The last annual compliance visit was conducted on 5/2/25. Upon arrival, I was greeted by Ms. Crawford. I stated the purpose of today’s visit and completed a walkthrough of the licensed spaces. There were 6 children present ages 0-3 years old. Children were observed playing with a variety of toys and taking a nap. Required items were posted, including emergency numbers and current activity plans. I monitored on-going training for you, Ms. Crawford, you completed Health and Safety trainings to meet the 5-year requirement. Your CPR/FA certifications are due for renewal by the end of the month. I monitored two children's files; C. Harris and O. Harris. I also monitored documented arrival and departure times, outdoor inspections, and fire and emergency drills. I verified compliance with lead in water, lead in paint and asbestos testing. There was 1 item of non-compliance observed during today’s visit and the corresponding violation was cited. Ms. Crawford called a parent to pick up their child and the child left during the visit, correcting the violation. Ms. Crawford requested an increase in capacity today, she meets the requirements to increase the capacity at her FCCH. Violation Number Comment Rule 102 Number of preschool children exceeded five children, including the provider's own preschool children. There were 6 children present today ages 0 - 3 years old. GS 110-91(7)(b) Corrective action is not required on your part at this time, since the violation was corrected during the visit. You must continue to maintain compliance with applicable child care requirements. Today, we discussed the Pathway to the Stars during the visit. We completed the discussion from and you have decided on Pathway 2. I advised you will need to start using a curriculum and I also left a copy of the Foundations book. During today’s visit I confirmed compliance with the requirements needed to increase your licensed capacity. On November 10, 2025, I received your formal request to increase the capacity of your family child care home from 8 children to 10 children. According to guidance received by the Greensboro Zoning Department, Per the standards of 30-8-10. 2,. Increasing the number of attendees within the requested parameter would not require a new letter from the City. During the visit today, I confirmed the following in compliance as required by Session Law 2025-36: 1. Rooms and areas within the family child care home where occupants receive care are on the same level of exit discharge. 2. Rooms and areas within the family child care home where occupants receive care are located on the same level with, and within a maximum of 40 feet travel distance to, at least one 2A:10B:C fire extinguisher. 3. The family child care home has and maintains a Fire Safety, Evacuation, and Lockdown Plan compliant with Section 404 of the North Carolina Fire Code. 4. The family child care home does not have any fuel fired appliances, fuel burning fireplace or an attached garage. A carbon monoxide alarm and detection system compliant with Section 915 of the North Carolina Building Code and Section R315 of the North Carolina Residential Code is not required. 5. The family child care home has smoke alarms compliant with Section R907 of the North Carolina Building Code and Section R314 of the North Carolina Residential Code. According to GS 110-91(7)(b) A family child care home is allowed to provide care for one of the following groups of children, including the operator's own preschool-age children and excluding the operator's own school-age children up to 13 years of age: • A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. • A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. • A maximum of 10 children if all children are older than 24 months of age. Based on the information verified today, I will submit a new permit for your licensed capacity to be increased. Your license will be effective today and valid for 10 children total and an age range of 0-12 years. This will allow you flexibility in which capacity option you are meeting based on the ages of the children you are serving at any time. You must meet one of the capacity items listed above at all times. Thank you for your hard work on this change for family child care homes in Guilford County. Congratulations! The last annual compliance visit was conducted 11/26/24. Upon arrival, I was greeted by Ms. P. Crawford, Owner/Operator. I stated the purpose of my visit and completed a walkthrough of the licensed spaces. There were five child present ages 3 months – 5 years old. Children were observed napping or resting quietly. An infant and toddler were observed drinking bottles and receiving care from the provider. Interactions observed were positive and caring in nature. Required items were posted, including, emergency numbers and activity plans. I monitored outdoor inspections and emergency drills. I monitored two new children's files. I also verified a negative lead in water test and your homes exemption from asbestos and lead paint. Ms. Crawford stated there were no incident logs on file. I monitored the following child care requirements today; Supervision, • Capacity, • CPR / First Aid , • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation There was one item of non-compliance observed during today’s visit, and the corresponding violation was cited. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A cleaner (CLR) and Bath Epsom Salt were in the bathroom on a low, open shelf int he reach of children. .1719 (a)(7) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. There was no sleep check documentation on file for the enrolled infant K. Steele. .1724(a)(8)&(f) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by June 19, 2025, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. The letter can be emailed to cherissa.alston@dhhs.nc.gov. During today's visit we discussed the following items: 10A NCAC 09 .1724 SAFE SLEEP PRACTICES Each operator licensed to care for infants aged 12 months or younger shall develop, adopt, and comply with a written safe sleep policy that: (1) specifies that the operator shall place infants aged 12 months or younger on their backs for sleeping, unless: for an infant aged six months or less, the operator receives a written waiver of this requirement from a health care professional; or for an infant older than six months, the operator receives a written waiver of this requirement from a health care professional, or a parent or a legal guardian; (7) specifies the operator shall visually check sleeping infants aged 12 months or younger at least every 15 minutes; (8) specifies how the operator shall document compliance with visually checking on sleeping infants aged 12 months or younger; There was an alternative sleep waiver in the infant's file, however, I observed the infant being placed on her back while in the pack and play. I reminded you that a doctor's note is required for alternative sleep positions for children under 6 months of age. I also reminded you that, based on your safe sleep policy, infants should be visually checked every 15 minutes while sleeping and the checks should be documented. The documentation should be kept for at least 30 days. ABCMS Provider Portal The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Additional information and instructions are available in the ABCMS Provider Portal Guide. Once the training has been completed and you have been notified that access has been granted, log into the provider portal to locate the unique provider code for the facility. Applicants and employees will use this code to create a connecting application to be associated with your facility. You must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
