A checklist was used to note the requirements I monitored today. This center currently operates with a Five Star- Rated License effective September 26, 2022. Upon arriving, I was greeted by Ms. Stephanie Ramirez the Assistant Director. Ms. Ramirez accompanied me during my walkthrough. Ms. Traci Jones, Director arrived and assisted me with the staff training worksheets and files. A full assessment of child care requirements was conducted during today’s visit. There were 115 children present during the visit. During today’s visit, I observed the children participating in free choice activities, teacher directed activities, completing bathroom routines, eating lunch, and transitioning to rest time. In the infant room, I observed one child in a crib, and four children playing on the floor. Staff and child ratios were observed in compliance during today’s visit. Positive staff and child interactions were also observed. A sample of children’s files and staff files were monitored during today’s visit. A sanitation inspection was completed 7/12/24 with a “Superior” classification. A satisfactory fire inspection was conducted on August 26, 2024. Monthly playground and fire drills have been conducted and documentation was available for review. Quarterly disaster drills have also been documented. The facility has an 18-month compliance history of 96%. The facility’s corporation status was verified as active not current on the NC Department of the Secretary of State today, 2/21/25. Two violations of child care requirements were cited during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Leftover diaper cream was not returned to the parent after authorization had expired. This violation was corrected during the visit by the assistant director removing the diaper cream from the classroom to be given back to the parent at the end of the day. .0803(12) 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility corporation status was not current on the NC Secretary of State Website. GS 110-91 Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before March 7, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If I do not receive your letter by March 7, 2025 a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. We discussed the following: Child care administrators can access ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Please complete the process for your facility as soon as possible. Extension of Hold Harmless provisions - star rated license assessments and reassessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments continue to be required for new child care programs. Updated Program Assessment Tool – Beginning on February 1, 2025, child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. This updated edition includes language and interaction measures, a need consistently identified by early childhood professionals during listening sessions. Currently this center operates with a Five Star License, issued September earning 7 points in the Education Component, 7 points in the Program Component (meeting enhanced space and ratios) and 1 Quality point for all lead teachers having an AAS or higher in early childhood education or child development. Limited monitoring of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. I reviewed three staff files. The NC Secretary of State website was reviewed August 13, 2024 and Early Foundations GSO New Irving Park, LLC was listed as current- active. Upon arrival, I was greeted by Traci Jones, Administrator. I stated the purpose of the visit and completed a walkthrough of the facility. Ms. Jones assisted me with required documents. There were 125 children present during today’s visit. Children were observed participating in free choice and teacher directed activities. Infants were observed napping in appropriate sleep positions, in bouncers with a teacher nearby, crawling on soft toys. They were also observed receiving routine care from their providers. Teachers were observed interacting in a positive and nurturing manner. One violation of child care requirement was cited today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on 6/13/23. 10A NCAC 09 .0304(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. On or before August 27, 2024 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violation was corrected.
