A checklist was used to note the requirements I monitored today. This facility currently operates with a three-star license issued 1/10/2019. This facility has a 95% compliance score as of 7/6/26. The last annual compliance visit was on 7/28/25. As of 7/6/26 the facility was current and active according to the Secretary of State website. The last sanitation visit was 12/11/25 and resulted in a Superior rating with six (6) demerits. The facility tested negative for lead in water 9/25/25, according to the sanitation report and is exempt for lead in paint and asbestos according to the Clean Classrooms website. Upon my arrival, I was greeted by Brenda Verzeni I informed Ms. Verzeni of the purpose of today’s visit. Ms. Verzeni gave me the necessary paperwork I needed to complete today’s visit. The last fire drill was conducted 6/26/26. The last emergency drill(Shelter-in-place) was conducted on 6/4/26. The last playground inspection was 6/5/26. The last fire inspection was 12/3/25. Eighty-six children were present during today’s visit. I observed the indoor and outdoor spaces. I observed both staff/child ratio and supervision and found both to be in compliance. During today’s visit, I observed children playing outside, playing in centers and completing table -top activities. All medication and hazardous items were out of the children's reach. The paperwork for medication was complete. Staff and children’s files were monitored during today’s visit. Ten (10) children files were reviewed. The staff and training worksheets were reviewed during today’s visit. Your signature on these forms will serve as verification of the information provided is accurate and complete. There were three violations cited during today’s visit. Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff member did not have a completed orientation available for review. .1101(a) 1757 A valid qualification letter was not on file and available to review at the facility. One staff member had an expired CBC. It expired 7/4/26. The staff member experienced difficulty logging into the ABCMS portal, but had fingerprints taken 7/1/26. G.S. 110-90.2(b) & (d) & .2703(e) To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by 07/22/26. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected and information regarding how the violation was corrected. I provided technical assistance regarding: Staff Training: EQUiPD The EQUiPD website has many webinar trainings that can be completed online for no cost and have been approved by NCDCDEE. The newly hired staff will need trainings as appropriate. Within 90 days of hire staff need to take Recognizing and Responding to Suspicions of Child Maltreatment and CPR/First Aid. Newly hired staff need health and safety training within the first year of employment. There are a number of staff who will need to redo their 5 year health and safety trainings by the end of 2027. Criminal background checks (CBC) are completed to protect the children from abuse and reduce liability risks. The CBC process must be completed before a person can begin work; therefore, a Qualification Letter must be on file prior to employment. Any employee in a licensed childcare facility who is ever left alone with a child or group of children for any length of time must have a background check completed through the Division of Child Development and Early Education (DCDEE). A CBC must be completed every five years. An expiration date is listed on each Qualification Letter. Submissions can be made six months before the expiration date. Operators will need to be proactive and make staffing decisions and arrangements in advance to avoid non-compliance with this and other DCDEE staffing requirements. Having applicants submit a copy of their Qualification Letter along with their resume and application can facilitate a shorter selection and hiring process. Criminal background checks must be completed online. To complete online criminal background check applications, go to www.ncchildcare.nc.gov/general/dhhscrc.asp. The $26.50 payment is a part of the online criminal background check application process. Fingerprints must be completed through LIVESCAN at your local county sheriff’s office. NC WORKS: make sure all staff have updated information in NC WORKS showing their most recent education. Application • The application needs to be submitted. • The facility is considering applying for Pathway #2. • The facility uses their own curriculum and assessment tool. Family and Community Engagement Standards • For a three star, the facility must meet 2 choices, from separate categories. Facility CQI plan • The facility needs to submit a Facility CQI plan. • Individual CQI plans are being maintained in staff folders. Staff Education • The facility has been experiencing difficulty getting people logged into the WORKS system. We reviewed the process. • I reminded Ms, Vernazi, Ms. Spires and Mr. and Ms. Kohler that the facility would not be able to receive a higher number of strs than what they receive in Education. Upon my arrival, I was greeted by Ms. K Johnson, the director. I informed Ms. Johnson the purpose of today’s visit. Ms. Johnson a gave me the necessary paperwork I needed to complete today’s visit. Ms. B. Verzeni, the floater, accompanied me while I observed and documented the attendance. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Seventy-six(76) children were present during today’s visit. They were observed being diapered, listening to a story, fixing puzzles, making a caterpillar. playing in centers, reviewing letters and numbers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch and napping. A sanitation inspection was conducted on June 17,2025. This facility received a superior rating with four (4) demerits. A fire inspection was conducted on December 6, 2024. A copy of the fire inspection will be mailed to Raleigh with this documentation. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as July 28,2025. I monitored a fifteen passenger bus this facility uses to transport the children to and from school and field trips. The insurance policy was current for the vehicle. The vehicle met the transportation requirements and was in compliance during today's visit. Six (6) violations were cited during today’s visit. Violation Number Comment Rule 322 Prior to children participating in aquatic activities, the center did not develop policies that included required information. The school participated in an aquatics activity without developing an aquatic policy. .1403(g)(1-5) 476 Staff did not use the list of participating children to check attendance when leaving the center, periodically when the children were involved in the activity, before leaving the activity to return to the center, and/or upon return to the center. There was no documentation available at the center that a list of the children participating in the off premise activity as left at the center before the children left for off premise activity. .1005(b)(6) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. There were foam blocks in a classroom with two year old children present. .0604(q) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Emergency and identifying information, including the child's photograph was not in the vehicle for fifteen children being transported. 10A NCAC 09 .1003(d) 1916 Written permission was not given by parent for child to participate in aquatic activity. There was no documentation on file that fifteen parent's gave permission for their child to participate in an aquatic activity. .1403(i) The above violations must be corrected immediately, and a letter must be emailed to me with your signature no later than August 11,2025 stating how each item was corrected. Technical Assistance: Aquatics Activities: Today I discussed with Ms. Johnson that if the facility a participates in an aquatic activity, then the facility will need to develop an aquatic policy. I reviewed the requirements. I will send you a copy of the requirements you will need in your aquatics policy. Transportation: Today I also discussed with Ms. Johnson the transportation requirements you must meet when you transport children on field trips and off premises activities. The following requirements must be met and will be monitored: The driver must be: (1) 21 years old (2) have a valid driver's license of the type required under North Carolina Motor Vehicle Law for the vehicle being driven or comparable license from the state in which the driver resides; and (3) have no convictions of Driving While Impaired (DWI) or any other impaired driving offense within the previous three years. (4) have emergency information in the vehicles (5) have current insurance policy for the vehicles These requirements must be met by all drivers, and it is up to you to verify all these requirements have been met. SAFETY PROCEDURES (10A NCAC 09 .1003) (1) A first aid kit must be located in each vehicle used on a regular basis to transport children. The first aid kit shall be firmly mounted or otherwise secured if kept in the passenger compartment. (2) For each child being transported emergency and identifying information must be in the vehicle. (3) Children shall not be left in a vehicle unattended by an adult. (4) Before children are transported, written permission from a parent shall be obtained which shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. (5) When children are transported, staff in each vehicle shall have a functioning cellular telephone or other functioning two-way voice communication device with them for use in an emergency. SEAT RESTRAINTS (10A NCAC 09.1001) These requirements are required if you transport children in vehicles with seat belts. (1) Each adult and child shall be restrained with an individual seat belt or appropriate child restraint device when the vehicle is in motion. (2) Only one person shall occupy each seat belt or child restraint device. **You must follow all state and local driving laws. --North Carolina state law requires that all children who are not over age 8 or do not weigh at least 80lbs. must be restrained in a booster or car seat appropriate for the child's weight and height. For more information on how to meet NC child restraint requirements please visit www.buckleupnc.org Staff/Ratio: (section.1004): (1) When children are being transported, the same staff-child ratios required for a child care program applies. Ratios for the youngest child in the vehicle must be maintained at all times. Field Trip Permission form: During today's visit, I reviewed your current field permission form. You can no longer use a blanket permission form for all transportation activities. A permission slip can be obtained by the parents daily, weekly or per each activity type. For example, if you are going to the movies every Wednesday, then have the dates the facility will be going to the movies on one field trip permission form. The following requirements must be included on each permission slip: 1. Written permission from each child's parent to transport. 2. Parents notified when and where child is to be transported; and expected time of departure and arrival, and the transportation provider. Additional requirements that must be met by staff during all field trips are: (1). Staff in each vehicle must have a functioning cellular phone or other function communication device with them to use for an emergency. (2) Staff must have a list of the children being transported. Staff members shall use this list to check attendance as children board and depart from the vehicle. (3)You must post a schedule of off premise activities in each participating classrooms, a copy must be given to parents. (4)A list of all children participating in field trips must be left at the center. (5) Travel Log for field trips must be maintained for at least one year (6) There must be a staff member present that has current safety training (CPR/First Aid). Rated License Assessment: I discussed it with Ms. Johnson, the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. Beginning on February 1, 2025, all child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. ABCMS Provider Portal North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. A checklist was used to note the requirements I monitored today. This facility currently operates with Three Star license issued on January 10,2019. Upon my arrival, I was greeted by Ms. B. Verzeni, the floater. Ms. Verzeni informed me that Ms. K Johnson, the director was gone and would be back shortly. I informed Ms. Verzeni the purpose of today’s visit. Ms. Johnson arrived shortly after my arrival. Ms. Johnson gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Eighty-one (81) children were present during today’s visit. They were observed being diapered, fixing puzzles, making a ladybug, playing in centers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch, napping waking up from napping, arriving from an off premise activity, washing their hands in preparation for snack and departing. A sanitation inspection was conducted on April 30,2024. This facility received a superior rating with three (3) demerits. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as August 1,2024. I monitored a fifteen passenger bus this facility uses to transport the children to and from school and field trips. The insurance policy was current for the vehicle. The vehicle met the transportation requirements and was in compliance during today's visit. Four (4) violations were cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There were tree roots which are tripping hazards a and sharp rocks that safety hazards on the playground where the children play. 10A NCAC 09 .0601(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not successfully complete certification in First Aid appropriate to the age of children in care. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not successfully complete certification in CPR training appropriate to the age of the children in care. .1102(d) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with one new staff prior to providing care with children. .0608(d)(1-4) The above violations must be corrected immediately, and a letter must be emailed to me with your signature no later than August 15,2024 stating how each item was corrected. My email address is at wytina.blackmon@dhhs.nc.gov If I do not receive your letter by August 15,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Johnson the staff members need in-service training hours. It-SIDS, health safety trainings, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. There are several children’s Emergency medications, topical creams, and permissions n to admission form and medical action plans and expirations of medications are due soon. Please have each teacher to check their forms. Rated License Assessment: I discussed with Ms. Johnson the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. Ms. Johnson informed me that she would like to increase in stars and have the environmental rating scales conducted. I explained to Ms. Johnson that the Cohorts no longer exist therefore there are no more prep assessments. This facility can request an assessment at any time. My recommendations are to have each teacher to look at their schedules and the activities they are providing the children and visit the NCRLAP.org to see the videos and resources for the rated license assessment. Education Standards: Today we discussed the education of the current staff. I informed Ms. Johnson that based on the education on the current license the staff meet four points. I informed Ms. Johnson can send me a list of the staff and I can check their education in our workforce database. This will be a guide to where to start with education of staff then this will determine what you will need to score on the rated license assessments. WORKS: Please ensure that all new staff member’s education information is current in the W.O.R.K.S. database. Each staff member must register in the WORKS system as soon as they are hired at this location. I email you the instructions for you to send to all your employees. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment before the end of the temporary time period. You may also want to consider having staff members complete the Early Educator Certification process to assist in getting higher education evaluations completed if their degree is not in early childhood education. Early Educator information is available at www.ncicdp.org. Technical Assistance: Today I found out once I arrived at the center of a situation that happened to the facility over the weekend. This event effected the daily operation of the facility. I explained with one of the owners Ms. C Kohler and Ms. Johnson about the recent situation that happen at the facility in the future they will need to inform me of any incident that effects the daily operation of the facility. This will allow me to be aware of situation in the event there is a problem with parents who may complain. You will need to have the fire inspector to complete the fire inspect from once you get the system corrected. Also, today there were children participating in an off premise activity at another facility. The facility did have permission for the children to go to the location where the summer camp was located. Ms. Johnson informed me that she thought she could have an unlicensed summer camp if it was less than four months. I explained to Ms. Johnson that a facility can have an unlicensed with school age children summer camp at another site but the children that come to the licensed space is enrolled at the licensed space then the staff and the children files must follow the licensed facility records. You will need to follow the field trip guidelines for the children who are enrolled at this facility. These forms that you will need to have available for review at the facility. Please send me a copy of your field trip schedule. Thank you for your time during today's visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
