The facility has an 18-month compliance history of 94%. The last annual compliance visit was documented on 7/18/24. The facility’s corporation status was verified current on the NC Department of the Secretary of State today, 1/19/24. Upon arrival, I was greeted by Denise Wright, Owner/Administrator. Therapist, Mercedes Cox, from an outside company was present today. Ms. Wright greeted me and invited me into the facility. There were 11 children present today. Children were observed playing in various centers of their choice, they also danced to music and engaged in group time. Emergency medications, topical creams, and permissions were observed, Epi-pens were in original packaging and had current permission to admission form and medical action plans. Outdoor spaces were monitored and there were no hazards observed today. The fence on the playground had been damaged by a delivery truck. You are providing gross motor activity on the inside until you can get the company to fix your fence. One randomly selected children’s file and staff files were monitored during today’s visit. I monitored health and safety training and other annual requirements. Trainings were not complete, they are due by December 2025. A satisfactory fire inspection was completed on 2/3/25 and the facility was approved for day and night time care only. Emergency drills were documented monthly. A sanitation inspection was completed on 12/5/24 and the facility received a Superior rating. Outdoor inspections were documented monthly. Required items were posted and current. There was one item of non-compliance observed today. The corresponding violation was cited. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. There were 11 children ages 2-4 years old present with the owner Ms. Wright. GS 110-91(7);.0713(a-d) To comply with childcare rules, violations cited today MUST be corrected immediately. A compliance letter must be received on or before March 12, 2025, the letter must address each violation, explain how it has been corrected and how you will maintain compliance in the future. Please make sure to include: Facility name, facility ID number, and each item number. The letter can be emailed to cherissa.alston@dhhs.nc.gov. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. I provided technical assistance today regarding staff/child ratios: --Staff/child ratios - low child:staff ratios are most critical for infants and young toddlers (birth to twenty-four months) (1). Infant development and caregiving quality improves when group size and child:staff ratios are smaller (2). Improved verbal interactions are correlated with lower ratios (3). For three- and four-year-old children, the size of the group is even more important than ratios. The recommended group size and child:staff ratio allow three- to five-year-old children to have continuing adult support and guidance while encouraging independent, self-initiated play and other activities (4). In addition, the children’s physical safety and sanitation routines require a staff that is not fragmented by excessive demands. Child:staff ratios in child care settings should be sufficiently low to keep staff stress below levels that might result in anger with children. Caring for too many young children, in particular, increases the possibility of stress to the caregiver/teacher, and may result in loss of the caregiver’s/teacher’s self-control (11). I also provided the following guidance and reminders: --On going training - I suggested completing training on a calendar year, January - December to better keep up with due date. --Health and Safety training - Training is due for all staff except Ms. Wright by Dec 2025. Please include the Maltreatment course. --Staff development plans and Annual Evaluations - Please document dates of annual evaluations with a document signed by the staff member. Update development plans each year with new goals. --Permission to administer forms - due for renewal by the end of February 2025. --Medical Action Plans - are due by the end of February 2025. The facility has an 18-month compliance history of 97%. The last annual compliance visit was documented on 7/26/23. The facility’s corporation status was verified current-active on the NC Department of the Secretary of State today. Mercedes Cox, a therapist, arrived during the visit today. She provided services in the space with the child. Upon arrival, I was greeted by Denise Wright, Administrator/Owner. Ms. Wright greeted me and invited me into the facility. There were eight (8) children present today. Children were observed playing with a variety of toys of their choice. They were also observed listening to a story, dancing to music and singing songs with the teacher. They had gross motor play on the inside of the facility today. They were observed riding tricycles and pushing carts and collecting items to put in the cart today. Teachers were observed interacting with children a positive manner. Emergency medications were observed, an Epi-pen and albuterol were not in original packaging and did not have current permission to admission forms. There were current medical action plans on file for both. The outdoor spaces was monitored and there were no hazards observed today. You stated you recently had a black snake in the outdoor place space so you were not playing in the space today, the grass was recently cut and you will use the space next week. One child’s file and all staff files were monitored during today’s visit. I monitored for completion of health and safety trainings and other annual requirements. A satisfactory fire inspection was completed on 2/15/24 and the facility was approved for daytime and night care. Fire and emergency drills were documented monthly. A sanitation inspection was completed on 5/6/24 and the facility received a Superior rating. Outdoor inspections were documented monthly. You stated you do not provide transportation for the children. You stated there are no children enrolled on 2nd shift, the last child leaves at 5 pm. Required items were posted and current. There were two items of non-compliance observed today. Violation Number Comment Rule 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. An Epi pen and albuterol for Aaron S. were not in the original packaging. 10A NCAC 09 .0803(2)(b)(i-v) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. There was no medication authorization for and Epi-pen and Albuterol for Aaron S. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 8/1/24, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. During today's visit, we discussed using the on-going training log and health and safety log, as a way to keep training organized by year. I monitored staff files and discussed the following training dates; Health and Safety is due by June 2025 for D. Wright and V. Roberts. V. Roberts also needs to complete maltreatment by 9/12/24. We discussed the professional development plans and various goals staff members could be working towards. CPR/FA, and other on-going trainings are already required, staff should work on other goals to enhance their career goals such as; obtaining Credentials or degrees, Foundations trainings and using the Foundations book to enhance lesson plans. etc. We discussed Moodle and SWDCDC as a way to complete on-going trainings. I provided consultation regarding prescriptions medications - all prescription medicines are required to be in the original packaging with the prescription label. You also will need to have a permission to administer form with each medication, I showed you what this form looks like on the website. Thank you for your time today. A checklist was used to note the requirements I monitored today. This facility currently operates with Three Star license issued on October 30,2019. Upon my arrival to the facility, I was greeted by Ms. D. Wright, the administrator. Ms. Wright assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Eight (8) children were present during today’s visit. I observed the children playing on the floor with available toys, playing in centers, washing their hands in preparation for lunch and eating lunch. A sanitation inspection was conducted on April 27,2023. This facility received a superior rating with twelve (12) demerits. A fire inspection was conducted on February 24, 2023. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as of July 25,2023. One (1) violation was cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. 10A NCAC 09 .0601(a) The above violation must be corrected immediately, and a letter must be sent to me no later than August 8 ,2023 stating how the one item was corrected. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Wright that the staff members need in-service training hours. It-SIDS, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. You need to email the training certificates once staff has completed the training hours. Rated License Reassessment: Education Standards: The administrator meets seven points. The lead teachers meet seven points. The facility earns seven points in education. Program Standards: The operational policies were approved during the original assessment. I discussed with you about having the Environmental Rating Scales done. At this time, you declined having Environmental Rating Scale done. The facility meets for two points in program standards. Compliance History: An updated compliance history was printed on July 26,2023, which showed a current score of % An updated compliance history will be printed to reflect the violation cited during this visit. Quality Point: This facility has chosen staff benefits package and parent involvement. Based on the center having earned seven (7) points in staff education, two (2) points in program standards and one (1) point in Quality. Your Child Care Center has earned a Four Star Rated License. I will request that a Four Star Rated License be issued and mailed to your Child Care Facility. Please keep in mind that the administration of the Division of Child Development and Early Education must approve this assessment and the points earned in each component prior to issuance of the license.
