This DPI NC Pre-K classroom currently operates with a Four-Star license, effective 02/27/2018. Upon arrival at the elementary school, I was greeted by the office staff. Ms. Gibbs and Ms. E. Abdelrahman assisted during today’s visit. Fifteen children were present. I observed the children during center activities, transitioning to the outdoor environment and eating lunch. The teachers interacted with the children in a pleasant manner. The public-school off-site records verification forms for staff records were completed and signed by the principal. A partial assessment of applicable child care requirements was conducted during today’s visit. The required items were posted and available for review. Required inspections were not current. A shelter-in-place was not documented at least every three months. The last fire inspection was completed 5/28/2024. Three violations were observed and documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed 5/28/2024 10A NCAC 09 .0304(a) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. Two pocketbooks and one container of lotion were accessible to children. 10A NCAC 09 .0601(a) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A letter stating how the violations were corrected and how you plan to maintain compliance in the future must be submitted to me by 12/15/2025. -A shelter-in-place was not documented at least every three months. Child care centers must conduct a shelter-in-place or lockdown drill, as defined in 10A NCAC 09.0102, at least every three months, Child Care Rule.0302(d)(8). They must keep a record that includes the date of each drill, time of day, the length of time to reach the designated location and the signature of the person conducting the drill, Child Care Rule .0604(u). I suggested to document the dates you plan to conduct the emergency drills and have someone available for backup in case the person that usually conducts the emergency drills is not present. -The last fire inspection was completed 5/28/2024. -Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were improperly stored in an unlocked cabinet in classrooms. It is extremely important that staff take the required time to lock doors, store hazardous items properly either in locked storage or five feet up from the finished floor. Personal belonging such as pocketbooks and lotion were accessible to children. Staff moved the items to a locked closet. We appreciate all you are doing to serve the children and families of NC. This DPI NC Pre-K classroom currently operates with a Four-Star License effective February 27, 2018. Upon arrival at the elementary school, I was greeted by the office staff. When I entered the classrooms, I was greeted by B. Gibbs and F. Jado. Both of you assisted me with the information required for the visit. The most recent sanitation inspection was conducted on September 3, 2024 and received a classification of Superior with eleven demerits. The last fire inspection was conducted on May 28,2024. I walked through the indoor and outdoor areas of the facility that the children use during the visit. Fifteen children were present today. I observed the children during outdoor activities, eating lunch and completing bathroom and hand washing routines. The teachers interacted with the children in a pleasant manner. The Lead teacher and teacher assistant are listed on the North Carolina Pre-kindergarten System. The public-school-off-site records verification forms for staff records and children’s records were completed and were signed by the principal. A partial assessment of applicable child care requirements was conducted during today’s visit. One violation was observed and documented. Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. One staff member did not have a qualification letter available for review. G.S. 110-90.2(b) & (d) & .2703(e) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A letter stating how the violations were corrected and how you plan to maintain compliance in the future must be submitted to me by December 19, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. 5. Signature of the person preparing the letter. Please submit the signed letter electronically to my email address. This North Carolina Pre-Kindergarten program currently operated by Guilford County School System earned a Four-Star License issued 2/27/2018. The program earned seven points in Program Standards, three points in Education and 1 Quality point (75% of lead teachers and teachers have at least 10 years EC work experience). The last Sanitation Inspection was conducted on 9/18/23 and the facility earned a “superior” classification with six demerits. The last Fire Inspection occurred on 8/31/2022 and the program was approved for daytime care only. The last Annual Compliance Full visit was conducted on 11/1/2022. J. Fossett, Lead Teacher and J. Chavis, Teacher and fourteen children were present during this visit. I observed children and staff members in Space#1 during a Snack meal and later while the children and the lead teacher were engaged in a “Leaf” Art Activity. The last Fire Drill was conducted 9/14/2023 and the drill lasted three minutes and five seconds. The lead teacher stated a” pull station” was used to activate the drill. The last Lockdown Drill was conducted on 10/23/23 and the Drill lasted one minute. I reviewed copies of the Staff and Worksheets, Criminal Background Qualification Letters, CPR and First Aid Certification Letters for staff members, Playground Inspections, Sanitation Inspection, Annual Fire Inspections, Site Monitoring Tool, Activity Plans, Attendance for Children enrolled in both Spaces, Daily Schedule, First Aid Chart, Fire and Lockdown Drills, In Service Training hours, and Menus. I assessed compliance with staff/child ratios, group size, use of space, space capacity, license restrictions, program requirements, supervision and required documents. The following Restrictions are currently listed on your License: 1.1st shift (daytime care) 2.Meets enhanced Ratios minus one 3.Meets Enhanced Space North Carolina Pre-Kindergarten Review: I reviewed the North Carolina Pre-Kindergarten requirements on this visit. The lead teachers are listed on the North Carolina Pre-Kindergarten System. I reviewed four children's files and health assessments which include hearing, dental and vision screening were completed for the children. Brigance Developmental were conducted within 6 months prior to enrollment or within 90 days after enrollment. The program uses the Creative Curriculum and completes Teaching Strategies Gold to document the evidence of children's ongoing progress. These assessments are completed quarterly. A parent/teacher conference is held three times per year. The hours of operation for this program are 7:15 am until 2:05 pm. Newsletters are sent home monthly. The information sent home monthly includes information on units of study, announcements, upcoming events, and activities to complete at home. The Early Childhood Environmental Rating Scale w/2018 and the overall score was 5.73. The program meets all Enhanced Standards and reduced Ratios minus one. The lead teacher earned a Bachelor of Science Degree in Early Childhood Education and Birth-Kindergarten Certification. The teacher earned a bachelor’s degree in business education. Staff members Files were reviewed. One violation of Child Care requirements was observed on this visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A large puddle of water was located in front of the area used by the children during outdoor activities. 10A NCAC 09 .0601(a) The violation documented above must be corrected immediately. A signed and dated Letter of Compliance including the facility's Identification number must be mailed to me by 11/08/2023 , describing when and how this violation was corrected.
