A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star license issued on December 12,2019. Upon my arrival, I was greeted by Ms. S. Robeson, program coordinator. I informed Ms. Robeson the purpose of today’s visit. Ms. Robeson gave me the necessary paperwork I needed to complete today’s visit. Sixty-five (65) children were present during today’s visit. During today’s visit, I observed the children arriving, playing in centers, washing their hands in preparation for snack, eating snack, playing in centers and departing. All indoor spaces used by the children were observed today. I was unable to monitor the playground due to weather. All required items were posted. All medication and hazardous items were out of the children's reach. A sanitation inspection was conducted on October 21,2024. This facility received a superior rating with eight(8)demerits. A fire inspection was conducted on April 25,2024. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Leftover medicines were not returned to the three parent after the medication expired. .0803(12) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan was not reviewed with all staff annually. 10A NCAC 09 .0802(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The trained staff did not review the EPR Plan annually. .0607(e) The above violations must be corrected immediately, and a letter must be sent to me no later than March 13,2025 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov If I do not receive your letter by March 13,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Robeson, several staff members, CPR/First Aid and criminal record rechecks, will expire on specific dates. You have two new employees that need maltreatment by 3-13-25 and 5-3-25. Please refer to the staff/training worksheet for the expiration dates. During today’s visit, there were several medications that had expired. These medications have not been returned to the parents. You must return medications to the parents within 72 hours of the authorization expiration date or when the medication expires. There are several children’s permissions to administer medication forms and medical action plans and expirations of medications are due by specific dates. Please have each group leader check their forms. Rated License Assessment: I also discussed with Ms. Robeson the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. I explained to Ms. Robeson that there is a new process that is still being developed. You can find information about the three pathways at https://ncchildcare.ncdhhs.gov under the what’s new tab. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
