0601(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. There was expired Vaseline, Nystatin and Derma Smooth did not have instructions on how to use it and the end date for the prescription. .0803(12) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for L. Dotson was expired. It expired on 9/18/24. G.S. 110-90.2(b) & .2703(n)&(o) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. Three medical action plans were not updated annually. One of them was updated and received from the parent before the end of the visit. .0801(b) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 12/10/24, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. During today's visit, I discussed the importance of materials being accessible to the children at all times. In two of the licensed spaces, there were plenty of toys but they were up on the shelf. Teachers explained they had been cleaned the night before and they forgot to bring them down. As a reminder toys must be accessible, and there should be a rotation of toys available. If toys have to be cleaned the rotational toys can come out for use. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs.
