A checklist was used to note the requirements I monitored today. I was greeted by Ms. Latonya Turner, owner/director of the childcare center. This center currently operates with a Three-Star license, effective September 10, 2018. A full assessment of child care requirements was conducted during today’s visit. A walk-thru of the indoor and outdoor spaces used by the children was monitored. Eighteen (18) children were present during today’s visit. During today’s visit I observed the children during center play, mealtime, during transitions and outdoor play. One infant was observed sleeping in a crib, while three infants were playing on the floor. Positive interaction was observed between the staff and the children. A sanitation inspection was conducted on September 15, 2025. This facility received a superior classification. An approved fire inspection was conducted on January 15, 2025. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. A sample of staff and children’s files were monitored during today’s visit. I received the staff and training worksheets via email prior to today’s visit. Your signature on these forms will serve as verification that the information provided is accurate and complete. I monitored the vehicle the facility uses for transportation. The required information was available for review. I cited one item of non-compliance of childcare rules during the visit today. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One child's medication was not stored in its original pharmacy-dispensed container with the required label. Instead, it was placed in an unlabeled plastic bag that did not include the child's name. .0803(2)(a) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before October 28, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If sufficient information is not received by the due date, a follow-up visit may be conducted. Email the information to: sharon.y.williams@dhhs.nc.gov Pathway to Stars Today, we discussed the Pathway to Stars and I showed you how to access the relevant information on the DCDEE website. The transition to Pathway to Stars will begin soon, and a new rule rollout module is now available on the DCDEE e-learning Moodle platform. We specifically went over the updated rules in Section .3200 regarding the Standards for Two through Five Star Rated facilities. We discussed each of the available pathway options: the Program Assessment Pathway, the Classroom and Instructional Quality Pathway, and the Accreditation and Head Start Pathway. Education requirements were also reviewed. I walked you through how to find this information on the DCDEE website, and we reviewed the forms for Pathway 1 and Pathway 2. You mentioned that you need more time to review the details to determine which Pathway is the best fit for your facility. We also touched on staffing concerns, particularly the education requirements for lead teachers. Clean Classrooms for Carolina Kids All schools and childcare facilities within school districts must complete the lead-based paint and asbestos enrollment surveys prior to receiving on-site inspections. This is a legislatively mandated initiative, and facilities are required to comply with the applicable rules (NC 10A NCAC 41C .1001–.1007; NC 15A NCAC 18A .2816). The Clean Classrooms for Carolina Kids program is designed to help facilities meet these requirements by providing direct support throughout the process, as well as access to communication resources. This free program includes testing, inspections, and any necessary mitigation, and aims to improve the safety of child-occupied facilities across North Carolina. You have only completed lead testing in water. Full environmental testing includes lead in water, lead-based paint, and asbestos. To complete the required testing in all three areas, please register at: https://www.cleanwaterforuskids.org/en/carolina/. Be sure to follow all the guidance provided regarding any necessary remediation. Please also note that childcare centers are required to complete this testing every three years. This facility currently operates with a Three Star license issued on September 10, 2018. Upon arrival to the facility, I was greeted by the administrator, Ms. LaTonya Turner. Ms. Turner assisted me during my visit today. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. A walk-thru of the indoor and outdoor spaces used by the children was monitored. Twenty-one (21) children were present during today’s visit. I observed the children playing outside, completing bathroom routines, eating lunch, participating in teacher directed and free choice activities. In the infant room, I observed two infants with the teacher playing on the floor and one infant sleeping in a crib. Staff and child ratios were observed in compliance during today’s visit. In the classroom for infants and toddlers, I observed the teachers providing routine care and interacting with the children in a nurturing manner. A sanitation inspection was conducted on December 18, 2024, resulting in a superior classification. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. During today's visit, one violation of child care requirements was cited. The violation was corrected during the visit therefore a letter of compliance is not required to be emailed to me. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. On the preschool playground vegetation was growing in the mulch. Ms. Turner corrected this during the visit by pulling up the vegetation and discarding it. 15A NCAC 18A .2832(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. We discussed: Clean Classrooms for Carolina Kids Due to the recent impacts of Hurricane Helene and other natural disasters, the Clean Classrooms for Carolina Kids enrollment deadline has been revised statewide, and schools and child care facilities that have not yet enrolled should complete the online process by May 31, 2025. Enroll now at: https://www.cleanwaterforcarolinakids.org/enroll The lead-based paint and asbestos enrollment surveys must be completed for ALL schools and childcare facilities within school districts prior to receiving on-site inspections. As this is a legislatively mandated effort, facilities are required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina Kids™ is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. This program is free, covers testing, inspections, and mitigation, and is intended to improve child-occupied facilities across North Carolina. Some programs have only completed the process for lead testing in water. Testing includes lead in water, lead based paint, and asbestos. You should complete the online process for all 3 by May 31, 2025. In addition, testing should also be completed every three years for childcare centers. Extension of Hold Harmless provisions - star rated license assessments and reassessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments continue to be required for new child care programs. Updated Program Assessment Tool – Beginning on February 1, 2025, child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. This updated edition includes language and interaction measures, a need consistently identified by early childhood professionals during listening sessions. Latonya Turner, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Turner and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. A staff member failed to maintain an accurate account of daily departures of enrolled children on 8/28/24. 10A NCAC 09 .0302(d)(4) 303 Children were not adequately supervised at all times. Staff members responsible for the supervision of children while on the playground on 8/28/24, failed to provide adequate supervision for children and were not aware or in a position to intervene when an incident occurred between two children and a child sustained injuries. .1801(a)(1-5) 415 A current schedule was not posted for each group of children for reference. The facility had a current schedule posted in each classroom however a staff member failed to follow the posted schedule of activities in the classroom for three and four-year-old children. GS 110-91(12);.0508(a) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. A staff member failed to complete an incident report with all information required in rule and was not signed by a parent. .0802 (e) 853 Incident logs were not completed and maintained as required. A staff member failed to utilize an incident log with all information required within the child care rule for an incident that occurred on 8/28/24. .0802(g)(1-6) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members failed to successfully renew their First Aid certification on or before the expiration. .1102(c) 1911 An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. Meg Riddle, Licensing Supervisor was present and assisted during today’s visit. This facility currently operates with a Three Star license issued on September 10, 2018. Upon arrival to the facility, I was greeted by the administrator, Ms. LaTonya Turner. Ms. Turner assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was monitored. Twenty-seven (27) children were present during today’s visit. The children were observed during center time playing in various, meal time, during outdoor play and during several transitions. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. All staff had current Criminal Background qualifying letters on file and current CPR/First Aid Training as required. One new staff file was reviewed. A sanitation inspection was conducted on December 18, 2023. This facility received a superior classification and twelve (12) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. An evacuation crib was not available in the space for infants. .0604(r) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please email me this letter by May 30, 2024. Playground Recommendations: During today’s visit several hazards were observed on the playground area. It is imperative to keep your outdoor area safe for the children to use at all times. We discussed an older piece of playground equipment that continues to require a lot of maintenance to stay compliance, it was suggested that you remove this equipment. As a reminder continue with your daily checks of the playground area before the children go outside. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. Health & Safety Requirements were also reviewed. RATED LICENSE UPDATE: As you have been made aware the hold harmless legislation was extended and signed into law on Monday June 12, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. You are in COHORT #1. Your preparation year for your three-year reassessment began July 2023-June 2024. ** You attended my training on January 29, 2024, where I reviewed the upcoming Rating License process and the recommended steps I suggested, to assist you with preparing for your upcoming renewal year. After the training you contacted me and stated you wanted to have a practice ERS assessment conducted at your facility. This would be the first time you have had the assessment conducted. I requested the ERS for you on your behalf and your assessment window will begin on May 27, 2024. I also conducted a technical assistance visit with you on April 24, 2024, to assist you with preparing for your ERS assessment. I also recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024, and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. --Have all staff update their WORKS account to reflect all currently completed coursework (DCDEE-WORKS). --Request technical assistance with your Child Care Consultant and local partners. --Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. --Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) --Reach out to your local Community College to discuss educational opportunities UPDATED RULES: Effective July 1, 2023, we have new rules posted on the DCDEE website. RATED LICENSE UPDATE: As you have been made aware the hold harmless legislation was extended and signed into law on Monday June 12, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. You are in COHORT #1. Your preparation year for your three-year reassessment began July 2023-June 2024. ** You attended my training on January 29, 2024, where I reviewed the upcoming Rating License process and the recommended steps I suggested, to assist you with preparing for your upcoming renewal year. After the training you contacted me and stated you wanted to have a practice ERS assessment conducted at your facility. This would be the first time you have had the assessment conducted. I requested the ERS for you on your behalf and your assessment window will begin on May 27, 2024. I also conducted a technical assistance visit with you on April 24, 2024, to assist you with preparing for your ERS assessment. I also recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024, and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. --Have all staff update their WORKS account to reflect all currently completed coursework (DCDEE-WORKS). --Request technical assistance with your Child Care Consultant and local partners. --Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. --Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) --Reach out to your local Community College to discuss educational opportunities UPDATED RULES: Effective July 1, 2023, we have new rules posted on the DCDEE website. The changes are within the rules for: staff/child ratios and group sizes .0713, cooperative arrangements .2513, and enhanced space requirements .2809. Please refer to the website for more information. NEW CHILD CARE SANITATION RULES: The NC Children’s Environmental Health has re-adopted child care sanitation rules effective July 1, 2023.
