Name of Operation: MYRA'S LEARNING KIDS ACADEMY, LLC Facility ID: 41001152 Consultant: ELIZABETH CHILDRESS Operation Type: Center Case Number: Visit Date: 1/7/2026 Number Present: 15 Completed Date: 1/7/2026 Age: From 0 To 5 Total Minutes: 195 Time In: 09:30 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. I completed an Annual Compliance Monitoring Checklist for Centers during today’s visit, and a completed copy was emailed to you. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 12/31/2025. The NC Secretary of State website was reviewed on 12/31/2025 and Myra’s Learning Kids Academy, LLC was listed as current- active. Upon arrival, I was greeted by Crystal Cunningham. There were fifteen children present today. Children were observed engaged in group times and outdoor activities. One infant was sleeping and toddlers were engaged in interactions with the teacher. Children were being attended to according to their individual needs. The staff were observed engaged in activities with the children and were positive in their interactions with the children. Your program currently operates with a four-star license, issued 11/6/2018. The last Annual Compliance visit was conducted 1/23/25. The sanitation inspection was completed 11/20/2025 with a “Superior” classification. The last fire inspection was conducted 7/16/2025 and your facility was approved for daytime care only. Required items were posted and current. The outdoor space was monitored, and two hazards were observed today. Outdoor inspections were documented monthly. Four children's records and three staff member's file were monitored. Required program records were also monitored during today’s visit. There were two items of non-compliance observed today, one items were corrected during the visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Children had access to the cord of a bottle warmer. The teacher corrected the violation during the visit. Areas at the back corners of both playgrounds had build-up of leaves and will need to be cleared and maintained regularly. 10A NCAC 09 .0601(a) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. Sections of the fence on both play areas have settled and is no longer measuring the required four foot in height. GS 110-91(6); .0605((i) Violations documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 01/21/2026, describing how and when the violations were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. During the visit, I provided technical assistance regarding: The outdoor learning environment offers a sense of freedom for children. Since children are exploring and taking risks it is very important to provide a safe environment. Proper maintenance is a key factor when trying to ensure a safe play environment for children. Over time, sections of the fence on both play areas have settled and is no longer measuring the required four foot in height. A section of the fence on the preschool side is also pushed in and broken and needs repair. The build-up of leaves creates a potential hazard, as they can conceal animals, snakes, insects, and uneven ground. For the safety of the children, the areas at the back corners of both playgrounds will need to be cleared and maintained regularly. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. The cord to a bottle warmer was exposed and children had access to the cord. The teacher corrected the violation during the visit. We also discussed QRIS Modernization Pathways: The Pathway Discussion Template was discussed and completed. Your last rated license assessment was completed 4/10/2018, you will need to request an assessment on or before September 2026. I reminded you that an additional visit could be conducted to discuss any needed information as you transition through the process. You were provided with an opportunity to ask questions regarding the implementation of Child Care Rules.
