Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Additionally, I observed the indoor and outdoor space used by the children. I observed the teacher’s interaction with children and, they were positive. I observed the children eating snacks, transitional activities, diaper routines and arriving from school. Based on information provided, on 5/6/2025 a seventeen-month-old child was left asleep in the cozy corner of the classroom. The child was unattended by staff for seven minutes. One staff member remained in the classroom to sweep while the other staff member took seven children outdoors. Ms. Kabler stated the staff member did not notice the child in the cozy area while sweeping the floor and left the child in that area. I was unable to interview the two staff members that were assigned to the classroom but spoke with Ms. Kabler about the allegations. Proper supervision of children was observed today in each space. Ms. Kabler explained the incident did occur and we discussed things to put into place that would assist in the supervision and staff/child ratios monitoring process. Ms. Kabler stated and agreed to the use of walkies in each classroom daily moving forward. She stated the walkies would be distributed each morning and collected each afternoon prior to closing. Ms. Kabler also stated her staff will be required to take supervision training through ProSolutions by May 22, 2025. I also reviewed the Supervision Policy. Ms. Kabler stated the policy was in place and specified as to how the staff were to use transition sheets during their inside and outside times. Based on information received from staff interviewed, documentation received, and my observations, the finding regarding the allegation of inadequate supervision and staff/child ratios were substantiated. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Seven toddlers were with one staff member on the playground area while another staff member was inside sweeping the floor. GS 110-91(7);.0713(a-d) 303 Children were not adequately supervised at all times. A seventeen-month-old child was sleeping and was left alone for seven minutes in the classroom. .1801(a)(1-5) You were provided with an opportunity to ask questions regarding the implementation of Child Care Rules. Name of Operation: THE SUNSHINE HOUSE Facility ID: 41000203 Consultant: ELIZABETH CHILDRESS Operation Type: Center Case Number: Visit Date: 3/13/2025 Number Present: 41 Completed Date: 3/13/2025 Age: From 0 To 5 Total Minutes: 120 Time In: 10:00 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with child care rules during a routine unannounced visit. Upon arrival, I was greeted by LaShanta Smith, Assistant Director. Ms. Smith accompanied me on the walk-through of the facility. There were forty-one children present today. Children were observed engaged in outside activities and eating lunch. Infants were being held and sleeping. Children were being attended to according to their individual needs. The staff were observed engaged in activities with the children and were positive in their interactions with the children. Four new staff members’ files were monitored. Required program records were also monitored during today’s visit. A satisfactory fire inspection was completed on 8/30/2024. Emergency drills were documented monthly. A sanitation inspection was completed on 11/21/24 and the facility received an approved rating. Required items were posted and current. One violation was documented during todays visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. One child was given medication without an action plan and necessary authorization was not available. 10A NCAC 09 .0803(1)(a & b) A signed and dated letter of compliance must be emailed to me by 03/24/25, describing how and when the violation was corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. You were provided with an opportunity to ask questions regarding the implementation of Child Care Rules.
