Name of Operation: SHINING LIGHT ACADEMY Facility ID: 41000420 Consultant: WYTINA BLACKMON Operation Type: Center Case Number: Visit Date: 1/27/2025 Number Present: 15 Completed Date: 1/27/2025 Age: From 3 To 5 Total Minutes: 220 Time In: 09:30 AM Time Out: 01:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for compliance during an annual compliance visit. A checklist was used to note the requirements I monitored today. Upon my arrival to the facility, I was greeted by Mr. D. Robinson, the administrator. Mr. Robinson assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Fifteen (15) children were present during today’s visit. I observed the children coloring, playing with blocks, reviewing letters, counting and identifying numbers, listening to a story, using the bathroom, washing their hands in preparation for lunch and eating lunch. A sanitation inspection was conducted on October 31,2024. This facility received a superior rating with four(4) demerits. A fire inspection was conducted on September 6, 2024. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as of January 27,2025. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 476 Staff did not use the list of participating children to check attendance when leaving the center, periodically when the children were involved in the activity, before leaving the activity to return to the center, and/or upon return to the center. The teachers did not use the list of participating children to check attendance periodically when the children were involved in the activity nor before leaving the activity to return to the center. .1005(b)(6) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There was burn cream, antiseptic wipes and Hydrocortisone cream in an unlocked first aid kit on the bus. 15A NCAC 18A .2820(d) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The children's photograph was not in the vehicle for each child being transported. 10A NCAC 09 .1003(d) The above violations must be corrected immediately, and a letter must be emailed to me with your signature no later than February 11,2025 stating how each item was corrected. My email address is at wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by February 11,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Mr. Robinson that the staff members need criminal rechecks, CPR and First Aid trainings by specific dates. Please refer to the staff/training worksheet. Technical Assistance: Transportation: Today I also discussed with Mr. Robinson the transportation requirements you must met when you transport children on field trips and off premises activities. Today I monitor a thirty-three-passenger bus. Your insurance policy was current, a first aid kit and a mounted fire extinguisher was observed on the bus during today’s visit. Today I observed burn cream, antiseptic wipes and hydrocortisone cream in an unlock first aid kit. Mr. Robinson informed me these items belong to the Church. I explained Mr. Robinson that since the bus is share that he would need to make sure that the items that belong to the church is inaccessible to the children when they are being transported on the bus. The following requirements must be met and will be monitored: The driver must be: (1) 21 years old (2) have a valid driver's license of the type required under North Carolina Motor Vehicle Law for the vehicle being driven or comparable license from the state in which the driver resides; and (3) have no convictions of Driving While Impaired (DWI) or any other impaired driving offense within the previous three years. (4) have emergency information in the vehicles (5) have current insurance policy for the vehicles These requirements must be met by all drivers, and it is up to you to verify all these requirements have been met. SAFETY PROCEDURES (10A NCAC 09 .1003) (1) A first aid kit must be located in each vehicle used on a regular basis to transport children. The first aid kit shall be firmly mounted or otherwise secured if kept in the passenger compartment. (2) For each child being transported emergency and identifying information must be in the vehicle. (3) Children shall not be left in a vehicle unattended by an adult. (4) Before children are transported, written permission from a parent shall be obtained which shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. (5) When children are transported, staff in each vehicle shall have a functioning cellular telephone or other functioning two-way voice communication device with them for use in an emergency. SEAT RESTRAINTS (10A NCAC 09.1001) These requirements are required if you transport children in vehicles with seat belts. (1) Each adult and child shall be restrained with an individual seat belt or appropriate child restraint device when the vehicle is in motion. (2) Only one person shall occupy each seat belt or child restraint device. **You must follow all state and local driving laws. --North Carolina state law requires that all children who are not over age 8 or do not weigh at least 80lbs. must be restrained in a booster or car seat appropriate for the child's weight and height. For more information on how to meet NC child restraint requirements please visit www.buckleupnc.org Staff/Ratio: (section.1004): (1) When children are being transported, the same staff-child ratios required for a child care program applies. Ratios for the youngest child in the vehicle must be maintained at all times. Field Trip Permission form: During today's visit, I reviewed your current field permission form. You can no longer use a blanket permission form for all transportation activities. A permission slip can be obtained by the parents daily, weekly or per each activity type. For example, if you are going to the movies every Wednesday, then have the dates the facility will be going to the movies on one field trip permission form. The following requirements must be included on each permission slip: 1. Written permission from each child's parent to transport. 2. Parents notified when and where child is to be transported; and expected time of departure and arrival, and the transportation provider. Additional requirements that must be met by staff during all field trips are: (1). Staff in each vehicle must have a functioning cellular phone or other function communication device with them to use for an emergency. (2) Staff must have a list of the children being transported. Staff members shall use this list to check attendance as children board and depart from the vehicle. (3)You must post a schedule of off premise activities in each participating classrooms, a copy must be given to parents. (4)A list of all children participating in field trips must be left at the center. (5) Travel Log for field trips must be maintained for at least one year (6) There must be a staff member present that has current safety training (CPR/First Aid). Thank you for your time. This facility currently has an eighteen-month compliance history of 99%. Upon arrival, I was greeted by Danny Robinson, Administrator. We completed a walkthrough of the indoor and outdoor licensed spaces. Attendance and staff/child ratios were noted. Children were observed transitioning to the cafeteria and eating lunch. Emergency drills and playground inspections were current and documented. Required documents were posted. A fire inspection was completed on 3/9/23 and a sanitation inspection was completed on 8/31/23 with a Superior rating. There were three items of non-compliance observed during today’s visit and the corresponding violations were cited. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. There was a aerosol can of Aquaphor in the locked cabinet for the child A.H. that was was not returned to the parent once the permission to administer form was not received. .0803(12) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. There was not a medical action plan for children L.A., A.H. There also needs to be a permission to administer the medication form on file. .0801(b) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. An Epi pen for child A.H. was not in the original box. 10A NCAC 09 .0803(2)(b)(i-v) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 2/22/24, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. I provided technical assistance on where to find the medical action plan and permission to administer forms. I also stated that the medical action plan should be attached to the application on the day the child is enrolled. This facility currently has an eighteen-month compliance history of 100%. Upon arrival, I was greeted by Danny Robinson, Administrator. We completed a walkthrough of the indoor and outdoor licensed spaces. Attendance and staff/child ratios were noted. Children were observed having group time sitting at tables. They recited the alphabet and listened to a story read by the teacher. Emergency drills and playground inspections were current and documented. Required documents were posted. There was one item of non-compliance observed during today’s visit. Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. A. Eagle did not have a qualification letter on file for review. G.S. 110-90.2(b) & (d) & .2703(e) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 10/24/23, describing how and when the violation(s) were corrected. During today’s visit, I provided technical assistance regarding the following: -Qualification letters - Staff working with preschool aged children must have a current qualification letter on file. - Enrollment Application - Your current application is missing the section on health care needs. This needs to be added or you must use the state application. If a child has a health care need that requires emergency medication, the proper medical action plan must be attached to the application. -Emergency medications and trainings with your CHHC - I advised you to contact your health care nurse to get your new teachers proper training on how to administer emergency medications.
