A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star license issued on October 2,2021. Jen Jones, a child care consultant, accompanied me during today’s visit. Upon our arrival to the facility, we were greeted by Ms. K Johnson, the administrator. We introduced ourselves and informed Ms. Johnson the purpose of today’s visit Ms. Johnson. assisted us during this visit today. Ms. Johnson accompanied us while we observed and documented the attendance. All indoor and outdoor spaces used by the children were observed during this visit. All required items were posted. All medication and hazardous items were out of the children's reach. One hundred and one (101) children were present during today’s visit. I observed the children playing on the floor with available toys, playing in centers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch and napping. A sanitation inspection was conducted on January 16,2025. This facility received a superior rating with two (2) demerits. A fire inspection was conducted (an not approved) on April 23,2024. I informed Ms. Johnson this will be a violation. I contacted Mr. Timothy Brown, the fire inspector, to find out what is the status of this facility’s fire inspection. Mr. Brown informed me that this facility is in the process of getting the approved inspections for the work that was done. I explained to Ms. Johnson that I will have to discuss this situation with my supervisor to determine an acceptable timeframe this violation will need to be corrected. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as August 14,2025. Four (4) violations were cited during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Operator has not obtained a fire inspection within 12 months of the previous inspection. A fire inspection was conducted and was not approved on April 23,2024. 10A NCAC 09 .0304(a) 219 Outdoor area was not designed so staff are able to adequately supervise the entire area. The preschool playground had several tall plants that obstructs the view for staff to adequately supervise the children. 10A NCAC 09 .1402(e) 445 For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. The infant and toddlers classrooms need to add fine motor, sensory, pretend play and blocks materials to their classrooms. .0510 (e ) (1)(A-G) 488 For children under three years of age, materials were not offered in sufficient quantity to allow all children to use them at some time during the day and to allow for a range of choices. There were not enough of blocks, dolls and fine motor materials in the infants, toddlers and two year old classrooms. .0510(e)(3) The above violations must be corrected immediately, and a letter must be emailed to me no later than August 28,2025 stating how each item was corrected. If I do not receive your letter by August 28,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Johnson that the staff members need in-service training hours, It-SIDS, health and safety training, CPR and First Aid trainings by specific dates. Please refer to the previous staff/training worksheet. Rated License Assessment: I discussed it with Ms. Johnson, the Rated license assessment hold harmless has ended. Effective July 1,2025 the new QRIS has been approved. We will be holding director's meeting in September to explain the new timeline for rated license reassessment to take place. I explained to Ms. Johnson that there is a new process that is still being developed. You can find information about the three pathways at https://ncchildcare.ncdhhs.gov under the what’s new tab. Beginning on February 1, 2025, all child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. You can visit ncrlap.org for any upcoming training courses for ITERS-3 and ECERS-3. Thank you for your time. Name of Operation: THE NEST SCHOOL Facility ID: 41002385 Consultant: SHERRY PROCTOR Operation Type: Center Case Number: 0524-346L Visit Date: 5/28/2024 Number Present: 104 Completed Date: 5/28/2024 Age: From 0 To 5 Total Minutes: 70 Time In: 11:20 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance of child care rules after receiving a report with concerns that a staff member did not notify the DCDEE of any pending charges, indictments, or convictions within one business day of being notified by the child care provider or household member of their incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division. This has been substantiated and the staff member has been issued a disqualifying CBC letter. Upon arrival, I was greeted by Ms. Kim Johnson, assistant director, she and I discussed the violation. Ms. Johnson walked with me through the center as I gathered attendance. I monitored supervision, staff/child ratios, and staff interactions with children. There were one hundred and four children present today ranging in age from 0- 5 years old. All classrooms I observed had adequate supervision, classrooms also were in correct ratios for the children’s ages and group size. Children were participating in classrooms activities, lunch and transitioning to rest time. A violation was documented due to the operator did not notify the Division of any pending charges, indictments, or convictions within one business day of their incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division. At this time a correction letter is not due. The violation has been corrected. Ms. Johnson is aware the staff member may not be on the children care property. Violation Number Comment Rule 1802 The child care provider and/or household member who incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division, did not notify the operator of these charges within five business days or before returning to work. The operator did not notify the Division of these pending charges, indictments, or convictions within one business day of being notified by the child care provider or household member. A staff member was arrested in December 2023 and did not report the arrest to DCDEE within one business day. When the complaint was called in the staff member was issued a Disqualifying CBC letter. G.S. 110-90.2 & .2703(m) Correction: The staff with the arrest and pending charges was immediately suspended. The center and their corporate office are aware the staff may not return to the center in any capacity due to the Disqualifying CBC letter.
