A checklist was used to note the requirements I monitored today. The center currently operates under a Four-Star License, effective December 12, 2019. The last annual compliance visit was conducted on June 25, 2026. Upon arrival, I was greeted by Ms. Rita Abraham, Owner/Director. Ms. Abraham accompanied me throughout the walkthrough of the facility while I observed the children and reviewed attendance records. I conducted a full assessment of the child care requirements and completed a walk-through of the indoor and outdoor spaces used by the children. Eleven children were in attendance during today’s visit. I observed the children participating in free choice and teacher directed activities and completing bathroom routines. In the infant room, two infants were asleep in cribs; three children were interacting with the teacher. The sanitation inspection was completed 4/1//26 with a “Superior” classification. A fire inspection was conducted on 5/12/25. Playground, fire drills, and emergency drills have been conducted, and documentation was available for review. I reviewed a sample of children's and staff's files. One item of noncompliance with child care requirements was cited during today’s visit. The violation was corrected during the visit. Violation Number Comment Rule 613 Crib sheets were not changed as needed or at least daily. One crib sheet was found to be soiled. The provider addressed this during the visit by replacing it with a clean crib sheet. 15A NCAC 18A .2821(f) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Pathways to the Stars We reviewed the updated rules in Section .3200 regarding the Standards for Two- through Five-Star Rated facilities (Pathway to Stars), and I demonstrated how to access the relevant information on the DCDEE website. A new rule rollout module is now available on the DCDEE e-learning Moodle platform. As discussed, the facility should carefully consider the available QRIS options and determine which pathway it plans to pursue for its rated license reassessment. While the reassessment process may begin at any time, the facility should be prepared to submit the Application for Assessment for a Rated License no later than between April 1, 2026, and September 30, 2026. You chose Pathway 2: Classroom and Instructional Quality, please complete the following steps: • Submit the “Application for Assessment for a Rated License for Centers” electronically to your child care consultant no later than July 31, 2026. • Confirm that all staff members have active WORKS accounts. • Complete the QRIS Staff Information and Education Worksheet electronically. • Identify Continuous Quality Improvement (CQI) goals and begin planning for implementation. • Develop strategies to strengthen Family and Community Engagement. • Select and implement a curriculum. (Four- and Five-Star centers must use an approved curriculum and a formative assessment tool for all age groups.) • Arrange coaching and/or training opportunities for administrators and lead teachers. The facility has an 18-month compliance history of 97%. The last annual compliance visit was documented on 7/16/25. Upon arrival, I was greeted by Mrs. Rita Abraham, Administrator. Mrs. Abraham accompanied us on a walkthrough of the indoor and outdoor spaces. I noted attendance and monitored staff/child ratios. Mrs. Abraham assisted us with the required documentation and answered any questions we had. There were 13 children present today, ages 0-8 years old. Infants were observed playing with toys and receiving routine care. Other children were observed playing outside and eating meals. Interactions observed were positive and nurturing in manner. You stated there were no emergency medications or topical creams in the facility. There were current activity plans available for review today. A current menu was also posted. You stated there were no children with allergies or asthma enrolled. Four randomly selected children’s file and all staff files were monitored during today’s visit. I monitored health and safety training and other annual requirements. Training must be completed by 7/31/25 for the 24-25 year. A satisfactory fire inspection was completed on 5/12/25 and the facility was approved for day and night time car. A sanitation inspection was completed on 1/3/25 and the facility received a Superior rating. Outdoor inspections, fire and emergency drills were documented monthly. Required items were posted and current. There was one item of non-compliance observed today. The corresponding violation was cited. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. Portions of the chain link fencing was less than 4 feet high. There is also a portion of the fence near the climbing structure that is protruding and pulling away from the fence. GS 110-91(6); .0605((i) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 7/9/25, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. The letter can be emailed to cherissa.alston@dhhs.nc.gov. Fencing I provided technical assistance regarding fencing requirements. Portions of your fence is below four feet, this should be corrected immediately. You are going to have some of the mulch and dirt removed from around the bottom of the fence. You are also going to call to have a fencing company come out to fix the low places and the panel that was starting to protrude. ABCMS Provider Portal The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Additional information and instructions are available in the ABCMS Provider Portal Guide. Once the training has been completed and you have been notified that access has been granted, log into the provider portal to locate the unique provider code for the facility. Applicants and employees will use this code to create a connecting application to be associated with your facility. You must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Licensing Supervisor Meg, Riddle accompanied me on the visit today. Upon arrival, I was greeted by Rita Abraham, Administrator. I stated the purpose of my visit, and she invited me inside the facility. There were 14 children present, ranging in age from 0- 5 years old. In space #2 there were ten children; 1-2 year old, 2- 3 year olds, 4 -four year olds, 1 – five year old and 2- 6 month olds. Ms. Ebony was present feeding one of the infants. Staff: child ratios were being met during the visit, however, children under one year of age were not separate from children two years and older. Mrs. Abraham stated they were preparing for group time. I explained that children under two years old can not be combined with children two and up except in the first and last hour of operation. She immediately moved the infants to Space #3 with Ms. Barbara where there was 2 one year old children and 1 – 2 year old. The two year old was picked up by his parent during the visit. We then observed children 2-5 years old playing with learning cards at the table. They were also observed eating lunch. Infants and toddlers were in bouncers, cribs preparing for nap and walking around the space. There was one item of non-compliance observed today. The corresponding violation was cited today’s visit, the violation was not related to the allegations in the report. Violation Number Comment Rule 316 Children under one year of age were not kept separate from children two years and older. There were two 6 month old children in space #2 with children two years old and older. There was also a 2 year old child in Space 3 with the 6 month old children. 10A NCAC 09 .0713(a)(5) Based on observations, documents reviewed, and information obtained during today’s visit, we were unable to determine that lapses in following required minimum staff-child ratios and inadequate supervision had occurred. However, upon my arrival, children under one year of age were not separate from children two years and older. This is a violation of child care rules. We were not able to determine that children were being cared for in unlicensed spaces. We were also unable to determine that there were sanitation and health violations, there were baskets in the licensed spaces for dirty toys, and there were toys in the basket. Mrs. Abraham explained how toys are sanitized during the day. Therefore, the allegations in the report were unsubstantiated today. Thank you for your time today.
