Name of Operation: PILOT ELEMENTARY ACES PROGRAM Facility ID: 41001389 Consultant: ELIZABETH CHILDRESS Operation Type: Center Case Number: Visit Date: 4/16/2026 Number Present: 46 Completed Date: 4/16/2026 Age: From 5 To 12 Total Minutes: 180 Time In: 01:30 PM Time Out: 04:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with child care rules during an annual compliance visit and to review the Environmental Rating Scales results conducted on 4/9/2026. The overall assessments score was 4.01. The last annual compliance visit was conducted on 05/6/2025. An Annual Compliance Monitoring Checklist for Centers checklist was used to note the requirements I monitored today. This program is currently operated by Guilford County School System and has a Three-Star License issued 5/10/2018. The program has an 18-month compliance history of 96%. Upon arrival at the ACES program, I was greeted by the office staff. Mr. T. Flowers, Site Coordinator, assisted me with the visit. Fifty-six children were present during today’s visit. I observed the children during arrival and transition activities. Staff interacted with the children in a pleasant manner. The public-school off-site records verification forms for staff and children records were completed and were signed by the principal. I reviewed six out of sixty-one children’s files. A full assessment of applicable child care requirements was conducted during today’s visit as per the attached checklist. There was one item of non-compliance observed today Violation Number Comment Rule 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Two staff did not have documentation for BSAC training. .1103(a) We discussed the following items of technical assistance today: Basic School-Age Care (BSAC) training -10A NCAC 09 .2510 STAFF QUALIFICATIONS: Staff who are responsible for supervising groups of school-age children must complete training. This includes group leaders and assistant group leaders. Two staff did not have documentation of BSAC training. We discussed the following items today: WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their workforce information to support education requirements. The WORKS portal is available online and requires that the user maintain an individual North Carolina Identity Management (NCID) username and password. Education has not been added to the Workforce Online Reporting & Knowledge System. ERS score: The total average ERS score of 4.01 Family and Community Engagement Standards: The facility met all foundational practices and two additional choices: Two-way communication with families and two-family events annually. The center appears eligible for the Three Star Level in Family and Community Engagement Standards: Facility Continuous Quality Improvement Plan: The facility completed a CQI plan for both the facility and individual staff members. See the attached Facility CQI plan. This center may qualify for a Three Star License. This will not result in a reduction of stars. This program has Three Stars. Guilford County Schools has made the decision to license all their ACES school-age programs at a Three Star Level. This allows consistency across their programs. The Administration of the NC Division of Child Development and Early Education must approve this assessment prior to issuance of the license. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Name of Operation: PILOT ELEMENTARY ACES PROGRAM Facility ID: 41001389 Consultant: ELIZABETH CHILDRESS Operation Type: Center Case Number: Visit Date: 12/4/2025 Number Present: 61 Completed Date: 12/4/2025 Age: From 5 To 12 Total Minutes: 150 Time In: 01:00 PM Time Out: 03:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. Mr. T. Flowers, Program Coordinator, assisted me with the visit. This program is operated by Guilford County Schools. This ACES program currently operates with a Three-Star license effective May 10, 2018. The center's compliance history was reviewed with the operator. The program’s compliance history was 97% percent as of 12/04/2025. The license was posted, and the restrictions were in compliance. Required items were posted and current. A walk-through of the facility was completed today; all indoor areas were monitored. Sixty-one children were present during today’s visit, I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in outdoor activities, eating snacks, transitions and personal care routines. The public-school off-site records verification forms for staff records were completed and signed by the principal. A partial assessment of applicable child care requirements was conducted during today’s visit. The required items were posted and available for review. I reviewed four new staff files with Mr. Flowers during todays visit. One violation was observed and documented during today’s visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. The necessary documentation was not available for review for one child who had an inhaler. 10A NCAC 09 .0803(1)(a & b) The violation documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 12/15/2025, describing how and when the violation was corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. During the visit, I provided technical assistance regarding medication requirements. Mr. Flowers is new to his position as Site Coordinator. We reviewed The Division of Child Development and Early Education website. Mr. Flowers added the website to his bookmarks for future reference. We also reviewed the necessary forms needed for medications under the providers forms and documents tab.
