This is a center located in a residence and earned a Three-Star License on January 24, 2023. I was greeted by Ms. Sravon Wallace, the owner of the center. Ms. Wallace prepared a work site for me along with requested files. Currently you do not provide any transportation. As of January 22, 2025, the compliance history for this center is 100%. Emergency Drill/Fire drills and Playground inspections were current and accessible. The Annual Compliance Monitoring Checklist and the Item Number Listing (DCD 0375) were used as basic tools to assess compliance with applicable child care requirements pertinent to this center. At this time eleven children are enrolled, and they were all attending. There are plenty of activities for children. The outdoor play area is fenced in and there are many items for active play. The infant and toddlers were playing with toys on the floor, one infant was being held while taking a bottle. The preschool children were playing in centers, all centers were open and assessable to the children. All the interactions I observed were warm and caring. The children’s files were reviewed today, along with Ms. Wallace and Ms. Abonza files. Program files were also reviewed. To verify compliance with specific requirements, the provider’s information was recorded onto the Verification Form for CLIR and FCCH, Ms. Wallace completed the FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form. I received the signed forms during today’s visit. Your signature on these forms will serve as verification that the information provided is accurate and complete. I observed one item of noncompliance during the visit today. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member's qualification letter has expired. G.S. 110-90.2(b) & .2703(n)&(o) Violations are to be corrected within two weeks, February 7, 2025 is two weeks. The violation cited today is for an expired CBC Qualification letter. She has an appointment to get her fingerprints completed on February 10, 2025. Please contact me by email when your fingerprints are completed. All special training is up to date, as is the health and safety training. MOODLE on the DCDEE website offers free, online training that provides training hours. Remember Child Maltreatment is also to be completed every five years as is the health and safety trainings. The “Hold Harmless” status has been extended until the new QRIS is implemented. At this time, you do not need to complete your Rated License Assessment.
