A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on February 15,2018. Upon my arrival, I was greeted by Dr. A. Hines, one the lead teachers. I informed Ms. Hines the purpose of today’s visit. Dr. Hines informed me there was a new teacher. I introduced myself to the new lead teacher, Ms. S. Walker. Dr. Hines gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Twenty-six (26) children were present during today’s visit. During today’s visit, I observed the children playing in centers, participating in a literacy activity and playing outside on the playground, washing their hands in preparation for lunch, the finishing up lunch, napping, waking up napping and departing. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medications were out of the children's reach. A sanitation inspection was conducted on September 192025. This facility received a superior rating with twelve (12) demerits. A fire inspection was conducted on June 12,2025. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Eight (8) violations were cited during today’s visit. Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. .0510(d)(2)(A-C) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An EpiPen were not returned to one child's parent after the medication had expired. .0803(12) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. The teachers did not maintained a copy of the children's incidents in the children's file. .0802 (e) 853 Incident logs were not completed and maintained as required. The teachers did not document the children's incidents on the incident log. .0802(g)(1-6) 1043 All staff records, except financial records, were not made available for review. Two staff file were not available for review. G.S. 110-91( 9) 1302 Individual applications were not on file for each child. There were no application on file for four children. 10A NCAC 09 .0801(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The teachers did not practiced shelter-in-place or lockdown drills every three months. .0604(u);.0302(d)(8) 1911 An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. One child received medical treatment and the teacher did not send a copy of the incident report to a Division representative within seven days. .0802(f) The above violations must be corrected immediately, and a letter must be emailed to me no later than January 21, 2026, stating how each item was corrected. If I do not receive your letter by January 21,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Consultation: I discussed with Ms. Walker the medication requirements and what information needed to be documented when a child’s medication needs to be administered. Medication needs to be returned to parents within three days of completion of the medication. I explained to Ms. Walker that she needs to go through all the children’s files to ensure all files have what they need. It is very important that staff know what health condition each child may have as well as if medication needs to administered. I reviewed the process of completing an incident report and log. I explained to Ms. Walker that if a child receives medical treatment from a physician, then she must submit an incident log to me within seven days. I also discussed with Dr. Hines and Ms. Walker that the one staff member need maltreatment , CPR and First Aid trainings by specific dates. Please see the staff/training worksheets for specific dates Rated License Assessment: We discussed with Dr. Hines and Ms. Walker the Rated license assessment process for the Hold Harmless is now over. The new QRIS has been adopted effective July 1,2025. I informed Dr. Hines and Ms. Walker that the NC Pré-K administration has chosen Pathway 1. We reviewed the new application process. I left a copy of the QRIS form that discussed the information for the Pathways to the Stars. WORKS: Please ensure the education information is in the W.O.R.K.S. database. We checked the W.O.R.K.S. database Ms. Lindsay needs to complete the education process in W.O.R.K.S. updated her education to I reviewed the instructions on the DCDEE website. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed to ensure your staff meets minimum education requirements. All indoor spaces used use by the children was observed today. I was unable to monitor the playground due to inclement weather. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit, I observed the children napping, waking up napping, playing with table toys and departing. Twenty-six (26) children were present during today’s visit. A sanitation inspection was conducted on January 3, 2025. This facility received a superior rating with eight (8) demerits. A fire inspection was conducted on January 3,2025. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The fence had a sharp metal bar exposed. Four children did not have their health care professional. .0802(c)(2) The above violations must be corrected immediately, and a letter must be email to me no later than January 31,2025 stating how each item was corrected. Upon my arrival, I was greeted by Dr. A. Hines and Ms. J. Wingate, the lead teachers. I informed them the purpose of today’s visit. Ms. Hines gave me the necessary paperwork I needed to complete today’s visit. Dr. A. Triplett, the school the principal was present during my visit. I observed and documented the attendance. Twenty-nine (29) children were present during today’s visit. They were observed listening to a story cleaning up, washing their hands in preparation for lunch, eating lunch using the bathroom and napping. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions; CPR and FA certification. One (1)violation was cited during today’s visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One of the lead teacher did not submit required forms to complete a five criminal background check before the expiration of the qualification letter. G.S. 110-90.2(b) & .2703(n)&(o) The above violation must be corrected immediately, and a letter must be email to me no later than October 7,2024 stating how the one item was corrected. I will also need a letter from Amy Pendergrass, Director of Early Learning stating this school is relocating to another space. This DPI NC Pre-K classroom currently operates with a Five-Star License, issued February 15, 2018, earning 7 points in Education, 7 points in Program, and 1 Quality Point, programmatic option met for enhanced policies, staff benefits package and parent involvement. This preschool program meets enhanced ratios and space and has reduced ratios by one child per age group. Upon my arrival, I was greeted by Ms. Jocelyn Wingate, lead teacher. There are two NC Pre-K classrooms on site. In the second classroom, the lead teacher is Dr. Andrea Hines and assistant teacher is Ms. Geraldine Curtis. There were 29 children present. The staff/child ratios were the following: 4’s/5’s – 2:15 (A. Hines and G. Curtis) 4’s/5’s – 2: 15 (J. Wingate and J. Bryant-substitute) In Space #303, I observed the children playing participating in a group activity. In Space #305, I observed the children completing bathroom routines and eating lunch. The following violation documented during the 4/29/24 visit was monitored for compliance during this visit: 1775: NC Pre-K program staff/child ratios and group sizes were not met. Today the staff/child ratio was in compliance in each classroom. During today's visit, violations of child care requirements were cited. Name of Operation: PECK ELEMENTARY NC PRE-K PROGRAM Facility ID: 41002134 Consultant: SHARON WILLIAMS Operation Type: Center Case Number: Visit Date: 4/29/2024 Number Present: 30 Completed Date: 4/29/2024 Age: From 4 To 5 Total Minutes: 180 Time In: 10:00 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's routine unannounced visit was to assess this facility to determine compliance with applicable child care requirements for a center and NC Pre-K requirements. This DPI NC Pre-K classroom currently operates with a Five-Star License, issued February 15, 2018, earning 7 points in Education, 7 points in Program, and 1 Quality Point, programmatic option met for enhanced policies, staff benefits package and parent involvement. This preschool program meets enhanced ratios and space and has reduced ratios by one child per age group. Limited monitoring of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. Upon my arrival, I was greeted by Ms. Jocelyn Wingate, lead teacher. There are two NC Pre-K classrooms on site. In the second classroom, the lead teacher is Dr. Andrea Hines and assistant teacher is Ms. Geraldine Curtis. Dr. Hines was absent today. There were 30 children present. The staff/child ratios were the following: 4’s/5’s – 2:15 (G. Curtis and B. Gillis-substitute) 4’s/5’s – 2: 15 (J. Wingate and B. Olson-substitute) In Space #303, I observed the children playing outside, completing bathroom routines, eating lunch, and resting on cots. In Space #305, I observed the children completing bathroom routines, eating lunch, and resting on cots. A sanitation inspection was conducted on March 22, 2024 and received a classification of Superior with 9 demerits. During today's visit, one violation of child care requirements was cited. The violation was corrected during the visit. Violation Number Comment Rule 1775 NC Pre-K program staff/child ratios and group sizes were not met. Upon my arrival, in Room 305 there was one teacher to 15 children ages 4 - 5 year old. Ms. Pettiford of Guilford County Schools NC Pre K Office was present but not counted in the staff/child ratio. Ms. Pettiford notified Principal Triplett who came to the classroom. Principal Triplett corrected the violation by placing a substitute in the classroom. The substitute meets the applicable requirements. .3009 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. During today's visit when I arrived one classroom was out of staff/ child ratio. I discussed with Principal Ashley Triplett and Ms. Lisa Pettiford of Guilford County Schools NC Pre K Office maintaining staff/child ratio. Ms. Triplett's plan going forward is for each classroom to notify her when staff/child ratio is not being met. Rated License Re-assessment Process Today I discussed with Ms. Pettiford that the facility is in Cohort One. A new school is being built next door and anticipate the classroom will be moved into the new building in December 2024. Ms. Pettiford stated that new required inspections will be completed prior to the classroom moving into the new building. They would like to have the ERS assessment in the Spring of 2025 prior to the end of the school year. A checklist was used to note the requirements I monitored today. This DPI NC Pre-K classroom currently operates with a Five-Star License, issued February 15, 2018, earning 7 points in Education, 7 points in Program, and 1 Quality Point, programmatic option met for enhanced policies, staff benefits package and parent involvement. This preschool program meets enhanced ratios and space and has reduced ratios by one child per age group. Upon my arrival, I was greeted by you, Dr. Andrea Hines, lead teacher. There are two NC Pre-K classrooms on site. In the second classroom, the lead teacher is Ms. Joycelyn Wingate. There were 29 children present. A full assessment of child care requirements was assessed. In Space #303, I observed the children participating in teacher led activities. In Space #305, I observed the children participating in a group activity completing bathroom routines and eating lunch. The sanitation inspection was completed on November 28, 2023 with a "Superior" classification with 11 Demerits. Today I reviewed six children's files. During today's visit, one violation of child care requirements was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The green climber structure on the outdoor playground did not have six feet of fall zone. 10A NCAC 09 .0601(a) The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before February 13, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date each violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. Please email the compliance letter to: sharon.y.williams@dhhs.nc.gov Rated License Reassessment Process The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year will not begin until July 1, 2024, the preparation year for cohort one begins July 1, 2023. Your facility is in cohort one. Throughout the next few months I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. • Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) • Determine whether you wish to have Environment Rating Scale (ERS) Assessments [ITERS-R, ECERS-R, SACERS-U] conducted • Request technical assistance with your child care consultant and local partners • Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) • Reach out to your local Community College to discuss educational opportunities • Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets.
