I completed an Annual Compliance Monitoring Checklist for Centers during today’s visit, and a completed copy was given to you during the visit today. Upon arrival, I was greeted by Doris Thompson, Administrator. Ms. Thompson assisted me with the visit. Five children were present today. Children were observed engaged in teachers directed activities, playing in a variety of centers and transition activities. Children were being attended to according to their individual needs. Ms. Thompson was observed engaged in activities with the children and was positive in her interactions with the children. Your program currently operates with a three-star license, issued 7/10/2019. The last annual compliance visit was conducted on 7/15/2025. The sanitation inspection was completed on 5/26/2026 with a superior classification. The last fire inspection was conducted on 5/30/2025. A fire inspection was completed on 4/16/2026. However, it was not approved. The provider is currently working with the local fire inspector to correct identified problems and obtain an approved, updated fire inspection. The center's compliance history was reviewed with the operator. The program’s compliance history was 97% as of 7/7/2026. Required items were posted and current. Outdoor inspections were documented monthly. I reviewed two children’s files and staff records were reviewed along with the staff and training worksheet. Lead paint- waiting for on-site visit results. Asbestos- enrollment started. Transportation is not provided by this facility. One violation of child care requirements observed on this visit. Violation Number Comment Rule 718 Swings did not have surfacing that extends two times the length of the pivot point to the surface below beyond the external limits of the equipment. .0605(m)(2) The violation documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by 7/17/2026, stating that all violations have been corrected. The letter can be emailed to elizabeth.childress@dhhs.nc.gov. If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by 07/17/2026, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. Technical assistance was provided on the following: Surfacing - The number one cause of injury on playgrounds, is falls to the surface. Maintaining the correct depth of protective surfacing material is crucial for safety. On the playground there is a metal swing structure. Swings did not have surfacing that extends two times the length of the pivot point to the surface below beyond the external limits of the equipment. We discussed the following consultation items today: Rated License Review: During today’s visit, we discussed your transition to the new QRIS Rated License. We reviewed QRIS Pathway 2- Classroom and Instructional Quality for Centers and the requirements. I shared resource and planning documents with you. WORKS: Please ensure that all staff members’ education information is current in the W.O.R.K.S. database. Each staff member must register with the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Please remember it is your responsibility to know and follow all child care rules and requirements. Doris Thompson, owner/operator, greeted me. Ms. Thompson assisted me with the visit. Your program currently operates with a Three-Star license effective July 10, 2019. The license was posted, and the restrictions were in compliance. The last fire inspection was completed on 5/30/25. A sanitation inspection was completed on December 09, 2025, and was given a Superior classification. A walk-through of the family child care home was completed today; all indoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in center activities, personal care routines and table activities. Two violations were observed today. The violations must be corrected immediately. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members had expired training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two staff members had expired training. .1102(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before January 29, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Two staff had expired CPR/First Aid training. a. Must be approved agency b. Face-to-face only; no on-line or blended courses c. Counts for Health & Safety Training d. Renew every two years before the expiration date on the certification card (does not count towards on-going training hours) e. At least one person with current CPR/First Aid must always be on-site. If transporting children a staff member with current CPR/First must be in the vehicle. We discussed: QRIS Modernization Pathways: The Pathway Discussion Template was discussed and completed. An Annual Compliance Monitoring Checklist for Centers was completed during this visit. The facility has an 18-month compliance history of 99%. The last annual compliance visit was documented on 8/1/2023. Upon arrival, I was greeted by D. Thompson, Owner/Lead Teacher. Ms. Thompson accompanied me on the walkthrough of the facility. There were five children present today. Children were observed during arrival, eating breakfast and playing in a variety of centers. Ms. Thompson’s interactions with the children were positive. Five children's records and two staff members file were monitored. Required program records were also monitored during today’s visit. A satisfactory fire inspection was completed April 5, 2024. A sanitation inspection was completed on 4/15/2024, and the facility received a Superior rating. Required items were posted and current. The outdoor space was monitored, and one violation was observed today. There were two air conditioning units exposed. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Air condition units were exposed. 10A NCAC 09 .0601(a) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 7/30/2024, describing how and when the violation(s) were corrected and how compliance will be maintained in the future.
