This program currently operates with Three Star License issued 2/3/22 earning four points in Education, two points in Program Standards and One point in Quality for enhanced policies and parent involvement. Ms. Margaret Kelly, owner was present and assisted me during this visit. A partial assessment of applicable child care requirements was conducted during today’s visit. I completed a walk-through of the indoor and outdoor areas used by the children. There were 15 children present during today's visit. In Classroom 1, I observed the children interacting with the caregiver in a positive manner. In Classroom 2, I observed the teacher reading a story to the group. In Classroom 3, I observed the children participating in free choice activities. A sanitation inspection was completed on December 11, 2023 resulting in a "Superior" classification with 8 Demerits. During today's visit, one violation of child care requirements were cited. The violation was corrected during the visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in the three classrooms. Ms. Kelly printed and posted current activity plans during the visit. GS 110-91(12); .0508(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. Automated Background Check Management System As of February 1, 2024, child care administrators can access ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Please complete the process for your facility as soon as possible. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). A checklist was used to note the licensing requirements I monitored today. I also monitored health and safety requirements. This program currently operates with Three Star License issued 2/3/22 earning four points in Education, two points points in Program Standards and One point in Quality for enhanced policies and parent involvement. Ms. Margaret Kelly, operator was present and assisted me during this visit. A full assessment of child care requirements was completed during today’s visit. There were thirteen children present during today's visit. I observed the children completing bathroom routines, participating in teacher directed and free choice activities. The staff/child ratios were the following: Infants and 1’s – 2:5 (M. Kelly and A. Foster) Mixed Age (2yr -4yr) – 1:8 (C. Webster) The sanitation inspection was completed on December 11, 2023 with a "Superior" classification with 8 Demerits. A satisfactory fire inspection was completed on June 28, 2023. A fire drill was conducted on 2/16/24. A shelter in place drill was completed 2/7/24. A playground inspection was completed on 2/15/24. I reviewed three children files and all staff files. I received the staff and training worksheets today and updates were made. Today I checked the NC Department of the Secretary for the facility's corporation status. The LLC status is active and current. During today's visit, violations of child care requirements were cited. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The attached porch seating was rotted in the section on the right closest to back door. The green carpeting on the porch floor was not secured in placed near the steps leading to the playground. G.S. 110-91(6); .0601(b) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Three staff did not have an annual health questionnaire on file following the initial medical statement. .0701(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. Documentation was not available for review that staff reviewed the EPR Plan annually. .0607(e) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Documentation of the annual review was not maintained on file for four staff. .0607(f) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. A signed and dated letter of compliance must be emailed to me on or before March 11, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3.
