A checklist was used to note the requirements I monitored today. I also monitored health and safety requirements. This program currently operates with a Three Star License, issued 7/5/17 earning 5 points in Education, 2 points in Program a Quality point for serving no more than two children under age one. Upon my arrival, I was greeted by Ms. Patricia Maynard, owner. Three children were present. I observed the children finishing lunch, completing bathroom routines and resting on cots. A full assessment of child care requirements was monitored today. A satisfactory fire inspection was completed on April 17, 2024. To verify compliance with specific requirements, you were given a Verification of Required Information for Operator and Additional Caregivers form and the FCCH and Centers Located in a Residence Operator's Statement of Responsibility. I received the signed forms during today’s visit. Your signature on the forms will serve as verification that the information provided is accurate and complete. Today you stated that you do not transport the children enrolled in the family child care home and there are no other household members. During today's two visit violations of the child care requirements were cited. Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The up-to-date pet vaccinations for the Maltese was due on 6-17-23. A current copy was not available for review. .1719(b)(1) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The operator did not complete Recognizing and Responding to Child Maltreatment as part of the Health and Safety training every five years. It was last completed in 2018. .1703(d)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before May 27, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation numbers. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. To complete Recognizing & Responding to Suspicions of Child Maltreatment you may access the website at: https://preventchildabusenc-lms.org/courses/rr-update/ Rated License Review: Education Standards: Based on the information for review by the Education Workforce Unit you meet NCECC with 12 school hours in Early Childhood Education. You are eligible for 5 points in staff education. Program Standards: Meets Minimum Licensing Requirements; have approved operational policies and completed 3-month long self-study and self-assessment using the FCCERS-R; you are eligible for 2 points in program standards. Quality Point: You completed the Child Care Business Basics Training on 10/27/07 and have chosen this as your quality point. Breakdown of Rated License Points: Staff Education = 5 points Program Standards = 2 points Quality Point = 1 point TOTAL = 8 points = THREE STARS Based on the breakdown of the rated license points, you have earned a Three Star Rated License. A Three Star is recommended to be issued to this facility. Please keep in mind that the administration of the Division of Child Development and Early Education must approve this assessment and the points earned in each component prior to issuance of the license. The Three Star License will not be processed until the violations cited are corrected and I receive the compliance letter. Thank you for your cooperation during today’s visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
