The last annual visit was conducted on 5/14/25. Upon arrival, I was greeted by Cornelia Bailey, Administrator. I stated the purpose of my visit and I reviewed new staff files and monitored required paperwork while I waited for children to arrive. Children began arriving at 2:40 pm and they were observed putting their belongings away and using the restroom. They entered their licensed spaces and were observed completing homework, completing art activities, reviewing shapes, acting out scenarios regarding good and bad manners and eating snack. I reviewed playground inspections, and fire and emergency drill documentation, they were found to be current. I reviewed files of five new employees. There were current qualification letters, CPR/FA, BSAC, HQ, medical reports and TB skin test on file for the five employees. I also monitored the approved outdoor space. There were one item of non-compliance observed and the corresponding violation was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was a large grill stored on the playground. Although the propane was removed from the grill protruding objects, including where the grill hitches to be pulled, and sharp area along the edge of the grill, pose a hazard to the children while they are on the playground. 10A NCAC 09 .0601(a) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 12/29/25, describing how and when the violation(s) were corrected. Please remember your letter should include facility name, facility ID#, and original signature. The letter can be emailed to cherissa.alston@dhhs.nc.gov. Today, we discussed the QRIS modernization and the three pathways. You are unsure of when you will be ready to request either Pathway. Cornelia Bailey, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Bailey. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 107 The center did not comply with the permit restrictions. The center failed to comply with the center's permit for age requirements for children 5-12 years, and enrolled two (2) four-year-old children. GS 110-91; GS 110-106 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member failed to renew their criminal background check prior to the expiration date. G.S. 110-90.2(b) & .2703(n)&(o) 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. A staff member failed to obtain written permission prior to transporting enrolled children that included when and where children were being transported and their expected time of departure and arrival. .1003(i)(j) 1128 For routine transport of children to and from the center, staff did not use the list to document attendance as children boarded and departed the vehicle. A staff member failed to document the attendance of enrolled children as they boarded and departed the facility’s vehicle on 9/5/24 and 9/6/24. 10A NCAC 09 .1003(l) Violations must be corrected immediately.
