This facility is owned and operated by GenerationED. This center currently operates with a Five Star license reissued on May 28, 2024. Upon arrival to the facility, I was greeted by the lead teacher in charge, Mr. Joshua Sanders. I made you aware of the purpose of today’s visit. Mr. Sanders assisted me during today’s visit. The director, Ms. Miranda Sistrunk was not present during today's visit. In the spaces for toddler-preschool aged children (Early Head Start classrooms) I observed the children actively engaging in free play. The teachers were observed interacting and engaging with the children as they explored their indoor and outdoor learning environment. In one classroom I observed the children singing with their teachers and dancing. Several transitions were observed and appropriate handwashing from the children and staff was completed as necessary. Age-appropriate sufficient materials were also observed in the learning environment. In the space for toddlers individualized lesson plans were also planned for the children. During today’s visit I observed the classrooms for preschool aged children (NC-Pre-K Classrooms). I observed these classrooms during center play, and teacher directed small group activities. The children were also observed during gross motor play, lunch time, and several transition times. The teachers were actively engaged with the children while they moved freely throughout their learning environment. In one classroom I observed the children playing musical chairs and one group of children had water play while they were outside on the playground. Age-appropriate sufficient materials were also observed in the learning environment. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. During my observation the staff was observed using the “Face to Name” method to document the attendance of the children. The teachers were also observed supervising in “zones” as the children played on the playground. I observed a staff member complete a playground sweep as the children transitioned from the playground to the classroom. This same staff member conducted another “Face to Name “check when the children returned to the classroom. Forty-eight (48) children were present during today’s visit. Staff files were monitored for compliance for current Criminal Background qualifying letters and current CPR/First Aid and other specialized training as required. A sanitation inspection was conducted on July 2, 2024. This facility received a superior classification and nine (9) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. This facility’s corporation status was verified current on the NC Department of the Secretary of State on June 4, 2025. The following violations of childcare requirements were cited today: Violation Number Comment Rule 851 When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. .0803(13)(a-e); .2318(3) 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The fire extinguisher had not been inspected since January, 2024. This is a repeat violation. This was cited during the last visit to the facility on 2/18/2025. GS 110-91 Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by June 20, 2025. CONSULTAITON & RESOURCES: MEDICATION: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. Prevention is the key to prevent poisoning by making sure medications are inaccessible to children. Prescribed medications as well as over the counter (OTC) medicines must be labeled properly when brought to the childcare facility by the parent/guardian. The medication must be in the original container (with a label that includes the child’s name, date filled, prescribing clinician’s name, pharmacy name and phone number, dosage/instructions, and relevant warnings). Tracking System: We discussed in detail options to assist you with monitoring on a more consistent basis for compliance with medication requirements in your classrooms. I recommend that you keep a tracking log available for review that lists the medications (prescribed/over the counter) in each classroom. The tracking log could include the medication, the date it expires and when to notify the parents. It is also recommended that you complete at least monthly checks of the classrooms to ensure all required forms, authorization and action plans are current and accessible. Medical Action Plans: Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. As discussed with you today it is important to have all your staff use this time to submit updated educational information to our Workforce system. All staff will need to have a current Workforce letter on file. It is also suggested that you visit the NCRLAP (www.NCRLAP.org) website for upcoming training and view their resources for the ITERS-3, and ECERS-3 . This center currently operates with a Five Star license reissued on May 28, 2024. The center director, Ms. Miranda Sistrunck was not present during today’s visit. Mr. Joshua Saunders and Ms. Khadijah Macklin, Lead Teacher assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was conducted. Seventy-three (73) children were present during today’s visit. During today’s visit I observed the children during center play, small group activities and outdoor play. Several classroom transitions were observed. The children were also observed during meal times and rest time. Positive interaction was observed between the staff and the children. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. A sanitation inspection was conducted on July 2, 2024. This facility received a superior classification and nine (9) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. A satisfactory fire inspection was completed on January 18, 2024. This facility was approved for daytime care only. This facility’s corporation status was verified current on the NC Department of the Secretary of State on February 13, 2025. The following violations of childcare requirements were cited today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facility did not have a current fire inspection. The last inspection was completed on January 18, 2024. 10A NCAC 09 .0304(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. (It is recommended that you use one sheet for each year). Also have at least two years available for review (current year and previous year). --Also please make sure Health & Safety training is documented on the Health & Safety training log with the certificates attached for each staff member. During today’s visit I reviewed with you the importance of having a system to monitor the expiration dates of the action plans, mediation authorization and the expiration dates of medicines. We discussed several options for monitoring the classrooms for compliance. STAFF TRAINING: All staff members that have responsibility for planning and supervision of a childcare program; as well staff who work directly with children must complete in-service training annually. --The required number of in-service training hours to be completed yearly is based on the education and/or experience of each individual staff member. --Please use the Record of In-service Training Chart to keep track of your training completed each year. (It is recommended that you use one sheet for each year). Also have at least two years available for review (current year and previous year). --Also please make sure Health & Safety training is documented on the Health & Safety training log with the certificates attached for each staff member. These trainings should be kept accessible. --If you receive more than the required number of hours required, you may only carry forward half of what you are required for the next year. *For a complete review of the number of annual training hours required please review child care rule.1103 (On-going training and professional development requirements). A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on June 11,2019. Upon my arrival to the facility, I was greeted by Ms. J. Redfear, one of the family advocates. Ms. Redfear informed me that Ms. M. Sistrunk, the director was in a meeting. Ms. Redfear informed me that Mr. J. Saunders was the person in charge when Ms. Sistrunk is not present in the building. I informed Mr. Saunders the purpose of today’s visit. Mr. Saunders gave me the necessary paperwork and assisted me during my visit today. Ms. Sistrunk arrived before my departure and assisted me for the remainder of my visit. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit, I observed the children building with blocks, playing with playdough, coloring, playing outside on the playground, washing their hands in preparation for lunch, eating, lunch, preparing for naptime, napping, waking up from napping and departing. Fifty-four (54) children were present during today’s visit. A sanitation inspection was conducted on December 18,2023. This facility received a superior rating with thirteen (13) demerits. A fire inspection was conducted on January 18,2024. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as of February 21,2024. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A safe outdoor environment was not provided for the children. There was cement exposed at the base of a pole , rocks, tree roots and black tarp on the playground. These were tripping and safety hazards. 10A NCAC 09 .0601(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Rated License Assessment: I also discussed with Ms. Sistrunk the Rated license assessment process will begin July 1,2024. I informed Ms. Sistrunk this facility is in Cohort 2. Today I discussed with Ms. Sistrunk about the two assessment options before the facilities three year rated license reassessment. Ms. Sistrunk informed me that she would let me know when she was ready to request the Prep assessment. Your prep year is July 1,2024 to June 30,2025 and your assessment year is July 1,2025 to June30, 2026. You must let me know if you ready to start the process before June 30,2024.
