Ms. Vickey Phillips, Administrator, assisted me with the visit. Your program currently operates with a Five-Star license effective June 29, 2017. The license was posted, and the restrictions were in compliance. The last fire inspection was completed on June 13, 2024. A sanitation inspection was completed on November 24, 2025, and was given a Superior classification. A walk-through of the facility was completed today; all indoor areas were monitored. I observed children in the indoor learning environments. Children were participating in center activities, art projects, personal care routines and eating lunch. Infants were being held, and in bouncy chairs. Four toddlers were eating lunch. Four violations were observed today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed June 13,2024. 10A NCAC 09 .0304(a) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Dates were not on two bottles. 15A NCAC 18A .2804(d) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Documents were not completed for visually checking on sleeping for two infants present. The last documented check for the infants were December 01,2025 and December 02, 2025. .0606(g) 1301 Center did not maintain a record of daily attendance. Attendance for toddlers present in the infant room had not been completed. GS 110-91(9) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before December 22, 2025. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. -Safe sleep position checks: It is important to check on sleeping infants. Use of safe sleep policies and continued education efforts for child care professionals and increased monitoring and observation of infants while they are sleeping are critical to reduce the risk of Sudden Infant Death Syndrome in child care. -Daily attendance: Attendance for toddlers present in the infant room had not been completed. -Bottles for children must be dated and labeled with the child's name. Two bottles did not have dates. -We reviewed QRIS Pathway 1- Program Assessment for Child Care Centers. Together we reviewed the application process. - WORKS- Please ensure that all staff member’s education information is current in the W.O.R.K.S. database. Each staff member must register in the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment before the end of the temporary time period. You may also want to consider having staff members complete the Early Educator Certification process to assist in getting higher education evaluations completed if their degree is not in early childhood education. The last annual compliance visit was conducted on 6/25/2024. An Annual Compliance Monitoring Checklist for Centers checklist was used to note the requirements I monitored today. The facility has an 18-month compliance history of 95%. This limited liability company is current-active as of June 10, 2025. Upon arrival I was greeted by V. Phillips. Ms. Phillps, the Administrator, assisted me with the visit. I walked through the indoor and outdoor licensed spaces of the facility that the children use during the visit. I observed the children eating lunch, transitional activities, and outside activities. Infants were being held, and toddlers were eating lunch. Staff interacted with the children in a pleasant manner. Twenty-six children were present during today’s visit. Twenty-six children were present. I reviewed three out of twenty-six children’s files and two new staff files. We reviewed staff files and staff and training worksheets together during the visit today All inspections were current and complete. A sanitation inspection was completed on May 21, 2025, with a superior rating and twelve demerits. The fire inspection was completed on June 13, 2024, and I received a copy during the last annual compliance visit. We reviewed requirements for off premise activities in child care centers, requirements for staff, information for children and required items to be posted. Ms. V. Phillips provided all the necessary forms and information requested and is meeting requirements for transportation. I monitored two vehicles used to transport children. The license/tags and insurance for both vehicles were current. The vehicles were equipped with a fire extinguisher, first aid kits, emergency identifying information for children and staff and a cellular telephone. All drivers had CPR/First Aid, current driver’s licenses and proof of insurance to drive the vehicles. A full assessment of applicable child care requirements was conducted during today’s visit as per the attached checklist. During today’s visit one violation of Child Care Requirements was observed and documented. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. A permission form was not provided for one child who had diaper cream. 10A NCAC 09 .0803(1)(a & b) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A letter stating how the violations were corrected and how you plan to maintain compliance in the future must be submitted to me by June 23, 2025. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. 5. Signature of the person preparing the letter. -Analysis is complete for Lead in Water. Lead-Based Paint and Asbestos has not been completed. The provider is taking necessary steps to complete the process. -Starting on February 1, 2025, ECERS-3, ITERS-3, and FCCERS-3, also known as the "3s”will be used for program assessments and reassessments.
